Abstract

Surgery is an invasive medical procedure performed to restore the anatomy and physiology of impaired body organs. One type of surgery is fracture surgery, which aims to restore the anatomy and physiology of bones that are broken for various reasons. The purpose of this study was to provide nursing care to a post-operative tibial fracture patient with a physical mobility disorder in the Flamboyan Room of Dr Harjono Ponorogo Hospital. This study used a descriptive design with a case study approach involving a participant who met the criteria. Data collection techniques included interviews and observation of muscle strength and range of motion (ROM) values. The results of the assessment found that the patient had grade 2 muscle strength in the left extremity, and the ROM value was reduced. The nursing diagnosis was Physical Mobility Disorder. Nursing interventions according to SIKI included early mobilisation and early ambulation for seven days. The evaluation results showed that muscle strength increased from grade 2 to grade 5 by the sixth day, and the ROM value improved to reach the average normal limit by the sixth day. ROM exercises and the use of crutches were effective in improving physical mobility. ROM exercises are effective for improving physical mobility because they can reduce pain levels, thus decreasing concerns related to mobility.

Abstract

Latar Belakang: Stroke merupakan penyakit serebrovaskular ditandai dengan adanya kematian jaringan otak akibat kekurangan suplai darah dan oksigen, sehingga dapat menimbulkan manifestasi klinis yaitu kesulitan berbahasa. Afasia merupakan gangguan berbahasa yang terjadi akibat adanya kerusakan otak di bagian kiri,  hal ini menyebabkan penderita sulit berbicara, membaca, dan menulis. Tujuan: Penelitian ini bertujuan untuk menganalisa asuhan keperawatan pada pasien pasca stroke dengan gangguan komunikasi verbal. Metode: Penelitian ini dirancang dengan metode deskriptif kualitatif berbentuk studi kasus. Asuhan keperawatan diberikan kepada Ny.S selama 7 hari. Teknik pengumpulan data diperoleh dari hasil wawancara, observasi, dan pemeriksaan fisik. Hasil: Hasil penelitian ini menunjukkan pasien mengalami afasia dibuktikan hasil skrining FAST ditemukan skor 16 pada usia 56 tahun, serta hasil komunikasi verbal pasien mengalami peningkatan dengan pemberian terapi wicara AIUEO selama 7x24 jam dengan penilaian DFCS didapatkan pada hari pertama total skor 12 dan mengalami peningkatan pada hari keenam hingga hari ketujuh dengan total skor 18. Hal ini menunjukkan terdapat pengaruh pemberian terapi wicara AIUEO dengan penilaian skala komunikasi fungsional derby (DFCS) pada pasien pasca stroke yang mengalami gangguan komunikasi verbal di wilayah Puskesmas Jenangan Ponorogo. Kesimpulan: Terapi wicara AIUEO dengan penilaian DFCS terbukti efektif dilakukan untuk meningkatkan kemampuan komunikasi verbal pada klien pasca stroke yang ditunjukkan adanya peningkatan dari afasia sedang menjadi afais ringan pada hari ke-6 hingga ke-7 setelah pemberian terapi wicara AIUEO berdasarkan penilaian skala komunikasi fungsional derby (DFCS). Sehingga asuhan keperawatan pada pasien pasca stroke dengan gangguan komunikasi verbal di wilayah puskesmas jenangan ponorogo dapat teratasi sebagian.

Abstract

The prevalence of Diabetes Mellitus (DM) globally continues to increase until it triples in 2030, namely 21.3 million (Perkeni, 2019). Physical, psychological, economic and social changes occur due to various complications. Diabetes mellitus, which cannot be completely cured, often results in anxiety and decreases quality of life. The role of patients and families in managing DM is vital because DM is a chronic disease that patients will carry for life (Perkeni, 2019). The research aims to identify family support and the level of anxiety among patients with type 2 diabetes mellitus at the Internal Medicine Clinic, Dr. RSUD. Harjono Ponorogo. This research design used cross-sectional research with the Zung Self-Rating Anxiety Scale and Hensarling's Diabetes Family Support Scale). Data analysis used statistical tests with Spearman's Rink through the SPSS program with α of 5%. The results of the Spearman Rink test obtained a value (p-value = 0.027 ˂ 0.05) indicating that there is a significant relationship between family support and anxiety in patients with diabetes mellitus. Based on the results of the analysis, it is known that family support has a significant influence on the level of anxiety among patients with type 2 diabetes mellitus

Abstract

Background: Diabetes mellitus remains a major public health problem in Indonesia due to its high prevalence and risk of severe complications. Health volunteers play a strategic role in primary health care, particularly in early detection and community-based prevention efforts. Strengthening cadres’ skills in blood glucose measurement is essential to support effective screening and health promotion activities. This community service aims to enhance the practical skills of health cadres in performing accurate blood glucose measurements to improve early detection and prevention of diabetes in the community. Methods: The Simanis cadre team from Kutuwetan Village, consisting of 10 members, participated in this community service activity. This community service activity employed a participatory approach through training and direct mentoring. The program involved health volunteers who received theoretical instruction and hands-on practice in blood glucose measurement. Cadre skills were evaluated using pre- and post-training assessments.  Results: The program involved ten health volunteers. The proportion of cadres with good blood glucose measurement skills increased from 83.85% before the intervention to 100% after mentoring, indicating a measurable improvement in cadre competency.  Conclusion: Mentoring-based training effectively improved health volunteers’ skills in blood glucose measurement. It is recommended that cadres apply these skills routinely in community health activities, with periodic re-evaluation every 3–6 months, and that further training be provided on fasting blood glucose and oral glucose tolerance testing to enhance early diabetes detection.

Abstract

Integrated community-based screening for Tuberculosis (TB) and Diabetes Mellitus (DM) is an essential strategy to enhance early detection of communicable and non-communicable diseases. This community engagement activity was conducted in July 2024 in Kadipaten Village, Ponorogo, using four screening approaches: village hall service, Posbindu, elderly health screening, and door-to-door. A total of 265 participants underwent TB screening and 166 underwent DM screening. Results showed 16 suspected TB cases (6%) and 15 individuals (9%) with elevated blood glucose, including four new DM cases (2.4%). The door-to-door method yielded the highest detection in both TB and DM screenings. The findings support existing evidence that active case finding improves detection coverage among at-risk populations. This integrated screening emphasizes the need for structured referral pathways, community health education, cross-program collaboration, and integrated health information systems to ensure sustainable TB–DM detection programs.  ABSTRAK Kegiatan skrining terpadu Tuberkulosis dan Diabetes Mellitus (skrining TB-DM) berbasis komunitas merupakan salah satu strategi penting dalam meningkatkan deteksi dini penyakit menular dan tidak menular, khususnya di wilayah dengan akses terbatas terhadap layanan kesehatan. Pengabdian masyarakat ini dilaksanakan pada Juli 2024 di Kelurahan Kadipaten, Kecamatan Babadan, Kabupaten Ponorogo, melalui empat pendekatan, yaitu Balai Kelurahan, Posbindu, kegiatan lansia, dan door-to-door. Sebanyak 265 peserta mengikuti skrining TBC dan 166 peserta mengikuti skrining DM. Hasil menunjukkan 16 peserta (6%) terduga TBC dan 15 peserta (9%) dengan gula darah tinggi, termasuk 4 kasus DM baru (2,4%). Pendekatan door-to-door memberikan kontribusi temuan terbanyak baik pada TBC maupun DM baru. Temuan ini sejalan dengan bukti penelitian bahwa active case finding meningkatkan cakupan deteksi pada komunitas berisiko. Skrining terpadu ini menegaskan perlunya jalur rujukan konfirmasi, edukasi kesehatan masyarakat, kolaborasi lintas program, dan integrasi sistem informasi kesehatan untuk keberlanjutan program deteksi dini TB–DM.
