Abstract

Human error can be prevented by measuring human reliability through the Human Reliability Assessment approach. PT A is a national scale salt producer that produces salt. The quality division identifies human error problems made by operators in the salt refining process. This study aims to identify, calculate probability values, and provide recommendations for improvement to reduce human error in salt refining production operators. HEART and SHERPA are research approaches used because they are considered the most suitable for this study. The results showed that the most visible human error for the error mode category was C2, incomplete checking, and 42 error modes because the operator infrequently did a complete check. The process with the highest Human Error Probability value is the drying process 1, with a value of 7.49. In contrast, the process with the most minor Human Error Probability value score of 0.085 is the bagging process 2. Human error prevention efforts are carried out for each process based on the highest score on the Human Error Probability Index, including Personal Protective Equipment, Standard Operating Procedures, and training to improve operator skills.

Abstract

Pertumbuhan konsumen peminum kopi berdampak positif pada peningkatan permintaan kopi bubuk. Produsen kopi giling mengalami pertumbuhan signifikan setelah masa pandemi. Kopi Bubuk Eyang Kakung merupakan produsen penggilingan kopi UKM di wilayah Gresik. Pemilik usaha saat ini berupaya melakukan evaluasi mutu pada di sepanjang proses penggilingan kopi. Tujuan dari penelitian ini yaitu mengidentifikasi,menghitung, menganalisis, dan membantu perbaikan pada kualitas produksi menurun yang disebabkan oleh defect produk Hasil dari penelitian ini menggunakan statisti process control atau biasa disebut seventools dengan jumlah produksi 1.305.000 gram otal kecacatan 67.050 gram dengan dua jenis kecacatan yaitu butiran kasar 50.500 gram,jumlah berkurang 16550 gram dengan proposi cacat tantara 0,049 sampai dengan 0,050 Diketahui faktor utama penyebab defect pada penelitian ini dipengaruhi oleh mesin yang sudah tua dan perlu adaya perawatan mesin yang rutin dan ekstra.

Abstract

Abstrak: Mitigasi Risiko Rantai Pasokan Pemeliharaan Ikan Hias Koi Menggunakan Metode House of Risk. Ketidakpastian adalah suatu kejadian dalam pengelolaan rantai pasokan suatu usaha. Usaha pemeliharaan Ikan Hias Koi juga tidak terlepas dari risiko sepanjang rantai pasokan usaha. Risiko ikan mati disebabkan oleh ketidakpastian pengiriman pakan dan keterlambatan logistik ke pelanggan memerlukan penanganan lebih lanjut. Penelitian ini bertujuan untuk mengidentifikasi, menghitung, dan menganalisis potensi dan penyebab risiko mulai dari hulu sampai hilir rantai pasokan pemeliharaan Ikan Koi sehingga didapatkan sekenario terbaik dalam meminimalisir risiko. House of Risk merupakan pendekatan yang digunakan dalam penelitian ini. Penelitian dimulai dengan identifikasi risk event dan risk agent berbasis Supply Chain Operation Refference, selanjutnya menghitung tingkat severity dan skenario mitigasi. Hasil penelitian ini yaitu teridentifikasi 53 Kejadian risiko dan 33 penyebab risiko dengan 10 prioritas perencanaan guna meminimalisir risiko sepanjang aliran ranti pasok. Kata Kunci : Ikan Koi, HOR, Mitigasi, Rantai Pasok, Risiko Abstract: Risk Mitigation of Koi Ornamental Fish Maintenance Supply Chain Using House of Risk Method. Uncertainty is considerend as challenges as well as opportunities in managing bussiness supply chain. A bussiness has been mitigating a way in order to eliminate such risk through supply chain. As a medium-sized bussiness owner in aquaculture, Mr. Choiruddin have been facing a number of bussinees risks, namely uncertainty of feed delivery and delays to deliver gold fish to customers. This study aims to identify, estimate, and analyze the potential and causes of risk from upstream to downstream of gold fish supply chain in order to figure out the best scenario in minimizing the risk. House of Risk is the approach used in this study. The research began with the identification of risk events and risk agents based on Supply Chain Operation Reference, then calculated the severity level and mitigation scenarios. The results of this study able to identify 53 risk events and 33 risk causes with 10 planning priorities to minimize risk through streams of supply chain. Abstract

PT. AAA is a chemical-based producer with zero defect program in term of maintaining product quality. However, there were found waste which were higher than based line quality acceptance through production process line of purified gypsum. This research aims to improve production process effectiveness in order to sustaining zero defect using Lean Six Sigma method. Define, measure, analyze, improve, and control, known as DMAIC, is used as research approach. The research begun by describing current state map to identify waste that occurs along the value stream. Followed by identifying activities based on three classifications, namely value added, non-value added, and necessary but non-value-added activities. Once such activities known, cost of poor quality and Sigma level is measured. Fish-bone diagram as well as failure mode and effect analysis are used in order to analyze risk priority number. This study found three wastes namely defect, waiting, and inventory as main focus to eliminate then improve effectiveness along production process.

Abstract

Abstrak: Evaluasi Risiko Ergonomi Pada Operator Pengamplasan Kayu Berbasis Metode Rapid Entire Body Assessment. UKM Harjo Jaya merupakan suatu usaha yang masih mengandalkan tenaga manusia sebagai kebutuhan utama dalam kegiatan produksi, usaha ini bergerak dibidang furniture untuk memenuhi kebutuhan rumah tangga. Hasil kuisioner NBM Nordic body map menunjukan bahwa operator mengalami beberapa keluhan rasa sakit dibagian tubuhnya diantaranya punggung, leher, pinggang, lutut, dan kaki. Tujuan penelitian ini mengevaluasi postur kerja operator pengamplasan dan merancang alat bantu kerja guna meningkatkan produktivitas. Rapid Entire Body Assessment (REBA) adalah digunakan untuk mengalisis risiko sebagai sistem analisis postural yang sensitif terhadap risiko musculoskeletal disorder. Hasil penelitian ini ditemukan bahwa setelah dilakukan perhitungan REBA pekerja pertama memiliki skor 6 yang masuk kedalam golongan sedang dan pada pekerja kedua memiliki skor 10 yang masuk kedalam golongan tinggi dan segera dilakukan perbaikan postur kerja saat melakukan pengamplasan.dilakukan perbaikan postur kerja saat melakukan pengamplasan. Usulan desain yang telah dibuat dengan ukuran antropometri Indonesia ukuranya sebagai berikut: tinggi sandaran kursi adalah 50,5, lebar sandaran punggung kursi adalah 46,6, lebar alas dudukan kursi adalah 34,5, tinggi kaki kursi adalah 42,65, dan panjang alas 36,75. Abstract: Ergonomic Risk Evaluation Of Wood Sanding Operators Based On Rapid Entire Body Asessment Method. UKM Harjo Jaya is a business that still relies on human labor as the main need in production activities, this business is engaged in furniture to meet household needs. The results of the Nordic body map questionnaire showed that the operator experienced several complaints of pain in his body including the back, neck, waist, knees, and legs. The purpose of this study is to evaluate problems and reduce pain in the operator's body by designing tools that aim to ease the burden on operators and increase productivity. Rapid Entire Body Assessment is used to analyze risk as a postural analysis system that is sensitive to the risk of musculoskeletal disorder. The results of this study found that after the calculation of REBA workers 1 have a score of 6 which falls into the moderate group and in workers 2 have a score of 10 which falls into the high group and immediately improved work posture when sanding. Abstract

Abstrak: Deteksi Defect Produk As Hidrolis Berbasis pendekatan Failure Mode and Efefct Analysis. Pertumbuhan kendaran niaga jenis truck menimbulkan inovasi adanya penambahan sistim hidrolis. UD. RZ merupakan pemasok komponen as hidrolis dump tuck. Manajemen UD. RZ saat ini berupaya menemukan solusi menyeluruh guna mengurangi defect pada proses produksi as hidrolis. penelitian ini bertujuan untuk mengevaluasi kegagalan di sepanjang proses produksi as hidrolis menggunakan pendekatan Failure Mode and Effect Analysis. Penelitian ini diawali dengan pemetaan alur proses produksi guna untuk mengidentifikasi produk defect, penentuan skala occurence, severity, dan detection oleh karyawan dan pemilik usaha, perhitungan perioritas risiko, dan diakhiri dengan pemetaan sebab akibat timbulnyadefect berbasis 5M.. Hasil penelitian menujukan dua defect yang teridentifikasi yaitu kesalahan ukuran dengan nilai Risk Priority Number 224, kesalahan titik pengeboran dengan nilai Risk Priority Number 147. Faktor penyebab kegagalan yaitu pekerjan kurang teliti dan dan tidak adanya alat bantu titik pengeboran. Adapun solusi yang bisa dilakukan dengan pembubutan ulang jika dimungkinkan, meningkatkan ketelitian pekerja, menamba SOP pekerja, dan adanya alat bantu titik pengeboran solusi tersebut diharap bisa mengurangi defect pada produksi as hidrolis damp truck. Kata kunci: FMEA; Defect; RPN; as Hidrolis; Truck Abstract: Evaluation of defect in Hydraulic spare-part production process Based on Failure Mode and Effect Analysis approach. The growth of truck-type commercial vehicles has led to innovation in adding a hydraulic system. UD. RZ is a supplier of dump truck hydraulic spare parts. The producer has been observing a comprehensive solution to reduce defects in the production process of hydraulic spare-part. This study aims to evaluate failures along the production process of hydraulic axles using the Failure Mode and Effect Analysis approach. This study begins with mapping the flow of the production process in order to identify product defects, determining the scale of occurrence, severity, and detection by employees and business owners, calculating risk priorities, and mapping the causes and effects of 5M-based defects. This study result distinguished the two topmost Risk Priority Numbers, which scored 224, namely miscalculation in the measurement process and drilling point errors, with a score of 147. The defect has been studied, and found factors such as inaccuracy in work, and the lack of drilling point tools. The solution can be done by re-turning, increasing the accuracy of workers, adding workers' SOPs, and having drilling point tools. The solution is expected to reduce defects in producing hydraulic spare-part of dump trucks. Keywords: FMEA; RPN; Defect; as Hydraulic; Dump Truck

Abstract

Abstrak: Evaluasi Pengendalian Stock Obat Obatan di Apotek FDF Berbasis Pendekatan EOQ dan Min-Max. Apotek FDF melaksanakan kegiatan kefarmasian yang didalamnya terdapat pengadaan obat, penyimpanan obat, peracikan dan penyaluran. Permasalahan yang sering terjadi pada apotek adalah pada kontrol stok barang dan juga kadaluwarsa obat, dimana pada apotek ini penulisan laporan seperti laporan barang masuk dan keluar, laporan penjualan, laporan persediaan barang dan lain-lain masih menerapkan metode konvensional yaitu penulisan laporan secara manual pada buku, sehingga kurang efektif dan efesien. Penelitian ini bertujuan untuk mengevaluasi kebijakan perusahaan dalam mengendalikan persediaan dengan membandingkan metode Min-Max, EOQ, dan menentukan ROP. Penelitian ini menggunakan data kuantitatif yaitu permintaan barang pada bulan sebelumnya, biaya penyimpanan dan pemesanan. Penelitian menunjukkan bahwa hasil metode Min-Max menghasilkan efisiensi terbaik dengan total biaya persediaan yaitu Rp.44.950 untuk 3 jenis obat berjenis tablet 6. Kata kunci: stok, apotek, ABC, EOQ, metode Min-Max. Abstract: Evaluation of Drugs, Inventory Control in FDF {Pharmacy using EOQ and Min-Max Method. Pharmacy FDF is a drug and medical equipment store where pharmaceutical activities are carried out, which include drug procurement, drug storage, compounding, and distribution. The management has been figuring out a comprehensive method for evaluating problems exceeding supply stock control and drug expiry. This study aims to evaluate the company's policy in controlling the supply of drugs and medical types of equipment by comparing the Min-Max method, Economic Order Quantity, and Re-Order Point. This study uses quantitative data, namely the demand for goods in the previous month and storage and ordering costs. The results showed that the Min-Max method produced the best efficiency with a total inventory cost of IDR 44,950 for three types of six tablets. Keywords: stock, min-max, ABC, pharmacy, Min-Max.

Abstract

Quality assurance is an action to maintain customer satisfaction. As a prominent fabrication producer, Firm A, which receives regular orders from corporate clients, has been challenged with maintaining quality consistency during the production process of filter tanks. This study aims to identify risks and evaluate failures that occur during the filter tank production process based on the Failure Mode and Effects Analysis method. The FMEA stage was started with a walkthrough survey to identify defects, followed by brainstorming sessions with two expert respondents to determine the severity, occurrence, and detection score. Subsequently, the Risk Potential Number score was calculated. Research findings distinguished three types of defects during production stages, namely welding, asymmetric, and painting. Based on risk quantification, it was found that the highest defect score, 392, was attributed to the painting failure category, characterized by excessively thick paint results. This finding was followed by the second-highest score of 336, and the third with value of 294 as failure of asymmetric joints. The practical implication in complementary of the FMEA finding showed that systematic training, audits, and briefings are suggested to address the issues that cause the highest number of failures on fabrication process.

Abstract

Pharmacy CBA is a trading business that engages in pharmaceutical and medical devices. Uncertainty and complexity have been identified as risks in business supply chains. Further measurement is needed in order to enhance the effectiveness of the pharmacy business process. The purpose of this study is to identify risk events and risk agents, calculate the risk value in the supply chain flow from upstream to downstream, and determine efforts to minimize risk. The method used in this research is HOR 2 phases, through a SCOR-based approach. The study begins with the identification of risk events and risk agents based on SCOR, then calculates the level of severity, occurrence, and correlation for the calculation of the HOR 1. Identified 45 risk events, 23 risk agents, and 19 actions to minimize risk. Aggregate risk potential is calculated as a result of HOR 1. From 23 risk agents, ten are prioritized based on the most significant ARP value. In the calculation of HOR 2, 10 risk mitigation strategies are obtained to minimize risk along with the supply chain stream. There are four approaches to implementing the ten mitigations: HR Development, Customer Relationship Management-based approach, Supplier relationship management, and facility design.

Abstract

CV. AAA is a company engaged in the production of water tanks. As a player in the manufacturing industry, CV. AAA performs continuous improvement in each production process to minimize losses due to non-value-added activities due to waste and ineffectiveness in carrying out the production process. This research objective evaluates activities that cause waste in the water tank production line with a Lean approach and ergonomics. This research begins by describing the current state map to identify waste throughout the production flow. Followed by the analysis of the occurrence of non-value-added activities at the stages of the production process. The results showed that three wastes were identified, namely Waiting, Defect, and Excess Processing. The non-optimal production process is because the tools used are manual labor which can lead to the risk of muscle injury to the operator. Nordic Body Map Scoring result obtained from four operators found that the final score is from medium to high level. Scenario improvement presented in future state map shows in reducing cycle time as well as lead time.
