Abstract

Hypertension is one of the number one causes of death, globally. Hypertension is the most common cause of cardiovascular events and is a major problem in both developed and developing countries. Cardiovascular is also the number one cause of death in the world every year. Respondents in this study were patients with a diagnosis of hypertension in hospitalized patients at Mardi Rahayu Kudus Hospital, totalling 41 people. the characteristics of hypertension patient respondents seen from the age of the most patients are in the late adult age category (36-45 years), namely 31 people (75.6%), while for early adulthood (26-45 years). 35 years) as many as 10 people (24.4%). The characteristics of hypertension patient respondents seen from the sex of the most patients were male, namely 25 people (61.0%), while the female gender was 16 people (39.0%). Results The highest systolic blood pressure was 170 mmHg and the lowest was 145 mmHg. The average systolic blood pressure was 152.90 mmHg. In hypertensive patients on the left arm, the highest systolic blood pressure was 170 mmHg and the lowest was 145 mmHg. The average systolic blood pressure was 152.90 mmHg. Based on the dependent t-test, the t value is -12.491 with a p-value of 0.000. It can be seen that the p-value is 0.000 < (0.05), this indicates that there is a significant difference in the results of blood pressure measurements between the right arm and the left arm in hypertensive patients in the inpatient room of Mardi Rahayu Kudus Hospital. The difference in blood pressure variations obtained in this study is still considered normal because the difference in MAP between the right and left arms is 6.11 mmHg (normal 10-20 mmHg).

Abstract

An unsystematic handover process can lead to important information not being properly conveyed, which increases the risk of medical errors and jeopardizes patient safety. The application of the SBAR (Situation, Background, Assessment, Recommendation) communication method is designed to improve the quality of nurse handover by providing a clear structure for communication. The study used a quasi-experimental method with a pre-test-post-test design without a control group. The sample consisted of 30 executive nurses at Kudus Private Hospital. Data were analyzed using paired t-test and Kolmogorov-Smirnov normality test to compare handover quality before and after applying the SBAR Communication. Results were considered significant if p < 0.05. Before using SBAR communication, 33.3% of nurses showed low handover scores, 40% were in the medium category, and only 26.7% were in the high category, with a mean score of 2.8 (SD = 0.6). After the implementation of SBAR communication, there were no more nurses with low scores, 90% of nurses were in the high category, with the mean score increasing to 4.3 (SD = 0.4). Wilcoxon statistical test results showed a value of Z = -4.467 with p < 0.05, indicating a significant improvement in handover quality after using SBAR communication. The application of the SBAR communication significantly improved the quality of nurses' handover in Kudus Private Hospital, with an average increase in score of 1.5 points. This method is effective in improving communication structure, reducing errors, and increasing patient safety.

Abstract

Ineffective communication between the medical team, especially nurses and doctors, can lead to medical errors that impact patient satisfaction. The SBAR (Situation, Background, Assessment, Recommendation) communication method was introduced as an effective way to improve communication in the healthcare environment. This study aims to analyze the effect of implementing the SBAR communication method on patient satisfaction through a literature review. The technique used was a literature review that followed the PRISMA guidelines. Articles were retrieved from Pubmed, Google Scholar, and Science Direct databases, and published between 2019 and 2024. Of the 29,400 articles found, filtering was carried out until 25 articles that met the criteria were left, using an analysis of the PRISMA guide. The results showed that the implementation of SBAR significantly improved the clarity and quality of communication between medical personnel, reduced misinformation, and contributed to improved patient safety. In surgical and palliative care, SBAR was shown to speed up decision-making and improve patient satisfaction. However, several studies have revealed that SBAR is considered too rigid in emergencies that require flexibility and rapid response, and less effective in complex cases. Other challenges include inconsistent adoption in busy hospitals and the need for ongoing training to maintain effective use of SBAR. Despite implementation challenges, SBAR remains an important communication tool to improve patient safety and satisfaction, especially in clinical environments requiring clear and structured information transfer.

Abstract

ABSTRAK Dukungan keluarga merupakan faktor penting yang memengaruhi tingkat kepatuhan terapi pada pasien kanker payudara. Penelitian ini bertujuan untuk menganalisis hubungan antara dukungan keluarga dan kepatuhan terapi pada pasien kanker payudara di RSUD Dr. Soetomo Surabaya. Penelitian ini menggunakan metode kuantitatif dengan desain cross-sectional, melibatkan 30 pasien yang dipilih menggunakan teknik purposive sampling. Data dikumpulkan melalui kuesioner terstruktur yang mencakup pengukuran dukungan keluarga (emosional, instrumental, dan informasi) serta tingkat kepatuhan terapi. Analisis data menggunakan uji korelasi Pearson dengan tingkat signifikansi p < 0,05. Hasil penelitian menunjukkan bahwa dukungan keluarga memiliki hubungan signifikan dengan kepatuhan terapi (r = 0,678; p < 0,05). Sebanyak 98% pasien (29 pasien) menunjukkan kepatuhan tinggi terhadap terapi, sementara 2% (1 pasien) tidak patuh. Semua pasien dengan dukungan keluarga yang baik (95%) melaporkan kepatuhan terapi yang tinggi (100%), sedangkan pasien dengan dukungan keluarga yang kurang baik memiliki tingkat kepatuhan yang lebih rendah (50%). Faktor utama ketidakpatuhan meliputi kelelahan emosional, keterbatasan finansial, dan kurangnya dukungan emosional. Penelitian ini juga mengidentifikasi tiga jenis utama dukungan keluarga yang berkontribusi pada kepatuhan terapi: dukungan emosional, dukungan instrumental, dan dukungan informasi. Strategi yang diusulkan untuk meningkatkan kepatuhan terapi meliputi edukasi keluarga, peningkatan dukungan finansial, pengembangan program berbasis komunitas, pelibatan keluarga dalam pengambilan keputusan medis, dan pendampingan psikologis bagi pasien serta keluarga. Temuan ini menegaskan pentingnya peran keluarga dalam mendukung pasien kanker payudara secara holistik, yang dapat meningkatkan keberhasilan terapi dan hasil pengobatan pasien. Kata Kunci: Dukungan keluarga, kepatuhan terapi, kanker payudara
