Abstract

Introduction: Laparotomy is a frequent surgical procedure carried out in emergency settings due to various indications and related to a considerable amount of postoperative mortality rate. POSSUM and P-POSSUM are known as two of the most favorable methods to calculate the likelihood of postoperative mortality, so that optimization of patient management can be achieved. The purpose of this research is to assess the predictive power of POSSUM and P-POSSUM scoring systems for emergency laparotomy patients in terms of accuracy, sensitivity, specificity, and Area Under the Curve (AUC) value. Method: This study uses an observational analytical method with a prospective cohort approach, and was performed at Dr. Zainoel Abidin Regional General Hospital Banda Aceh since January until May 2024. Mortality status was observed for 30 days after surgery. The accuracy of POSSUM and P-POSSUM was assessed based on sensitivity, specificity, and AUC values obtained from the Receiver Operating Characteristic (ROC) curve. Results: A total of 13 subjects experienced mortality within 30 days after surgery out of 29 subjects involved in this study. Analysis based on the ROC curve shows that POSSUM and P-POSSUM equally have 100% sensitivity and 75% specificity. The AUC values obtained were 0.851 and 0.837 respectively. Conclusion: POSSUM and P-POSSUM scoring systems have good accuracy as mortality predictors in emergency laparotomy patients.

Abstract

Infeksi Luka Operasi (ILO) merupakan salah satu komplikasi pembedahan yang paling umum terjadi di dunia, terutama di negara berkembang. Tujuan penelitian ini adalah untuk melihat gambaran evaluasi pemberian antibiotik profilaksis terhadap kejadian ILO. Penelitian ini bersifat deskriptif observasional menggunakan pendekatan prospektif, rancangan penelitian yang dilakukan melalui pengukuran atau pengamatan dengan cara mengikuti perjalanan penyakit. Penggunaan antibiotik profilaksis secara rasional dinilai dengan menggunakan kategori Gyssen dan penilaian ILO dengan menggunakan skala Morrison, pada hari ke 3, 7, dan 30 setelah operasi. Cara pengambilan sampel menggunakan total sampling dengan keseluruhan 34 sampel penelitian yang diambil selama periode April 2019─Agustus 2019. Hasil evaluasi rasionalitas penggunaan antibiotik profilaksis dengan metode Gyssen diperoleh kategori 0 (rasional) sebanyak 76,5%, kategori I (tidak tepat waktu pemberian antibiotik profilaksis) 14,7%, kategori IIA (tidak tepat dosis pemberian antibiotik) 5,9% dan kategori IVA (ada alternatif lain yang lebih efektif) 2,9%. Hasil evaluasi kejadian ILO dari 34 sampel selama 30 hari diperoleh nilai rata-rata persentase kejadian ILO pada hari ke-3 (20,59%), hari ke=7 (11,76%), hari ke-30 (0%). Kesimpulan penelitian ini adalah dari 34 sampel tidak ditemukan ILO.

Abstract

Introduction: Tamoxifen (selective estrogen receptor) is the standard hormonal therapy in women with breast cancer with positive estrogen receptor. The use of this regimen proved to reduce the incidence of relapse at local, collateral, or distant body area, and also decrease breast cancer mortality. One of the effects of tamoxifen is to induce endometrial proliferation, cause various abnormalities in endometrial tissue.Method: This study is an unpaired categorical comparative analytic study with a retrospective single cohort (internal comparison group) design. All breast cancer patients at dr. Zainoel Abidin Banda Aceh from 2017 - 2019 was involved in this research. Evaluation of the incidence of endometriosis was carried out based on the duration of tamoxifen using the Mann-Whitney U test. This study is an unpaired categorical comparative analytic study with a retrospective single cohort (internal comparison group) design. All breast cancer patients at dr. Zainoel Abidin Banda Aceh from 2017 - 2019 was involved in this research. Evaluation of the incidence of endometriosis was carried out based on the duration of tamoxifen using the Mann-Whitney U test.Result: A total of 114 patients (46.15%) out of a total of 247 breast cancer patients using tamoxifen reported experiencing endometriosis with a mean age of 49 years old. Most of the patients diagnosed with stage IIIB breast cancer of ductal cell carcinoma. A total of 111 people (97.37%) experienced endometriosis after taking tamoxifen for 30-36 months and the remaining 3 (2.63%) after 24-29 months. The mean duration of tamoxifen use associated with endometriosis was 34 months (p less than 0.001).Conclusion: Long-term use of tamoxifen in breast cancer patients increases the risk of endometriosis
