Abstract

It is difficult to predict the end of Covid-19 pandemic. The emergency department (ED) as the main access for health services in the hospital has the highest risk of infection and overwhelmed by surge of patients. Modification of the emergency service system by implementing a pre-triage system can be a solution to dealing with surge of patients. The results of the preliminary study succeeded in forming a pre-triage system model. The pre-triage system model was carried out by identifying patients with the risk of Covid-19 in ED and modification of the emergency service flow. The implementation of the pre-triage system requires the readiness of the emergency department’s team. The sample used was all staff of emergency department of X Hospital which total 30. The results showed that 23,3% of respondents had good category knowledge, 56,67 % respondents had sufficient category knowledge, and 20% had less knowledge. The level of readiness found that 90% of respondents are ready for implementation of the pre-triage system. The Chi-Square test result show that the level of knowledge of the pre-triage system is not related to readiness to implement the pre-triage system.

Abstract

The Emergency Department (ED), as the main entrance of health service access in hospitals, faces the highest infection risk and high service load due to the increasing number of patients during the Covid-19 pandemic. Implementing a pre-triage system offers a solution to deal with a surge in the number of patients who demand staff preparedness. This study aimed to determine the knowledge level on the pre-triage system and its relationship with the preparedness of emergency room staff in implementing the pre-triage system. The study was conducted with a cross-sectional approach in a private type C hospital in Malang District on all 30 ER staff as respondents. The research instrument used were questionnaires. Data analysis was done using descriptive analysis and the Fisher exact test. The results show that although 76.7% of respondents have insufficient knowledge, 86.7% of respondents are ready to implement the pre-triage system. The test results prove that there is no relationship between the knowledge and the preparedness levels of the emergency room staff in implementing the pre-triage system.

Abstract

The JKN mobile application (MJKN), is an application presented by BPJS with the aim of providing convenience for its participants. MJKN utilization in registration process is one of the BPJS compliance indicators for hospitals. As part of BPJS partners, the achievement of compliance indicators is something that must be achieved by Hospital X. Until September 2024, the highest rate of MJKN utilization in X Hospital was 21% of the target achievement of ≥30%. This study aims to determine the challenges and barriers of fulfilling the MJKN utilization indicators in registration process at X Hospital. This research is a qualitative research with a case study approach. In-depth interviews were conducted with eight BPJS patients and eight people from X Hospital management related to the implementation of MJKN. Data analysis is strengthened by triangulating based on observations and secondary data related to the MJKN process. Seven challenges were identified, including the low quality of evaluation of staff performance at Hospital X related to MJKN, the difficulty of bridging between hospital information system and the MJKN information system, patient difficulties during the MJKN account registration process, low access to information related to MJKN, the complexity of the MJKN check in process, resistance to technology, and patient devices that do not meet MJKN standards. The results of this study can be the basis for Hospital X management in determining solutions to improve the achievement of MJKN utilization.

Abstract

Pre-hospital emergency care is a critical component of the Emergency Medical Services (SPGDT) to mitigate mortality and disability rates. However, village ambulance volunteers, acting as Community First Responders (CFR), frequently encounter a discrepancy between their significant responsibilities and limited clinical competencies, compounded by logistical deficits. This community service initiative aims to enhance the basic medical competence of village ambulance volunteers through structured training interventions. The implementation utilized a collaborative approach with an emphasis on hands-on practice and real-world scenarios. Data were qualitatively analyzed using Reflexive Thematic Analysis based on observations and focused discussions. The analysis identified three primary themes: (1) clinical skill deficits posing risks of evacuation malpractice (improper lifting), (2) logistical gaps in standard ambulance equipment, and (3) the efficacy of practice-based and simulation training methods. The training significantly improved participants' knowledge, technical skills (CPR, spinal stabilization, triage), and self-confidence. These findings underscore the necessity of standardizing ambulance logistics through village budgets and developing a Training of Trainers (ToT) model to ensure the sustainability of community-based health resilience.
