Abstract

Gastroesophageal reflux disease (GERD) is a medical condition defined by the retrograde flow of gastric contents into the esophagus, leading to various symptoms.  The principal therapy for GERD consists of proton pump inhibitors (PPIs).  Nevertheless, PPIs demonstrate reduced effectiveness in patients with non-erosive GERD or unusual symptoms.  Alginates operate by forming a gel "raft" that floats on the gastric contents, serving as a physical barrier against reflux.  Multiple studies demonstrate that alginate is more effective than antacids in alleviating GERD symptoms and has a longer duration of action.

Abstract

Chronic pancreatitis is a long-term inflammatory condition affecting the pancreas, with progression worsening over time. One of the effects of this disease is the formation of scar tissue, which can permanently damage the pancreas and disrupt its functions, both exocrine and endocrine. Chronic pancreatitis can result in various serious complications and even be fatal. In the effort to treat chronic pancreatitis, therapies targeting macrophages have begun to attract attention as an innovative approach. One of the drugs in the spotlight is dasatinib, a tyrosine kinase inhibitor, which has shown the ability to alter the inflammatory response by affecting the macrophage population in the pancreatic microenvironment. This literature review aims to dig deeper into the role of dasatinib in the treatment of chronic pancreatitis and explain the pathophysiology of this disease, especially how macrophage activation contributes to the inflammatory process, as well as the therapeutic effects of dasatinib, we can find new insights to develop more effective treatment strategies.

Abstract

Pembentukan karakter merupakan tugas terpenting bagi lembaga pendidikan. Realita menunjukkan bahwa masih banyak permasalahan dalam pembentukan karakter siswa di sekolah. Penelitian ini bertujuan untuk mengungkap fenomena program penguatan karakter di sekolah. Melalui pendekatan kualitatif dengan metode studi kasus, penelitian ini berupaya menggali secara mendalam pengalaman serta perspektif partisipan terkait fenomena yang dikaji. Pengumpulan data dilakukan melalui observasi langsung di lingkungan penelitian serta wawancara mendalam dengan tujuh siswa dan tiga guru kelas yang dipilih secara purposive. Untuk memastikan validitas data, dilakukan teknik member checking dengan meminta konfirmasi dari partisipan terkait keakuratan interpretasi peneliti terhadap temuan yang diperoleh. Data dianalisis menggunakan model analisis interaktif yang meliputi tiga tahapan utama, yaitu reduksi data, penyajian data, dan penarikan kesimpulan. Proses analisis dilakukan secara berkelanjutan hingga diperoleh saturasi data, sehingga temuan yang dihasilkan dapat merepresentasikan fenomena secara komprehensif. Hasil penelitian menunjukkan bahwa pencapaian nilai-nilai karakter siswa memerlukan berbagai strategi. Pertama, perlu adanya aturan yang jelas dan konsisten. Kedua, pendekatan keagamaan dapat menjadi sarana untuk melatih karakter siswa. Selain itu, pembentukan posko aktif di setiap kelas juga turut memberikan dampak positif terhadap kedisiplinan siswa. Pesan-pesan kedisiplinan perlu disampaikan melalui area-area tertentu di sekitar sekolah yang dikelola secara bergantian oleh masing-masing kelas, agar siswa terlibat dalam proses tersebut. Pemantauan kedisiplinan siswa dapat dilakukan melalui sinergi antara pihak sekolah, orang tua, dan masyarakat. _________________________________________________________________________________ Character building is the most important task for educational institutions. Reality shows that there are still many problems in character building of students in schools. This study aims to reveal the phenomenon of character strengthening programs in schools. Through a qualitative approach with a case study method, this study attempts to explore in depth the experiences and perspectives of participants related to the phenomenon being studied. Data collection was carried out through direct observation in the research environment and in-depth interviews with seven students and three class teachers who were selected purposively. To ensure data validity, a member checking technique was used by asking for confirmation from participants regarding the accuracy of the researcher's interpretation of the findings obtained. Data were analyzed using an interactive analysis model that includes three main stages, namely data reduction, data presentation, and drawing conclusions. The analysis process was carried out continuously until data saturation was obtained, so that the findings produced could represent the phenomenon comprehensively. The results of the study showed that achieving students' character values ​​requires various strategies. First, there needs to be clear and consistent rules. Second, a religious approach can be a means to train students' character. In addition, the formation of active posts in each class also has a positive impact on student discipline. Discipline messages need to be delivered through certain areas around the school that are managed alternately by each class, so that students are involved in the process. Monitoring of student discipline can be done through synergy between the school, parents, and the community.

Abstract

Background: Liver fibrosis is scar tissue that forms due to excessive accumulation of ECM proteins due to chronic liver injury, mediated by cytokines involving complex cellular interactions, such as platelet-derived growth factor-BB (PDGF-BB). In Indonesia, no research has been conducted to determine the correlation between PDGF-BB levels and the degree of liver fibrosis based on fibroscan. The aim of this study is to assess the correlation between PGDF-BB levels and the degree of liver fibrosis based on fibroscan in chronic hepatitis B. Methods: A cross-sectional study was conducted on the chronic hepatitis B population at the internal medicine clinic of Dr. Mohammad Hoesin General Hospital Palembang from January to Agustus 2023. The assessment uses fibroscan to determine the degree of liver fibrosis, measured in kilopascals (kPa). PDGF-BB levels measured by ELISA. Results: From 40 subjects, the majority of chronic hepatitis B patients were women (57,5%). The lowest PDGF-BB level was found in chronic hepatitis B patients with fibroscan F0, and the highest PDGF-BB level in fibroscan F4. Fibroscan results are directly proportional to PDGF-BB level. Conclusion: PDGF-BB has a strong positive correlation with the degree of fibroscan in chronic hepatitis B patients in Dr. Mohammad Hoesin General Hospital. The higher the range of PDGF-BB levels, the higher the degree of fibroscan in chronic hepatitis B patients.

Abstract

The small changes in oxygen tension that occur during viral hepatitis, metabolic disorders, steatohepatitis, inflammation, and carcinogenesis are sufficient to increase hypoxic response, namely an increase in HIF. HIF is a protein that belongs to the PAS (period circadian protein-aryl hydrocarbon receptor nuclear translocator-single-minded protein) family. HIF regulates the adaptation of cells to oxygen. HIF is a transcription factor containing α and β subunits. The expression of the α subunit is oxygen-dependent, while the β subunit is expressed continuously and independently of oxygen. HIFs regulate various signaling events by binding to specific DNA sequences known as hypoxia response elements (HREs) in target genes, leading to an increase or decrease in their transcription. HIF-1α is a key regulator of hypoxia signaling.

Abstract

Hepatocellular carcinoma (HCC) is a malignant tumor originating from liver cells, HCC occurs in around 85% of patients diagnosed with cirrhosis. Treatment options for HCC consider liver function, extrahepatic spread, invasiveness, and the number and size of nodules. HCC therapy options include surgical resection, liver transplantation, tumor ablation, transarterial therapy and systemic chemotherapy. Pembrolizumab is a second-line systemic therapy option for the treatment of HCC after sorafenib therapy. Pembrolizumab is a class of immune checkpoint inhibitors (ICIs) and more specifically works as a programmed cell death-1 (PD-1) inhibitor.

Abstract

Background: Hepatocellular carcinoma (HCC) is the most common primary liver malignancy and one of the leading causes of cancer death worldwide. Chronic hepatitis B virus infection is the most common etiology of HCC. Programmed death ligand-1 (PD-L1) is a protein that is overexpressed on the surface of tumor cells, which can be detected in serum (sPD-L1). Elevated sPD-L1 concentrations in the plasma of cancer patients are associated with poor prognosis in HCC patients, as indicated by increased AFP levels. There have been no previous studies examining the correlation of sPD-L1 levels with AFP in HCC patients. The aim of the study was to determine the correlation of sPD-L1 and AFP as predictors of prognosis in HCC patients. Methods: The research is an observational study with a cross-sectional design to see the correlation of sPD-L 1 with serum alpha-fetoprotein levels in HCC patients at Dr. Mohammad Hoesin General Hospital Palembang starts from April to July 2023. Measurement of sPD-L1 levels and serum AFP levels is carried out using ELISA examination. Results: Of the 28 subjects studied, HCC patients were more commonly found in men (89.29%) aged 18-60 years (75%). sPD-L1 and AFP values were found to be increased in the advanced stages of HCC. The sPD-L1 value is directly proportional to the serum AFP value. Conclusion: There is a strong positive correlation between sPD-L1 levels and serum AFP levels caused by hepatitis B in HCC patients, so the higher the sPD-L1, the higher the serum AFP levels.

Abstract

Background: Gastrointestinal symptoms are common among patients with chronic kidney disease, reported in up to 70%–80% of patients with end-stage kidney disease. Several factors are involved in the pathogenesis, including the accumulation of uremic toxins and intestinal flora changes. Pseudomembranous colitis is a manifestation of severe colonic disease that is usually associated with Clostridium difficile infection but can be caused by a number of different etiologies. Pseudomembranous colitis occurs when changes in the fecal flora allow Clostridium difficile overgrowth. Gut dysbiosis, an alteration of the composition and function of gut microorganisms, is commonly seen in patients with chronic kidney disease. Case presentation: This scientific report presents a case study of a 31-year-old woman diagnosed with pseudomembranous colitis and end-stage chronic kidney disease on hemodialysis. The colonoscopy showed yellow-white nodules that formed pseudomembranes, which were scattered between the edematous mucosa. The patient had been treated with metronidazole for 4 days, but there were side effects. The antibiotic was switched to meropenem for 6 days based on feces culture. The evaluation of colonoscopy result was normal. Conclusion: Pseudomembranous colitis is an inflammatory bowel condition most commonly caused by Clostridium difficile infection. The presence of pseudomembranous visualization in colonoscopy can significantly establish the diagnosis.
