Abstract

Background: Caesarean section is a surgery to give birth to a child through an incision in the abdominal wall and uterus. Problems that can arise in post-cesarean section patients such as ineffective breastfeeding. Ineffective breastfeeding is a condition where the mother and baby experience dissatisfaction or difficulty while breastfeeding (IDHS, 2017). Breast milk production can be accelerated with non-pharmacological measures, namely through oxytocin massage which can be done by massaging the area around the back (pars thoracic vertebrae). Purpose: Able to carry out nursing care for post sectio caesarea (SC) mothers with nursing problems ineffective breastfeeding using oxytocin massage in Metro City 2023. Method: Writing this final report using a descriptive research design with a case study design. Subjects used by 2 people who experienced breastfeeding nursing problems were not effective. This research was conducted in June 2023. This case study was conducted for 3 days with Oxytocin Massage for 2-3 minutes 2 times per day in the morning and evening. Results: Nursing management of ineffectiveness in breastfeeding with the application of oxytocin massage nursing actions that the author carried out for 3 days found the problem of Ineffective Breastfeeding in Postoperative Mothers with Sectio Caesarea associated with inadequate supply of breast milk in both clients resolved, all evidenced by clients Mrs. M and Mrs. A The production of breast milk is smooth and during the action the two clients are cooperative. Conclusion: There is an effect of oxytocin massage to increase milk production. Suggestion: For future researcher to examine factors that affect breast milk production in postpartum mothers such as nutrition during pregnancy and factors that affect anxiety production in postpartum mothers such as conditions after cesarean. Keywords: Oxytocin Massage; Pregnant Women; Sectio Caesarea. Pendahuluan: Seksio sesarea adalah suatu pembedahan untuk melahirkan anak lewat insisi pada dinding abdomen dan uterus. Masalah yang dapat muncul pada pasien post seksio sesarea seperti menyusui tidak efektif. Menyusui tidak efektif merupakan suatu kondisi dimana ibu dan bayi mengalami ketidakpuasan atau kesulitan pada saat menyusui (SDKI, 2017). Pengeluaran ASI dapat dipercepat dengan tindakan non farmakologi,yaitu melalui pijat oksitosin yang dapat dilakukan dengan cara memijat areadisekitar punggung (vertebra pars thoratica). Tujuan: Mampu melaksanakan asuhan keperawatan pada ibu post sectio cesarea (SC) dengan masalah keperawatan ketidakefektifan pemberian ASI menggunakan pijat oksitosin di Kota Metro 2023. Metode: Pada penulisan laporan tugas akhir ini menggunakan desain penelitian deskriptif dengan rancangan studi kasus. Subjek yang digunakan 2 orang yang mengalami masalah keperawatan menyusui tidak efektif. Penelitian ini dilakukan pada bulan Juni 2023. Studi kasus ini dilakukan selama 3 hari dengan tindaka Pijat Oksitosin selama 2-3 menit 2 kali perhari pada pagi dan sore hari. Hasil: Pengelolaan  keperawatan ketidakefektifan  dalam  pemberian  ASI  dengan  penerapan  tindakan  keperawatan  pijat oksitosin  yang  penulis  lakukan  selama  3  hari  didapatkan  masalah  Menyusui  Tidak Efektif   Pada   Ibu   Post   Operasi   Dengan   Sectio   Caesarea berhubungan   dengan ketidakadekutan suplai ASI  pada  kedua  klien  teratasi  semua  dibuktikan  dengan klien  Ny  M dan Ny. A produksi ASI nya lancar dan selama tindakan kedua klien koperatif. Simpulan: Terdapat pengaruh pijat oksitosin untuk meningkatkan produksi ASI. Saran: Kepada peneliti selanjutnya untuk meneliti faktor yang mempengaruhi penegeluaran ASI pada ibu post partum seperti nutria selama hamil dan faktor yang mempengaruhi pengeluaran kecemasan pada ibu post partum seperti keadaan setelah sesar.

Abstract

Psychological disorders in mothers cause reduced breastfeeding expenditure. Because it will inhibit the let down reflex. Maternal anxiety is increasing with the existence of social restrictions, so that the mother is worried that the child's needs cannot be met. and the mother said she was worried and confused about the current state of the Covid-19 pandemic. The purpose of this study is whether there is a relationship between the level of anxiety due to Covid-19 and the perception of adequacy in breastfeeding. The purpose of this study is whether there is a relationship between the level of anxiety due to Covid-19 and the perception of adequacy in breastfeeding. This type of research is a quantitative study and uses an analytical research design with a cross sectional approach. The subjects of this study were postpartum mothers who had babies. The object of this research is the level of anxiety due to covid-19. The location of this research was conducted at RSIA AMC Metro Lampung and the time of this research will be conducted in July - August 2020. Results of the study It is known that out of 33 respondents, 9 (27.3%) were not categorized as respondents, with 24 (72.7%) categories of respondents. It is known that out of 33 respondents with a perception of adequacy gave positive breastfeeding 17 (51.5%) respondents, and 16 respondents (48.5%) with a perception of adequacy. There is a significant relationship between the level of damage caused by Covid-19 and the perception of breastfeeding adequacy. (p-value 0.017 OR 13.333). This research concludes that there is a relationship between the level of anxiety due to Covid-19 and the perception of adequacy of breastfeeding.
