Abstract

Background: Neurofibromatosis is a genetic disorder which manifests as a tumor that surrounds the nerves and several other pathologic presentations.Case: Female, 11 years old, according to history, physical examination and supporting investigation was consistent with the diagnostic criteria of type 1 neurofibromatosis (NF-1). Patient also has intellectual disability. On the patient, a tumor on the foot region that was functionally disabling was found and a surgical treatment was performed.Discussion: Cognitive disorder is the most common neurological complication in individuals with NF-1 and usually present with low intelligence quotient (IQ). There is no specific treatment to prevent the development of NF-1. However, early treatment can minimize and prevent further complication. Treatment for the patient involve multiple clinical discipline.Conclusion: A neurofibromatosis case can be diagnosed clinically alone, however, since there are many other systemic involvements that require multi-disciplinary approach.

Abstract

Background: Herpes zoster (HZ) presents with unilateral dermatomal pain and rash resulting from the reactivation and proliferation of latent varicella zoster virus (VZV) that dwells in neurons following varicella. It mostly occurs in the elderly and in immunocompromised . Warfarin consumption can lead to dysregulation of cytokines that cause immune dysregulation, which can be a factor in the reactivation of herpes zoster. There is limited research or study exploring the correlation between herpes zoster reactivation and the use of warfarin in patients with atrial fibrillation This case report aim to investigate potential role of anticoagulant therapy, specifically warfarin, in the reactivation of HZ in elderly patients, and to explore the implications of polypharmacy and immune dysregulation in the management and prevention of HZ. Case Description: A 55-year-old male with a history of atrial fibrillation, currently on warfarin, was admitted with complaints of pain blister lession that developed 7 days post-discharge from the Cardiology department. His initial symptoms included pruritus and pain localized to the chest and right thoracic area. Dermatological examination revealed herpetiform vesicles on an erythematous base, accompanied by crusts and scabs, distributed along thoracic dermatomes T6-T8. Conclusion: Elderly patients often experience polypharmacy due to degenerative diseases. Cardiovascular treatment often includes anticoagulants such as warfarin. Following 1 week after the administration of topical antibiotic and steroid ointments, the patient demonstrated a positive clinical outcome with a few complications after HZ infection.

Abstract

This study aims to analyze the relationship between the implementation of clinical governance and the prevention of medical disputes at Wangaya Regional Public Hospital, Denpasar. A quantitative analytical approach with a cross-sectional design was employed. Data were collected through questionnaires distributed to approximately 30 healthcare professionals, including doctors, nurses, and other medical staff using a Likert scale (1–5). This study can be considered a preliminary study (pilot project) due to the relatively limited sample size (n=30). The results indicate that the implementation of clinical governance is categorized as good, reflected in high compliance with clinical standards, effective supervision, and structured risk management systems. The quality of healthcare services is also perceived as very good, particularly in terms of patient safety, communication, and professionalism. Furthermore, healthcare workers show a strong agreement that high-quality services can significantly reduce the risk of medical disputes.Statistical analysis reveals a significant and positive relationship between clinical governance and the prevention of medical disputes (r = 0.65; p < 0.05). Regression analysis shows that clinical governance has a significant effect on dispute prevention, with a coefficient of 0.72 and a determination value of 42%. This implies that improved clinical governance contributes substantially to reducing potential conflicts between healthcare providers and patients.In conclusion, clinical governance serves as an effective preventive strategy in minimizing medical disputes while enhancing the quality and safety of healthcare services.
