Abstract

Varikokel merupakan dilatasi abnormal dan pembesaran sistem vena pleksus pampiniformis dan vena testis dengan refluks darah vena secara terus-menerus ataupun intermiten. Modalitas pencitraan dapat digunakan untuk konfirmasi diagnosis varikokel. Varikokel merupakan salah satupenyebab infertilitas pada laki-laki. Tata laksana yang tepat akan meningkatkan parameter semen. Varikokelektomi dengan teknik bedah mikroefektif dengan tingkat rekurensi dan komplikasi yang rendah.

Abstract

Giant pyonephrosis (GP) is a rare and severe form of pyonephrosis characterized by a massive enlargement of the affected kidney due to the accumulation of infected fluid. This study presents case of a 51-year-old male patient with giant pyonephrosis who underwent nephrectomy as treatment. The clinical manifestations of giant pyonephrosis are nonspecific, necessitating the use of imaging and laboratory techniques to aid in diagnosis. Nephrectomy is the preferred treatment option for these patients. One week after nephrectomy, the laboratory results showed improvements. Making the right treatment decision is challenging in this particular case.

Abstract

Introduction: Spontaneous perirenal hemorrhage (SPH), also known as Wunderlich syndrome, is a rare condition that can potentially progress into life-threatening non-traumatic retroperitoneal hemorrhage. Common symptoms include acute flank or abdominal pain, a palpable flank mass, and fulminant hypovolemia, collectively referred to as Lenk’s triad, and diagnosis is typically established radiologically using a CT scan. Antiphospholipid syndrome (APS) - a rare, immune-mediated hypercoagulable disorder that occurs in only 5 cases per 100,000 people per year worldwide. It is immune-mediated, characterized by persistent antiphospholipid antibodies (APLA) that cause both arteries and venous thrombosis, and can present as peripheral arterial disease (PAD), acute arterial occlusion, and early atherosclerosis. Case : A 33-year-old woman with chronic kidney disease (CKD) presented with SPH and incidentally diagnosed with peripheral arterial disease (PAD) and APS. Conclusion : Conservative management was beneficial for SPH without the need for surgical intervention; however, antithrombotic therapy for PAD posed additional challenges before the patient was eventually discharged in improved condition for outpatient follow-up.

Abstract

Objective: To report our experience in treating a case of Testicular non-Hodgkin Lymphoma. Case(s) Presentation: A 55-year-old man came to the urology outpatient clinic with complaints of an enlarged left painless testicle that was lumpy and heavy. Urological examination found left unilateral testicle enlargement testicle, the size measured 10x7x7 cm with firm borders, solid consistency, and no tenderness. The patient then was treated with left orchiectomy. Histology and Immunohistochemical examination found non-Hodgkin B cells (CD20+) high grade, non-GCB. Discussion: In this case, the patient was treated with orchiectomy followed by a CHOP chemotherapy regimen. However, there was an unusual presentation due to the suspicion of metastases through the findings of a tender lump in the left inguinal region which was further confirmed by histological and immunohistochemical examination. Conclusion: Primary testicular lymphoma with a subtype of DLBCL (Diffuse Large B-Cell Lymphoma) is a rare disease. Orchiectomy is the initial treatment option for Testicular non-Hodgkin Lymphoma followed by combination therapy of chemotherapy to improve the prognosis. Abstract

Testicular cancer accounts for 1% of all malignancies and 5% of urological-related neoplasms; most commonly in young males aged 15-40 years, seminoma is the most common subtype (50%-60%). A 72-year-old patient with increasing right scrotum size since 3 years ago. A right testicular tumor was found. Trans-scrotal radical orchiectomy was done as the definitive treatment. The result of the histopathological examination was a classic seminoma pT1. GCT is a testicular cancer uncommonly diagnosed in the elderly. A better prognosis is achieved through early diagnosis and appropriate treatment.
