Abstract

Chronic kidney disease is a detrimental health problem in children with increasing morbidity and mortality rates. CAPD as an option for renal replacement therapy provides superior benefits over conventional hemodialysis in children with terminal kidney disease. However, the effectiveness of CAPD in long term needs to be anticipated because of the risk of infectious and non-infectious complications. Cross-sectional study was conducted in the Medical Record Department of RSMH. The samples were pediatric patients with CAPD in June (2016-2021) with complete medical records. The Sample study was analyzed by univariate and bivariate. The relationship between risk factors and complications was analyzed using the Chi-square test. The subjects were 14 people, male, >5 years old, normal nutritional status (42.9%), had abnormality anatomy kidney (57.1%), and surgery by laparoscopy (71.4%). The most infectious complication is peritonitis (50%) and the non-infectious complication is poor drainage ( 42.9 %). There was no meaningful relationship among risk factors to complications in patients children with CAPD in RSMH (p>0.05). By the descriptive analysis, every factor plays a role in the incident of complications. However, no there is a meaningful relationship among factor risk of complications infectious and non-infectious.

Abstract

Anorectal malformations are a broad spectrum of anal and rectal disorders ranging from stenosis to anorectal agenesis. The incidence of mortality in cases of anorectal malformations is influenced by many factors, including the type of anorectal malformation, associated congenital anomalies, sepsis, and prematurity as investigated in this study. This study aims to identify the association between anorectal malformation type, associated congenital anomalies, sepsis, and prematurity with mortality on anorectal malformation patient. This research is an observational analytic study with cross sectional study design. Secondary data collection was obtained from eighty-two medical records of anorectal malformation patients in Dr. Mohammad Hoesin Hospital Palembang from January 2015 to June 2017 that meets the inclusion criteria. Of the eighty-two cases of anorectal malformations, nineteen cases of mortality (23.2%) were obtained. Of the four risk factors studied, two risk factors were found to significantly affect the mortality of anorectal malformation patients, they are anorectal malformation type (p value = 0.036) and sepsis (p value = 0.049). Factors that have significant correlation to mortality of patients with anorectal malformations are the type of anorectal malformations and sepsis, whereas associated congenital anomalies and prematurity have no effect on the mortality of patients with anorectal malformations.

The incidence of mortality in cases of anorectal malformations is influenced by many factors, including the type of anorectal malformation, associated congenital anomalies, sepsis, and prematurity as investigated in this study. The aim of this study was to identify the risk factor of mortality on anorectal malformation patients. This study is an observational analytic study with cross sectional study design. Abstract

Introduction: Anorectal malformation (MAR) is a congenital abnormality with a significant incidence rate. Posterior sagittal anorectoplasty (PSARP) is the standard surgical procedure for MAR correction. However, achieving post-operative continence remains a major challenge. This study aims to identify risk factors associated with continence in MAR patients after PSARP at Dr. Mohammad Hoesin General Hospital Palembang. Methods: This prospective observational study involved MAR patients who underwent PSARP at Dr. Mohammad Hoesin General Hospital Palembang between January 2018 to December 2023. Demographic, clinical data, and RINTALA questionnaire results were collected. Statistical analysis was performed using a chi-square test, independent t-test, and logistic regression to identify significant risk factors associated with continence. Results: A total of 120 post-PSARP MAR patients met the inclusion criteria and participated in this study. The mean age of patients was 8.5 years (SD = 3.2). The results of the RINTALA questionnaire showed a mean score of 45.3 (SD = 12.8), with 42.5% of patients experiencing poor continence (RINTALA score < 40). Univariate analysis showed a significant association between poor continence and high MAR type (p = 0.002), age at PSARP < 1 year (p = 0.031), and the presence of a history of related surgery (p = 0.017). In multivariate analysis, MAR type was high (OR = 4.21; 95% CI: 1.54-11.52; p = 0.005) and age at PSARP < 1 year (OR = 2.87; 95% CI: 1, 05-7.89; p = 0.040) remained an independent predictor of poor continence. Conclusion: This study succeeded in identifying the type of MAR high and age at PSARP < 1 year as significant independent risk factors associated with poor continence in MAR patients after PSARP at Dr. Mohammad Hoesin General Hospital Palembang. These findings provide valuable information for surgeons and patient families in anticipating and managing post-PSARP continence problems.
