Abstract

The transition of elderly lives from productive periods to non-productive makes them need support from relatives, friends or family. The purpose of this study was to look at the relationship between family appreciation support with quality and life satisfaction of the elderly in the city of Depok, West Java, with a cross-sectional design. The subjects of this study were 135 elderly people> 60 years old, living with family, do not have infectious diseases, able to communicate in Indonesian; selected by purposive sampling technique. Data were analyzed using Chi square test. The results of the study indicate that there was a relationship between family appreciation support with the quality of life of the elderly (p-value = 0.022) and life satisfaction of the elderly (p-value = 0.014). The results of this study are expected to support the development of nursing science in the future, especially regarding the quality and satisfaction of life of the elderly. Keywords: elderly; award support; quality of life; life satisfaction ABSTRAK Transisi kehidupan lansia dari masa produktif menjadi non produktif membuat mereka memerlukan dukungan dari kerabat, teman atau keluarga. Tujuan penelitian ini adalah untuk melihat hubungan antara dukungan penghargaan keluarga dengan kualitas dan kepuasan hidup lansia di Kota Depok, Jawa Barat, dengan desain cross-sectional. Subyek penelitian ini adalah 135 lansia yang berusia >60 tahun, tinggal bersama keluarga, tidak memiliki penyakit menular, mampu berkomunikasi dengan bahasa Indonesia; yang dipilih dengan teknik purposive sampling. Data dianalisis menggunakan uji Chi square. Hasil peneltian menunjukkan bahwa ada hubungan antara dukungan penghargaan keluarga dengan kualitas hidup lansia (p-value = 0,022) dan kepuasan hidup lansia (p-value = 0,014). Hasil penelitian ini diharapkan dapat mendukung perkembangan ilmu keperawatan di masa mendatang, khususnya tentang kualitas dan kepuasan hidup lansia. Abstract

Abstrak Proses penuaan dapat menyebabkan penurunan fungsi kognitif. Lansia dengan penurunan fungsi kognitif mengalami peningkatan metabolisme yang dapat menyebabkan malnutrisi. Penelitian ini bertujuan untuk mengetahui hubungan fungsi kognitif dengan indeks massa tubuh lansia di Panti Sosial Tresna Werdha wilayah DKI Jakarta. Desain penelitian yang digunakan adalah cross sectional dengan melibatkan 98 lansia yang dipilih melalui proportional dan simple random sampling. Fungsi kognitif dinilai dengan menggunakan kuesioner Mini Mental State Examination, sedangkan status gizi dinilai dengan menggunakan indeks massa tubuh. Hasil penelitian ini menunjukkan bahwa terdapat hubungan yang signifikan antara fungsi kognitif dan indeks massa tubuh (r= 0,550; p= 0,0001; α= 0,05). Pemberi pelayanan di panti perlu memiliki keterampilan khusus untuk meningkatkan status gizi pada lansia dengan penurunan kognitif. Kata kunci: fungsi kognitif, indeks massa tubuh, lingkar lengan atas, Mini Mental State Examination, Panti Sosial  Tresna Werdha Abstract Cognitive Function Decline can Reduce The Body Mass Index of Elderly in Nursing Home. Aging process affects the decline of the cognitive function. Elderly with cognitive decline may have increased metabolism that can cause of malnutrition. This study aimed to determine the relationship between cognitive function with body mass index of elderly at Panti Sosial Tresna Werdha in DKI Jakarta. The study design was cross sectional with 98 elders, as the samples, who were selected through quota and simple random sampling. Cognitive function was assessed by using the MMSE instrument, while the nutritional status was assessed by using body mass index. The results of this study indicated that there was a significant correlation between cognitive function with body mass index (r= 0.550; p= 0.0001; α= 0.05). Service providers in nursing home must have certain skills to improve the nutritional status of the elderly with cognitive decline. Keywords: body mass index, cognitive function, MMSE, Panti Sosial Tresna Werdha

Abstract

Kecemasan merupakan perasaan takut atau khawatir yang disebabkan oleh berbagai peristiwa yang bersifat subjektif. Penelitian ini bertujuan untuk mengetahui hubungan jumlah penyakit kronis yang dimiliki terhadap kecemasan lansia dengan penyakit kronis di wilayah binaan UPF Puskesmas Pabuaran Indah, Kecamatan Cibinong. Desain penelitian adalah deskriptif korelatif dengan pendekatan cross sectional. Sampel berjumlah 105 lansia dengan penyakit kronis,  berusia 60 tahun atau lebih yang dipilih dengan teknik cluster sampling. Instrumen yang digunakan adalah Depression Anxiety Stress Scale (DASS) skala kecemasan dan kuesioner karakteristik responden. Hasil penelitian menemukan terdapat hubungan antara jumlah penyakit kronis yang dimiliki terhadap kecemasan lansia dengan penyakit kronis, (p= 0,004) dengan kekuatan hubungan (odd ratio) 3.549. Lansia yang memiliki lebih dari satu penyakit kronis memiliki risiko 3 kali lebih besar untuk merasa cemas. Pelayanan kesehatan disarankan untuk memberikan edukasi kesehatan dan meningkatkan peran keluarga untuk mengurangi kecemasan. Abstract Multiple Chronic Diseases Increase Anxious Feeling of Community-Dwelling Elderly in Cibinong District. Anxiety is fear and worry feeling that caused by various event that subjectively. This study aimed to determine relationship amount of chronic disease towards anxiety of elderly with chronic disease in UPF Puskesmas Pabuaran Indah area, Cibinong District or Kecamatan Cibinong. Descriptive correlative design with cross-sectional approach was applied. 105 elderly with chronic disease, age 60 years or above were taken in this study, which is using cluster sampling. This research using Depression Anxiety Stress Scale (DASS): Anxiety scale and characteristics of respondent questionnaire. The results of the study found that there is a relationship between amount of chronic disease towards anxiety of elderly with chronic disease (p= 0.004) with the strength of significance (odds ratio) 3.549. Elderly with multiple chronic diseases has 3 times more of risk to feel anxious. Abstract

Older people’s health care is carried out by involving the family as a support group. If done optimally, the support provided will help maintain the health of older people. The purpose of the study was to analyze the determinant factors (family support and burden) on the quality and satisfaction of life the older people. This descriptive correlational research was the second stage of previous research related to the identification of caregivers' burden when caring for older people. A sample of 104 respondents was taken through simple random sampling. The results showed that the family support and family burden affected the older people's quality of life (< 0.05), but did not affect the older people's life satisfaction (> 0.05). Family support and burdens affect the QoL for older people, which is due to the family's ability in caring for older people and the need for long-term services at home. Recommendations for health workers, especially nurses, are to focus not only on the elderly but also to the family's physical, psychological, and financial burdens to increase family support.AbstrakFaktor Determinan dari Kualitas dan Tingkat Kepuasan Hidup Lansia. Lansia yang sehat dapat dicapai dengan melibatkan seluruh anggota keluarga sebagai support group dalam merawat lansia. Karenanya, perawatan yang optimal oleh keluarga dapat menjaga kesehatan lansia itu sendiri. Studi ini bertujuan untuk menganalisis faktor determinan (dukungan keluarga dan beban keluarga) pada kualitas hidup dan kepuasan hidup pada lansia. Penelitian deskriptif korelasional ini merupakan studi tahap kedua dari penelitian sebelumnya, yaitu studi identifikasi masalah pada beban yang dihadapi oleh caregiver. Sebanyak 104 responden sebagai sampel untuk studi ini dipilih melalui teknik simple random sampling. Penelitian ini menunjukan bahwa faktor dukungan dan beban keluarga mempengaruhi kualitas hidup lansia (< 0,05), namun tidak memengaruhi kepuasan hidup pada lansia (> 0,05). Dukungan keluarga yang memengaruhi kualitas hidup lansia terjadi karena faktor kemampuan keluarga itu sendiri dalam merawat lansia dan faktor kebutuhan jangka panjang dalam merawat lansia itu sendiri. Rekomendasi untuk para tenaga kesehatan, khususnya perawat, adalah untuk fokus tidak hanya pada lansia namun juga pada keluarga dari aspek beban fisik, mental dan finansial, untuk meningkatkan dukungan keluarga.Kata Kunci: kepuasan hidup, kualitas hidup, lansia

Abstract

Health and functional status are standard measurements in older adult’s services that showed physical and mental condition. However, institutionalized older adults with several adjustments and their limited source might cause compromised health and functional status. The quasi-experimental study aimed to investigate the effect of MiRaDaSia (nurses and caregivers joined the partnership model) on health and functional status among institutionalized older adults in Jakarta. It included 106 participants as intervention groups and 106 participants as control groups, who selected by multistage random sampling. We used the SF-12 and Barthel index to measure older-adults health and functional status. MiRaDaSia model was implemented for twelve weeks. Generally, there has been an increase in the mean of health and functional status after the intervention. There were significant improvements in functional condition between each group (p=0,001); however, mean difference oh health status show the significant increase only on six weeks following the intervention. MiRaDaSia can be implemented as a practical model to enhance services among institutionalized older-adults by professional’s staff as it encourages partnership among the nurse, caregiver, and the institutionalized older-adults. Future research may consider the effectiveness of the model in private institutional, with widening variation of older adults and caregivers’ characteristics as well as the different working environment of the institution. Abstrak Model Kemitraan Perawat, Caregiver, dan Lansia (Miradasia) Dalam Meningkatkan Status Kesehatan Dan Fungsional Lansia di Panti. Status kesehatan dan fungsional merupakan pengukuran standar yang harus dilakukan dalam menilai pelayanan kesehatan lansia yang meliputi pengkajian sampai evaluasi. Kedua pengukuran tersebut pada akhirnya menggambarkan kondisi fisik dan mental lansia. Namun, kondisi lansia yang berada di panti dengan berbagai permasalahan kesehatan dan keterbatasan sumber daya dapat menimbulkan gangguan pada status kesehatan dan fungsional lansia. Penelitian ini bertujuan untuk mengetahui pengaruh pelaksanaan model praktik Kemitraan antara perawat, caregiver, dan lansia (MiRaDaSia) terhadap status kesehatan dan fungsional pada lansia yang tinggal di panti wilayah Jakarta. Penelitian dengan desain quasi-eksperimental melibatkan 2 kelompok yang terbagi menjadi 106 partisipan di kelompok intervensi serta 106 partisipan di kelompol kontrol. Pemilihan sampel dilakukan melalui multistage random sampling dengan alat pengukuran berupa SF-12 dan Barthel index untuk melihat staus kesehatan dan fungsional lansia. Model praktik keperawatan MiRaDaSia diimplementasikan selama 12 minggu pada kelompok intervensi. Secara umum, hasil penelitian menunjukkan bahwa terdapat peningkatan status kesehatan maupun fungsional status setelah intervensi model MiRaDaSia. Status fungsional secara signifikan mengalami peningkatan antara kelompok perlakuan (p=0,001) namun, beda reratastatus kesahatan menunjukkan peningkatan signifikan pada pengukuran 6 minggu setelah intervensi. terdapat peningkatan rerata status kesehatan setelah intervensi. Status kesehatan dan fungsional pada lansia dipengaruhi Model praktik keperawatan MiRaDaSia dapat diimplementasikan sebagai model praktik untuk meningkatkan pelayanan lansia oleh petugas maupuun tenaga profesional pada setting panti, karena memberikan penguatan pada kemitraan antara perawat, caregiver, dan lansia. Penelitian yang akan datang sebaiknya perlu mempertimbangkan penerapan model prakting di setting panti swasta, dengan variasi karakteristik lansia dan caregiver yang lebih banyak maupun lingkungan kerja institusi, untuk mengetahui lebih jauh tentang efektivitas model. Abstract

Penurunan fungsi sistem muskuluskeletal pada lansia berakibat pada meningkatnya kejadian jatuh. Hal tersebut menimbulkan perasaan takut jatuh lansia. Penggunaan alat bantu jalan merupakan salah satu faktor resiko peningkatan perasaan takut jatuh. Penelitian ini bertujuan untuk menganalisis hubungan antara alat bantu jalan dengan perasaan takut jatuh. Desain penelitian yang digunakan adalah cross sectional dengan melibatkan 111 lansia di Kota Depok dengan menggunakan multistage random sampling. Perasaan takut jatuh dinilai dengan menggunakan instrument Falls Efficacy Scale- International (FES-I) dan analisis bivariat menggunakan uji statistic chi square. Hasil penelitian menunjukan ada hubungan antara penggunaan alat bantu jalan dengan perasaan takut jatuh (p value 0,0001). Perawat perlu mengetahui cara melakukan skrining dan mengenal faktor resiko perasaan takut jatuh pada lansia.

Abstract

The elderly are an age group presenting a substantial risk of experiencing sleep problems. About 81.6% of the elderly from the author’s previous research showed poor sleep quality contributed to various diseases. The current approaches regarding sleep hygiene in society are keen to start mostly with an alternative or complementary therapy rather than consuming sleeping pills. Previous studies showed the possibility of strengthening productivity by combining three-intervention of nursing to relax the body and stimulate the brain’s waves for sleeping. This case study aimed to analyze the application of evidence-based practice as the primary intervention in dealing with Insomnia elderly at one of the social institutions in 2019. The procedure was a combination of aromatherapy, hand massage, and music therapy for 30-minute each repetition five times a week for five weeks at the same time for three women with sleep disorders: a Chinese senior (Mrs. S, 79 y/o), a wheelchair user (Mrs. N, 76 y/o), and well-spoken elderly (Mrs. A, 60 y/o). The therapy was measured subjectively and objectively amid participants’ opinions, achievement of nursing outcomes, and the Pittsburgh Sleep Quality Index (PSQI) instrument within three periods. The assessment in PSQI showed a significant-decreasing score for each participant: (1) 19-9-6, (2) 13-9-5, and (3) 18-10-5, respectively, during the pre-mid-post assessment. This modified intervention was provided to enhance the effectiveness of therapy from the previous study such as the lengthy duration, frequency, and scheduling.  Social institutions as a form of service for the elderly can encourage nurses or nursing students to apply this combined intervention to overcome the problem of Insomnia. The recommendation is to continue playing passive music at night as a bedtime ritual regularly performed each day. The benefits are reducing pre-bedtime (sleep onset latency), improving sleep quality, and allowing older people to be better and more productive during the day.

Abstract

Urinary incontinence (UI) is a prevalent condition that disproportionately affects institutionalized older adults and manifests differently across genders. This study aimed to examine gender-based risk factors associated with UI among older adults living in nursing homes. A cross-sectional study was conducted involving 317 older adults from four government-run nursing homes. Participants were selected using purposive sampling based on inclusion criteria. Participants completed the Questionnaire for Urinary Incontinence Diagnosis (QUID), the Geriatric Depression Scale (GDS), and the Pelvic Floor Distress Inventory-20 (PFDI-20). Data were analyzed using descriptive statistics and logistic regression across three models (overall, female, and male). The prevalence of UI was 28.07%, with higher rates among females (15.77%) compared to males. Pelvic floor muscle weakness was significantly associated with UI in all models. Among women, reproductive factors such as a menopause duration exceeding 10 years and a history of childbirth were also significantly associated with an increased risk of UI. Education level emerged as a significant confounding variable in the general model. Pelvic floor muscle weakness is a key gender-linked risk factor for UI, particularly among women with specific reproductive histories. These findings underscore the importance of incorporating gender-responsive assessments and pelvic floor interventions into institutional geriatric care practices.

Abstract

Functional status means the ability to perform self-care, self-maintenance, and daily activity. Older adults are at risk of experiencing a decline in functional status regarding their condition and other factors. The study literature found no clear evidence of the factors related functional status of older adults, especially those who have comorbid and regularly come to the geriatric clinic in the Primary Health Care Center. This study aimed to identify the prevalence and factors related to the functional status of older people who visit geriatric out-patient in the primary health care center. A cross-sectional design was utilized to assess 100 samples of older adult patients in one Primary Health Care Center, in Indonesia. The functional status of the respondent was good where the mean score was 7.45 (SD = 1.07) with a minimum score was 2, and a maximum score was 8. The mean age of the sample was 66 years old, the gender was primarily women (59%), the education level of the respondents was more likely only reach elementary school (45%), and primarily they consume drugs (71%) and also more than a half of respondent have multimorbidity (60%). The model fit of multivariate analysis resulted in 44% of all the variables giving effect to the functional status where the most influential factor was balance and health status of the older adults. Discussion: More than half of the older people who regularly visit the Primary Health Care Center have an excellent functional status. This condition is in accordance with the experience of older people itself who most of them could come to the clinic independently by public or private transportation. Older people who have good health or balance status and less morbidity also show a good performance in functional status, though, their age was older. All the variables give 44% effects of older people’s functional status where the most influential factor for functional status of the older adults was balance status. Therefore, the accessibility of health services for the older adult in the community can be improved by providing more elderly-friendly health facilities.
