Abstract

Penelitian ini bertujuan untuk mempelajari pengaruh gelombang ultrasonik intensitas rendah terhadap diferensiasi sel osteoblas, yaitu dalam hal morfologi, deposisi kalsium dan produksi osteokalsin sebagai pertanda spesifik terhadap aktivitas osteoblas. Penyembuhan fraktur telah terbukti dapat dipercepat dengan rangsangan gelombang ultrasonik intensitas rendah pada penelitian klinik dan percobaan binatang. Pengaruh gelombang ini melalui satu atau lebih jalur molekular, sehingga menimbulkan pengaruh biologi yang hingga saat ini belum diketahui mekanismenya. Kultur sel osteoblas manusia (NHOst CC-2538) dibiakkan menjadi 20 cawan, secara random dibagi menjadi kelompok yang dirangsang dengan gelombang ultrasonik intensitas rendah dan yang tidak dirangsang. Kultur sel ini dibiakkan pada inkubator dengan suhu udara 37°C, kadar CO2 5% sampai tercapai konfluensi 70%. Rangsangan gelombang ultrasonik dengan frekwensi 1MHz, 0,2 W/cm2, selama 3 menit pada hari ke 1, 3, dan 5. perubahan morfologi diamati dengan mikroskop fase kontras, deposisi regulator tulang dideteksi dengan Western blottting. dari hasil penelitian didapatkan bahwa rangsangan gelombang ultrasonik intensitas rendah mempercepat pembentukan nodul tulang (P<0.05). Klarifikasi dan produksi osteokalsin terbentuk lebih cepat dan lebih banyak dibandingkan tanpa rangsangan gelombang ultrasonik. Dapat disimpulkan bahwa gelombang ultrasonik intensitas rendah dapat mempengaruhi percepatan fungsionalisasi osteoblas.

Abstract

ABSTRACT Death  caused  by  cardiovascular  and  cerebrovascular diseases  are  still  the  highest.  Atherosclerosis  due  to hypercholesterolemia is a predispositioning factor of both diseases. Curcumin has an antioxidant character that can inhibit lipid peroxidase to atherogenesis process. The aim of this study was to observe the effect of curcumin in decreasing total cholesterol level, LDL-cholesterol, the number of  F2-isoprostan and foam cell of aorta’s wall of white rats strain wistar after giving atherogenic diet for 10 weeks. There were 6 groups in this study (n=24): atherogenic diet group (group I as positive control), atherogenic diet group + curcumin 50 mg/kg b.w. per day (group II), atherogenic diet group + curcumin 100 mg/kg b.w. per day (group III), atherogenic diet group + curcumin 200 mg/kg b.w. per day (group IV), atherogenic diet group + curcumin 400 mg/kg b.w. per day (group V), normal diet group (group VI as negative control). After 10th, the level of total cholesterol and LDL-cholesterol were measured by spectrofotometri. Staining F2-isoprostan was done with Avidin Biotin Complex imunohystochemistry method, and staining foam cell with HE-oil Red O. The number of  F2-isoprostan and foam cells were counting semi quantitatively using  light microscope. The highest total cholesterol level was found in group I (275,15 ± 10,01; Mean ± SD), the highest LDL-cholesterol level was found in group I (158,15 ± 12,19). Statistically total cholesterol level and LDL-cholesterol level in group I were significantly higher (p ≤ 0,05) than other

Abstract

ABSTRACT Fracture healing relative require more times.  Many effort have been done to shorten the time, including stimulation with low intensity ultrasonic.    Invitro  studies  and  animal  experimentation  have  been  proved  in  various  results.  Physical  therapy  equipments  in  the hospitals using ultrasonic wave posible to stimulate process of fracture healing. Clinical experimental study was done in our series, the samples was divided randomly into 2 groups.  One group was stimulated with low intensity ultrasonic from Sonopuls-591 with 1 MHz frequency, 0,2 W/cm2 intensity, for 15 minutes.  The other group withuot stimulation as the control group.  The variable were pain, sticky, and callus formation were evaluated perodically. The Results were the degrees of pain in control group higher than control. The bony union (sticky) was no difference within both group.  The callus formation was higher in the treatment group (p = 0,069). Our Conclusion is 15 minutes Low intensity ultrasonic stimulation, that was given 10 times within 3 days time interval was significanly  increase callus formation (p = 0,069). Abstract

Dicephalus Dipus Tetrabrachius is an extremely rare  form of conjoined twinning in which the infant has  four arms, two legs, one trunk, but two heads. The case that was born in Malang was girls had four arms that two of the arms had fused at the mid line. The case was delivered by caesareansection, due to pre-eclampsia and prolonged labor.Prior antenatal care was never done; the babies were born without any suspicion of conjoined twin. According to literature almost sixty percent of conjoined twins are either stillborn or lost in uterus, and 35% of twins that survive die within the first 24 hours. Antenatal diagnosis by ultrasound is possible in modern day obstetrics. The babies were born on Oct. 21st, 2006. The treatment planning were determined by degree of fusion and extent of joining of fetal organs. Surgicalseparation was not possible; the surgical correction is removal of fused arms in the near future, as an alternative plan for improving quality of life. Abstract

Abstract: A Maisonneuve’s Fracture is a fracture of proximal third fibula associated with a disruption of the distal tibiofibular syndesmosis, the interosseous membrane and associated injuries (eg, fracture of the medial malleolus, fracture of the posterior malleolus, and rupture of the deltoid ligament). The mechanism for the fracture is an external rotation force to the ankle with transmission of the force through the interosseous membrane which causes a proximal fibular fracture.In a very rare form, we can found an unusual pattern of the level of the involved fibula, such as middle third of fibula. A 19 year old male  presented to us with pain and swelling of his left ankle after felt while playing futsal. Physical examination showed deformity, swelling and tenderness on his left ankle. Range of motion was decreased. On imaging, plain radiographs of left ankle showed dislocation of the ankle, with disruption of syndesmotic and fracture comminutive middle third of fibula. We performed operation which are consists of three procedures; closed reduction, percutaneous fixation of syndesmotic by using a single transfixing screw, and plate-screw for the fibula. Normal range of motion is achieved well after 8 weeks, without pain on full weightbearing. The pattern of the fracture-dislocation of the ankle discussed is very rare. The mechanism of the injury is a twisting motion of the upperbody inward, while the foot is planted, resulting a more higher energy impacted to the ankle and fibula. Abstract

Promoting breastfeeding self-efficacy through self-directed learning requires behavior, goal setting, and self-reinforcement. This research aims to collect insights from health professionals on strategies for improving maternal confidence in breastfeeding using self-directed learning and existing knowledge. An in-depth exploration through a two-round Delphi method rooted in the self-efficacy theory of self-directed learning for breastfeeding mothers was conducted, involving expert input and an extensive literature review. Four key documents were identified, each undergoing rigorous expert rating to ensure quality. Six essential elements for health professionals to guide breastfeeding mothers were established, focusing on lactation physiology, successful initiation, confidence building, adversity management, cultural beliefs, and public breastfeeding. Three crucial topics, including prior knowledge, personal attributes, and autonomous processes, were designed to enhance self-efficacy through self-directed learning. In conclusion, the study emphasizes the vital role of health professionals in supporting mothers through comprehensive breastfeeding guidance and encouraging self-directed learning.
