Abstract

Role of gene P53 as a tumor suppressor gene in generating primary liver cancer is very important primarily in cellgrowth regulation and apoptosis. By using AS-PCR method, the gene mutation can be recognized. It has been reported a chronichepatitis B and C case with experiencing mutation of gene P53 Codone 249 in liver tissue of a 33 years old male with anti HCVpositive, HBsAg positive, on the liver biopsy indicated of chronic hepatitis without cirrhotic features and malignancy signs. OnAS-PCR examination found mutation on gene P53 codone 249. This finding was expected to be an early warning sign foroccurrence of primary liver cancer, so that, early intervention could be performed.

Abstract

Gastroesophageal reflux disease (GERD) adalah suatu penyakit yang ditandai dengan naiknya asam lambung ke kerongkongan yang menyebabkan keluhan heartburn dan regurgitasi. Prevalensi GERD di seluruh dunia bervariasi sekitar 13%. GERD sering dikaitkan dengan penurunan kualitas tidur yang berakhir dengan penurunan produktivitas di siang hari. Refluks asam lambung di malam hari berkaitan dengan berbagai komplikasi seperti barret’s esofagus dan adenokarsinoma esofagus. Penelitian ini bertujuan untuk mengetahui hubungan antara GERD dan kualitas tidur pasien dengan menggunakan desain studi cross sectional dan dianalisis menggunakan uji Chi Square. Sampel dipilih secara consecutive sampling sebanyak 104 pasien di Rumah Sakit Umum Daerah Sanjiwani Gianyar, Bali, Indonesia dari bulan Mei 2022 hingga Juli 2022. Instrumen penelitian berupa pittsburgh sleep quality indeks (PSQI) dan GERD-questionnaire (GERD-Q). Hasil penelitian menunjukkan p value=0,002 (p< 0.05). Simpulan, GERD memilki hubungan terhadap kualitas tidur pasien rawat jalan di RSUD Sanjiwani Gianyar secara signifikan. Pemberian proton pump inhibitor (PPI) disertai penerapan gaya hidup yang sehat dapat meperbaiki keluhan GERD dengan atau tanpa gangguan tidur.

Abstract

Background: Gastrointestinal disorders are a common health problem in Indonesia. Endoscopy is an important procedure in diagnosing and managing this disorder. This study aims to analyze risk factors and predictors of endoscopic findings in patients with gastrointestinal disorders at regional hospitals in Bali. Methods: This research is an analytical observational study with a cross-sectional design. The research subjects were patients with gastrointestinal disorders who underwent endoscopy at regional hospitals in Bali during the period January 2022 to December 2023. Data was collected through medical records and interviews. Data analysis was carried out using the Chi-square test and logistic regression. Results: A total of 1074 patients were included in this study. The mean age of patients was 45.3 years (SD 15.2). The most common endoscopic finding was gastritis (35.2%), followed by gastric ulcer (15.8%) and esophagitis (12.4%). Risk factors that were significantly associated with endoscopic findings were age > 45 years (OR 1.78; 95% CI 1.23-2.57), male gender (OR 1.54; 95% CI 1.11- 2.14), history of smoking (OR 2.31; 95% CI 1.65-3.24), and history of use of non-steroidal anti-inflammatory drugs (NSAIDs) (OR 1.98; 95% CI 1.39-2.82). Conclusion: Age > 45 years, male gender, smoking history, and history of NSAID use are risk factors that are significantly associated with endoscopic findings in patients with gastrointestinal disorders at regional hospitals in Bali.

Abstract

Background: Dengue hemorrhagic fever (DHF) is a viral infection transmitted by Aedes mosquito with a wide clinical spectrum, ranging from mild fever to life-threatening complications. Atypical clinical manifestations of DHF can cause delays in diagnosis and treatment, so it is important to increase clinician awareness of these unusual manifestations. Case presentation: We report four cases of DHF with atypical manifestations: (1) A 50-year-old woman with spontaneous psoas hematoma, (2) An elderly man with pulmonary edema, (3) A young man with acute myocarditis, and (4) A middle-aged man. middle-aged with encephalopathy. Conclusion: Atypical manifestations of DHF can occur at various ages and can involve various organs. It is important for clinicians to consider dengue fever as a differential diagnosis in patients with fever and unusual clinical manifestations, especially in dengue-endemic areas. Early diagnosis and appropriate treatment can improve patient clinical outcomes.

Abstract

The retail industry generates an extremely large and continuously growing volume of transactional data along with the advancement of digital technology, thereby requiring sophisticated and systematic data analysis approaches to support effective and evidence-based business decision-making. This study aims to analyze retail sales data by utilizing the Retail Sales Dataset obtained from the Kaggle platform, which consists of 100,000 transaction records and broadly represents the characteristics of retail transactions. The main focus of this study is to classify product categories and predict customer segments, including the identification of high-spending customers (high spenders), based on demographic attributes such as age and gender, as well as various transaction-related features. The research methodology includes data preprocessing, label encoding, and feature engineering to generate additional variables, including Age_Group, Is_Holiday, and Spender_Group, which are expected to enhance the predictive capability of the models. Several machine learning algorithms, namely Decision Tree, Random Forest, and XGBoost, were implemented and evaluated to compare their respective performance. The experimental results indicate that multiclass product category classification achieves relatively low accuracy, ranging from 27% to 34%. These findings suggest the high complexity of retail data and highlight the need for further model optimization, class balancing techniques, and feature refinement to improve predictive performance in future studies.

Abstract

Background : HIV patients have a life expectancy that is close to the life expectancy of the general population after using ARVs, so that increasing HIV patients suffer from age-related diseases and chronic inflammation, one of which is sarcopenia. Research on sarcopenia in HIV patients in Indonesia is still limited, so the author is interested in conducting this study with the aim of determining the prevalence and risk factors for sarcopenia in HIV patients. Method : A cross-sectional study conducted at Sanjiwani Hospital Gianyar on October 2025. This study included 125 HIV patients, aged 20-75 and receiving ARV for more than a year. Sarcopenia was determined based on the 2019 Asian Working Group for Sarcopenia (AWGS) criteria, health status based on questionnaires, measurments, and medical records. Logistic regression analysis was used to determine the risk of sarcopenia occurrence. Result : A total of 125 sampel consisted of 40.8% women, and 59.2% men with an average age of 48.35 ± 10.8 years. Most of them had suffered from HIV for more than 5 years (60%). The prevalence of sarcopenia was 20.8%. Risk factors for sarcopenia included advanced age (≥50 years, OR =6,01, female gender (OR=5,07). Conclusion : This prevalence of sarcopenia in HIV patients was 20.8%, advance age, female gender are risk factors for sarcopenia in HIV patients.
