Abstract

Medicaction error is an important problem in the health sector that can increase mortality, increase the duration of hospitalization, and increase the cost of treatment. Identifying the risk factors for medication errors is an important first step in preventing medication errors. This study aimed to identify the relationship between patient characteristics and medication errors in inpatients at clinic x. The study was conducted qualitatively with a cross-sectional design with recorded data. The study was conducted using clinical record data from July to December. The study sample consisted of 196 patients who met the inclusion criteria with a consecutive sampling technique. Analysis of the relationship between variables was carried out by chi-square analysis with a significance value of 5% (0.05). The results showed that inpatients at clinic x experienced medication errors of 17.86% which consisted of prescribing errors of 2.55% and administrative errors of 15.31%. Patient gender was not significantly associated with medication errors (p> 0.05). Patient age was significantly associated with medication errors (p<0.001). The number of drugs was significantly associated with medication errors (p<0.001). Length of stay was not significantly associated with medication errors (p < 0.05). Drug class was significantly related to medication error. The conclusion is that the patient's age, the number of drugs received by the patient and the class of drugs are factors related to the incidence of medication errors. The patient aspect also contributes to the risk factor for medication errors so that it can be considered in preventing medication errors in health practice by health workers.

Abstract

Background: Case Midwifery Notes (CMNotes) is a digital-based documentation system designed to develop learning methods and interactive consultations between students and lecturers as well as clinical instructors. CMNotes facilitates digital and comprehensive recording of patient data starting from assessment, diagnosis, plan of care, implementation to evaluation by applying Varney's management principles and SOAP documentation. Research Objectives: Identifying needs and Designing a Prototype Case Midwifery Notes (CMNotes) documentation system website-based Research Place: Langsa Midwifery Diploma-III Study Program, Aceh, Indonesia Research Design: Research And Development (R&D) to design a website-based prototype of Case Midwifery Notes (CMNotes) documentation system of Maternity Care. Population and Sample: The samples in this study is a total population of 45 respondents. Data collection techniques in this study were interviews and filling out questionnaires. Descriptive data analysis and presented in narrative form Results: The results of the interviews showed that all respondents wanted a digital-based midwifery care documentation application system that could replace the paper-based documentation method. The features available in CMNotes cover all aspects of a standardized midwifery format. The interface in the application is made to be user-friendly and attractive and able to store data properly. There needs to be further development of time efficiency, and features that can analyze important data in the history of pregnancy and childbirth. Kesimpulan : Prototipe CMNotes telah mencakup semua aspek dalam format standar kebidanan dan dapat memudahkan pengguna untuk mendokumentasikan asuhan kebidanan secara digital. Perlu dikembangkan sistem di CMNotes untuk menghitung Interpreted Delivery Date (TTP) dan Usia Kehamilan. Kata kunci: Sistem Dokumentasi, Perawatan Bersalin, Prototipe, Website, CMNotes

Abstract

The low level of public knowledge about healthy lifestyles is one of the causes of the increasing incidence of Diabetes Mellitus, which is characterized by a continuous increase in blood glucose levels that can be complicated. The role of pharmacists can provide real evidence in reducing the incidence and morbidity of patients. Community Service Activities are carried out in the Sukorame Village community, Kediri City with the aim of increasing knowledge and insight into healthy lifestyles to prevent diabetes mellitus. Community Service Activities will carry out several community empowerment activities, namely: (1) Providing an introduction to diabetes mellitus and healthy lifestyles to prevent diabetes mellitus, (2). Conducting socialization on how to use insulin properly and correctly, (3). Providing assistance to the Sukorame Village community, Kediri City related to the proper use of insulin. The results of this activity were measured by pretest and posttest where knowledge about Diabetes Mellitus increased significantly and they were able to use insulin independently.
