Abstract

Introduction: Depression is a common condition in epilepsy patients, especially temporal lobe epilepsy both before and after epilepsy surgery. However, not many studies have examined the depression status of patients undergoing surgery outside the temporal lobe (extratemporal resection). Differences in levels of depression between temporal lobe epilepsy (TLE) and extra-TLE patients are also not well known, so the effect of surgery on mood disorders in this group of epilepsy patients needs to be further investigated. Aim: This study aims to study whether there are differences in depression rates for TLE  and extra-TLE patients in case studies of post-epileptic surgery patients. Method: The design of this study was cross sectional by conducting research on patients with temporal lobe epilepsy (n = 29) and extra temporal epilepsy patients (n = 24) from 1999 to 2019. The study was conducted at the Hospitals in Semarang. Researchers recorded depression levels using the Beck Depression Inventory - II (BDI-II) after the patient had surgery. The collected data will be analyzed using the parametric 2 independent samples t test. Results: 53 patients who were subject were 29 TLE patients (54.7%) and 24 Extra TLE patients (45.3%). TLE patients who had normal BDI-II results were 14 patients (48.3%), mild depression by 6 patients (20.7%), moderate depression by 8 patients (27.6%) and severe depression by 1 patient (3,4%) while the Extra TLE patients showed normal results of 11 patients (45.8%), mild depression of 8 patients (33.3%), moderate depression of 4 patients (16.7%) and severe depression of 1 patient ( 4.2%). 2 independent samples t test showed insignificant results, namely p = 0.831. Conclusion: There are differences of depression level between TLE and Extra TLE cannot be confirmed, because there are many factors such as the duration of epilepsy and seizure frequency that influence patients. Multifactorial explanation for the depression in patients with TLE and extra TLE must be investigated.Keywords: TLE (Temporal Lobe Epilepsy), extra-TLE, Depression

Abstract

Latar Belakang : Insidensi disfungsi ereksi meningkat di Indonesia. Penyebab disfungsi ereksi merupakan multifaktorial; psikogenik, organik, neurogenik, hormonal dan vaskular. Faktor risiko lainnya yang masih diperdebatkan adalah bersepeda. Banyak sekali mitos bahwa bersepeda dapat mengganggu mekanisme veno-oklusi corporal yang merupakan mekanisme utama dalam terjadinya ereksi, sehingga terjadi disfungsi ereksi. Tujuan : Menganalisis hubungan trauma perineal berulang terhadap derajat disfungsi ereksi pada pesepeda di Kota Semarang. Metode : Penelitian ini menggunakan jenis penelitian analitik kuantitiatif dan rancangan penelitian cross sectional. Sampel yang digunakan adalah 89 responden yang diambil sesuai kriteria inklusi dan eksklusi penelitian dan responden bersedia berpartisipasi dalam penelitian. Data yang diambil adalah hasil kuesioner IIEF-5 (International Index of Erectile Function -5). Data yang telah dikumpulkan dilakukan pengeditan dan pengodean dan kemudian dilakukan pengolahan data secara statistik Chi-square. Hasil : Didapatkan hasil yang bermakna antara derajat disfungsi ereksi dengan usia, lama bersepeda, frekuensi bersepeda, durasi bersepeda, dan jenis saddle dengan nilai p masing-masing yakni 0.023, 0.046, 0.049, 0.042, 0.049. Lalu didapatkan hasil yang tidak bermakna antara derajat disfungsi ereksi dengan penggunaan padded cycling short, ketinggian handle bar, dan IMT, dengan nilai p masing-masing yakni 0.418, 0.086, dan 0.373. Simpulan : Terdapat hubungan yang bermakna antara trauma perineal berulang pada aspek usia, lama bersepeda, frekuensi bersepeda, durasi bersepeda dan jenis saddle yang digunakan terhadap derajat disfungsi ereksi pada pesepeda. Tidak terdapat hubungan yang bermakna antara penggunaan padded cycling short, ketinggian handle bar, dan IMT terhadap derajat disfungsi ereksi pada pesepeda.Kata Kunci : Disfungsi ereksi, trauma perineal berulang

Abstract

Latar Belakang : Batu saluran kemih merupakan masalah kesehatan yang cukup besar, menempati urutan ketiga setelah Benign Prostate Hyperplasia (BPH) dan Infeksi Saluran Kemih. Insidensi batu saluran kemih dapat disebabkan oleh berbagai faktor seperti konsumsi tinggi kalsium dan oksalat, intake cairan yang kurang, infeksi saluran kemih atau oleh karena drainase urine yang tidak adekuat seperti pada BPH.Tujuan : Mengetahui kejadian batu saluran kemih pada pasien Benign Prostate Hyperplasia (BPH) periode Januari 2013 – Desember 2015 di RSUP Dr. KARIADI Semarang.Metode : Penelitian ini merupakan penelitian deskriptif dengan menggunakan data sekunder. Cara pengambilan sample dilakukan dengan metode total sampling dengan menggunakan kriteria waktu penelitian periode Januari 2013 – Desember 2015.Hasil : Dari hasil penelitian didapatkan dari 255 sampel pasien Benign Prostate Hyperplasia (BPH) terdapat 25 sampel yang mengalami kejadian batu saluran kemih. Frekuensi terbanyak kejadian batu saluran kemih pada pasien Benign Prostate Hyperplasia (BPH) pada usia 65-69 tahun 15 sampel (60%), dengan keluhan terbanyak sulit buang air kecil (BAK) sebesar 18 sampel (72%) dan frekuensi kejadian terbanyak pada kelompok pekerjaan terjadi pada pekerja wiraswasta sebesar 22 sampel (88%).Kesimpulan : Dari hasil penelitian, dapat disimpulkan bahwa kejadian batu saluran kemih masih dapat ditemukan pada sebagian kecil pasien Benign Prostate Hyperplasia (BPH).

Abstract

Background: Acellular therapies from Mesenchymal Stromal Cells (MSCs), including the full secretome (conditioned medium, CM) and purified small extracellular vesicles (sEVs), are promising restorative treatments for erectile dysfunction (ED). It remains unknown if the therapeutic benefit is driven by the complete secretome or if purified sEVs are the primary, sufficient component. This study aimed to systematically review and meta-analyze the preclinical evidence. Methods: We conducted a systematic review and parallel meta-analysis adhering to PRISMA guidelines. PubMed, Scopus, and Web of Science were searched from January 1st, 2014, to July 31st, 2025. Studies were eligible if they were preclinical ED models evaluating MSC-CM or purified sEVs against a control. Two parallel meta-analyses were performed using a random-effects model. Primary outcomes were erectile function (Intracavernous Pressure / Mean Arterial Pressure ratio; ICP/MAP) and histopathology (Smooth Muscle / Collagen ratio; SM/Col). Results: Our search yielded 1,942 records, with 87 full-text articles assessed. After applying strict PICO criteria, 7 primary studies were eligible for the meta-analysis (3 secretome, 4 sEVs). The overall risk of bias was moderate to high (0% allocation concealment). No studies directly compared secretome versus sEVs. The first meta-analysis (Secretome vs. Control, 3 studies, 4 data points, n=70) demonstrated a large, significant improvement in ICP/MAP (Standardized Mean Difference [SMD]: 2.40; 95% CI [1.65, 3.15]; p-value < 0.001), with extreme heterogeneity (I-squared=85%). The second meta-analysis (sEVs vs. Control, 4 studies, n=68) also showed a large, significant improvement (SMD: 2.75; 95% CI [1.90, 3.60]; p-value < 0.001), also with extreme heterogeneity (I-squared=88%). Conclusion: Both the full MSC secretome and purified sEVs demonstrate large, significant therapeutic effects. However, this quantitative conclusion is severely limited by the exceptionally small number of studies and the profound biomolecular heterogeneity (in cell source and purification) that invalidates direct comparison. The primary finding remains the total lack of comparative data.
