Abstract

Gigi tiruan sebagian lepasan (GTSL) bertujuan untuk menggantikan gigi yang hilang serta jaringan pendukungnya, dapat dipasang dan dilepas sendiri oleh pemakainya. Jaringan periodontal mempunyai batas ambang dalam menahan tekanan oklusi, bila tekanan ini berlebihan dapat mencederai jaringan periodontal. Pasien diinstruksikan untuk menjaga kebersihan gigi tiruan dan gigi penyangga. Bila GTSL tidak dibersihkan dapat mengakibatkan akumulasi debris pada bagian gigi penyangga dan konektor minor, sehingga mengakibatkan terjadinya inflamasi gingiva. Penelitian ini merupakan penelitian deskriptif analitik dengan menggunakan rancangan penelitian cross sectional. Sampel pada penelitian ini adalah pasien yang membuat gigi tiruan sebagian lepasan di Rumah Sakit Gigi dan Mulut Universitas Sumatera Utara sejak bulan Juni 2014 sampai bulan Juli 2015 yang berjumlah 100 orang. Subjek penelitian dilakukan pemeriksaan klinis berupa pemeriksaan kelainan periodontal, traumatik oklusi dan oral hygiene. Subjek penelitian yang mengalami kelainan periodontal dibagi menjadi 4 kelompok derajat keparahan yaitu gingivitis sederhana, periodontal destruksi tahap awal, periodontal destruksi tahap mantap, dan penyakit pada tahap akhir. Hasil penelitian menunjukkan ada hubungan yang signifikan antara derajat keparahan kelainan periodontal (gingivitis sederhana, periodontal destruksi tahap awal, periodontal destruksi tahap mantap, dan penyakit pada tahap akhir) dengan traumatik oklusi dengan nilai p = 0,011.

Abstract

Oklusi fungsional terbagi atas gerakan lateral dan protrusif. Kaninus dan gigi posterior berkontak saatpergerakan lateral. Kontak saat gerakan lateral disebut skema oklusi. Hubungan antara skema oklusi danGangguan Sendi Temporomandibular (STM) masih diperdebatkan dalam kedokteran gigi. Etiologigangguan STM terbagi atas faktor perpetuasi seperti jenis kelamin, faktor predisposisi seperti usia dan skemaoklusi. Penelitian ini bertujuan untuk melihat prevalensi dan hubungan antara jenis kelamin, usia dan skemaoklusi dengan gangguan STM. Jenis penelitian ini adalah deskriptif analitik dengan metode cross sectional.Sampel penelitian ini adalah mahasiswa FKG USU. Jumlah sampel sebanyak 100 orang. Subjek diperiksadengan RDC/TMD dan pemeriksaan skema oklusi dengan shim stock. Kemudian dianalisis dengan uji chisquare dan kolmogorov smirnov. Hasil dari penelitian ini menunjukkan tidak ada hubungan antara jeniskelamin dan skema oklusi dengan gangguan STM, namun terdapat hubungan antara usia dengan gangguanSTM. Penelitian ini juga menunjukkan adanya subjek yang memiliki kombinasi antara skema oklusi canineguidance dan group function.

Abstract

Pendahuluan: Pemakaian gigi tiruan lengkap prevalensinya lebih tinggi pada usia lanjut daripada usia muda. Perubahan fisik yang terjadi pada usia lanjut yang memakai gigi tiruan lengkap akan berpengaruh pada kualitas dan kuantitas saliva serta resorpsi tulang alveolar sehingga berkurang kekuatan retensi gigi tiruan lengkap. Denture adhesive digunakan untuk meningkatkan kekuatan retensi gigi tiruan lengkap. Tujuan penelitian ini adalah untuk menganalisis pengaruh penggunaan denture adhesive sediaan powder dan cream terhadap kekuatan retensi pada gigi tiruan lengkap. Metode: Jenis penelitian ini adalah eksperimental dengan rancangan penelitian uji klinis. Penelitian ini dilakukan pada gigi tiruan lengkap rahang atas yang telah diberikan hook dengan menggunakan alat push and pull gauge pada 10 sampel setiap kelompok dan dibagi tiga kelompok (kelompok tanpa denture adhesive, denture adhesive powder dan denture adhesive cream). Data dianalisis dengan uji ANOVA dan dilanjutkan uji LSD. Hasil: Hasil uji statistik ANOVA menunjukkan adanya pengaruh penggunaan denture adhesive powder dan denture adhesive cream terhadap kekuatan retensi (p = 0,0001, <0,05). Hasil uji LSD menunjukkan adanya perbedaan pengaruh penggunaan denture adhesive sediaan powder dan cream terhadap kekuatan retensi, terlihat perbedaan yang signifikan antara kelompok Tanpa Denture Adhesive dengan Denture Adhesive Powder dan Denture Adhesive Cream, tetapi tidak signifikan antara denture adhesive powder dan denture adhesive cream. Simpulan: Kekuatan retensi pada denture adhesive sediaan cream lebih tinggi dibandingkan denture adhesive sediaan powder sehingga penggunaan denture adhesive sediaan cream lebih direkomendasikan pada pasien yang membutuhkan denture adhesive.Kata kunci: Retensi, gigi tiruan lengkap, denture adhesive. ABSTRACTIntroduction: The use of complete denture is more commonly found in elderly than the younger age. Physical changes that occur in elderly complete denture wearers will affect the quality and quantity of saliva and alveolar bone resorption so that retention strength of complete dentures will be reduced. Denture adhesive is used to increase the retention strength of complete dentures. The purpose of this study was to analyse the effect of using denture adhesives with the various form of powder and cream on the retention strength in complete dentures. Methods: This research was experimental research with a clinical trial research design. This study was measured on the maxillary complete denture that had been hooked using the push and pulls gauges towards 10 samples of each group, then divided into three groups (no denture adhesive; denture adhesive powder; and denture adhesive cream). Data were analysed by ANOVA test then continued by LSD test. Results: Based on the ANOVA test results, the influence of the use of denture adhesive powder and denture adhesive cream on the retention strength (p = 0.0001; p < 0.05). The LSD test results showed a difference in the effect of using denture adhesive powder and cream form on the retention strength of complete denture, also, a significant difference between the group with no denture adhesive with denture adhesive powder and cream form groups. However, there was no significant difference between denture adhesive powder and cream form groups. Conclusion: The retention strength of denture adhesive cream form is higher than powder form; thus, the use of denture adhesive cream form is more recommended in patients who need a denture adhesive.Keywords: Retention, complete denture, denture adhesive.

Abstract

Loss of eye does not only affect facial esthetics but also psychological health of the patient and leads to social disa-bility. A custom-made ocular prosthesis is a good alternative to promote physical, psychological and esthetically plea-sing appearance that can improve social acceptance of the patient. A 5-years-old female patient and 18-years old male patient were reported to Dental Hospital USU with the chief complaint of loss of an eye, making them often in-sulted by schoolmates, while the male patient lost confidence on socializing around. In these cases, ocular prosthe-ses with modification of custom tray was made by using a putty index obtained from wax pattern to produce a better fitting ocular prosthesis so it expected to be retentive as to produce comfort and increase patient confidence. Con-tact between ocular prosthesis and tissue bed is necessary to evenly distribute the pressure obtained with proper im-pression technique. This technique ensures a good fit of the custom tray thereby produce accurate adaptation to the tissue surface can increase the movement of the prosthesis and provide a good natural esthetic outcome. Post evi-sceration patients need psychological support to restore confidence and self-esteem in today's cosmetics challenging world. For these cases, patients’ self-confidence was restored and socializing as before without any embarrassment.

Abstract

Ocular prosthesis for post evisceration socket often has feeble adaptation to the surrounding tissue due to high sensi-tivity. Excessive pressure given by impression technique would lead to irritably intaglio and bulky palpebral contour of the prosthesis. This article describes a modified method of functional impression which made it easier not only to record the tissue bed surface of the defect but also to get the right contour of the palpebral surface at the same time. A 60-year-old male came to Dental Hospital Universitas Sumatera Utara with evisceration defect as a result of traumatic injury 40 years ago. He complained a facial disfigurement that made him formidable with social interacti-on. Pressureless impression combined with calibrated tray was planned for the patient. Light body PVS material was injected into the socket under slight pressure and the conformer made from visible light cure acrylic resin was in position as a tray. Vertical and horizontal lines marked in the conformer calibrate with the facial marking as a gui-dance for 3-D position. The method is intended to get the fast, accurate position offering the patient ocular prosthesis with great comfort as well as to provide facial contours that improve the patient's psychological and physical outlook.

Abstract

Patient with loss of eye demands natural ocular prosthesis for self-esteem and social intercourse. Iris texture and coloring technique are crucial procedure to recreate natural iris like the remaining eye. The purpose of this paper is to explain sculpting method using composite-resin to achieve natural iris pattern. A 29-year-old male came to Dental Hospital Universitas Sumatera Utara with chief complaint of demanding new ocular prosthesis because old ocular prosthesis had been used for 12 years, iris position was not good and not aesthetically pleasing. The modification is the use of a 12 mm diameter iris base made from light-cured composite resin then a thin layer of composite resin is placed evenly and texture of iris is sculpted directly on composite resin according to the designed pattern and then being light-cured again. Then coloring with acrylic paint according to the color of patient's left iris. The technique for du-plicating iris is highly dependent on the operator's ability, both in color selection and iris patterning. The use of com-posite resin make it easier to get a good texture of iris pattern so as to facilitate coloring process with the ultimate goal of achieving an aesthetically pleasing ocular prosthesis. Patient was satisfied with his new ocular prosthesis.

Abstract

Complete denture treatment in compromised ridges with class III jaw relationship are a challenge for prosthodontist because difficult to obtain a maximum denture bearing area in compromised ridges and to fulfil favorable functional and esthetics teeth arrangement. The purpose of this paper is to describe the modification of the custom tray, im-pression technique and teeth arrangement in edentulous case with flabby ridge, flat ridge and class III jaw relation-ship. A 58-year-old man came to Dental Hospital of Universitas Sumatera Utara with a chief complaint his loose den-ture was broken. Intraoral examination, full edentulous with flabby ridge on anterior region of maxilla, left and right maxillary tuberosity, flat ridge in maxilla and mandible with a class III jaw relationship were observed. The treatment planning included modifying custom tray and occlusal scheme. The modified custom tray of maxilla consists of dual tray with magnet retained which are made of autopolymerizing acrylic resin and thermoplastic vacuum formed. Cock-tail impression technique was used on mandible. A modified buccalized occlusion scheme to obtain good function and esthetic in class III jaw relationship. By these modification techniques, great outcome was obtained in terms of retention, stability, esthetics and functionality.

Abstract

Retentive force (RF) on the telescopic crown (TC) retainer will be distributed to the abutment teeth. The optimal load distribution measured by RF on the abutment is 5-9 N. If the force is applied excessively, it will damage the tooth's supporting tissue, causing periapical lesions, bone resorption, and mobility. However, if the RF is minimal, the dent-ure will not retentive. This paper will go through how RF will vary based on the chosen design and material. The cy-lindrical, conical, and resilient TC design are affected by the taper angle and the distance between the primary crown (PC) and secondary crown (SC). Resilience design can be modified by Marburg, Hofmann, and Yalisove. To create the precise taper angle and space in telescopic dentures (TD), CAD/CAM can now be used for the manufacture of TD using metal and non-metal materials. It is concluded that cylindrical TC design is rarely used because it is diffi-cult to get tight contact between PC and SC, therefore conus or resilience design is more recommended. The smal-ler the taper angle, the greater the RF, but this depends on the material used. While the space between PC and SC, which is less than 50 μm, can also affect RF

Abstract

Background: One of the success criteria in occular prosthesis is providing a prosthetic that is similar with the natural eye. Several methods for creating artificial iris are the use of iris from stock eyes, photo paper printing, painting with oil paints, and by implanting electronic components (dynamic iris). The oil painting method can produce an aesthetic 3D iris. However, when utilizing heat-cured acrylic resin to integrate the iris into the sclera, the use of a pressing machine when packing can distort the oil paint. Objectivess: The use of UV-resin material for iris fabrication can reduce the risk of distortion, reduce laboratory procedures, and produce an aesthetic iris. Case Report: A 28-year-old male came to Universitas Sumatera Utara dental hospital with complaint of the previous occular prosthesis felt loose and unaesthetic. Patient has been wearing the prosthesis for ±10 years. The patient wants a new occular prosthesis to replace the old prosthesis. Iris coloring is done with painting paper and oil paint, iris button is made with UV-resin using a pre-fabricated mold, then the iris is implanted into the sclera. The use of UV-resin for creating an iris requires only a few basic equipment and materials, has a quick-curing period, and facilitates iris position determination. Conclusion: Fabricating occular prosthetic with iris buttons made from UV-resin reduces the risk of laboratory failure and produces better esthetics.

Abstract

Background: Denture-bearing areas in edentulism are unable to tolerate masticatory load because of severe alveolar ridge resorption, which decreases masticatory function in the stomatognathic system. Due to the cushioning effect, the use of soft denture liner (SDL) helps reduce the masticatory load placed upon the denture-bearing areas. The physical properties and thickness of SDL have an impact on the stress distribution and cushioning effect. The masticatory load will harm the alveolar ridge and lead to more severe resorption, when modulus elasticity of SDL lower than the mucosa. The stress distribution of the masticatory load can be analyzed more accurately in-silico study using Finite Element Analysis (FEA) method. Objectives: In this paper, the stress distribution in denture-supporting areas with various SDL thicknesses is evaluated using FEA. The thickness of SDL contributes to the distribution of stress upon the denture-bearing areas by minimizing the masticatory load. Conclusion: Through the same modulus elasticity of the mucosa and SDL is able to equally distribute stress on the denture-bearing areas.
