Abstract

Intravenous immunoglobulin (IVIG) is a plasma derivative product that can provide passive immune protection against various pathogens. IVIG was given to COVID-19 patients who are seriously and critically ill. In this case report, a 43-year-old male patient who has been confirmed with COVID-19 is admitted to the COVID-19 ICU. The patient complains of tightness, coughing and weakness. The examination found decreased SpO2, hypertension and Chest X-ray revealed bilateral bronchopneumonia. On the first day the patient was given antiviral remdesivir 200mg drip IV/3 hours, amlodipine 1x 5mg, codeine 3x10 mg, combivent 3x1, furosemide, and albumin 25% 2 kolf. On days two to five patients were given antiviral remdesivir 100mg drip IV/3 hours, IVIG, namely Gamaras 5 flash, heparin 25,000 units. On the tenth day of treatment in the ICU, the patient’s complaints had decreased. On the eleventh day the patient had no complaints and the patient’s vital signs concluded that the problem was resolved.

Abstract

Aged people will experience several changes in all aspects of life including, physical, psychological, spiritual, and social. Their altered condition such as hearing deprivation, loss of vision, or fluctuating emotions will affect how the way they communicate with other family members. Thus, the family should be more concerned regarding this issue, unless they will have ineffective communication. Moreover, it may lead to trigger a conflict among family members. To unravel this problem, an education on effective communication for aged people using a module will help them to improve strategies for managing the elderly's communication problems. This activity involved 67 families with elderly and 7 health volunteers in Kelurahan Patangpuluhan Wirobrajan Yogyakarta. The result showed that education could elevate the score of effective communication among aged people, both family, and health volunteers. However, health volunteers had a higher average score than family

Abstract

Peritonitis is an inflammation of the peritonium which consists of a serous membrane that lines the abdominal cavity and visceral organs in it and is an emergency that can be accompanied by sepsis. Inflammation in peritonitis can be localized or generalized in the peritoneum cavity which is generally caused by bacteria or fungi, but can also be caused by non-infectious substances such as the contents of the stomach or bile contents. Peritonitis due to infection is classified as primary, secondary, or tertiary. This is rare, but it generally appears in teenage women. 90% of SBP cases occur due to monomicrobial infections. Streptococcus pneumoniae is usually the causative organism. It has been reported that a 15-year-old woman was brought to the emergency room complaining of severe abdominal pain. The pain is felt continuously as if it were pricked. Previously, the patient had taken Chinese medicine given by the patient's mother but it did not decrease. On physical examination, signs of peritonitis were obtained and a laparotomy was planned. The patient is categorized as an ASA II patient, due to the absence of a history of systemic disease.

Abstract

Thyrotoxicosis is defined as a state of excess thyroid hormone and is not synonymous with hyperthyroidism, which is the result of excessive thyroid function. The prevalence of hyperthyroidism ranges from 0.2% to 1.3% in iodine-sufficient regions of the world. The combination of increasing FT4 and suppressing TSH is one way to enforce the hyperthyroidism diagnosis. The case of a 22-year-old woman with a diagnosis of G6P3A2H3 premature contractions has been reported , who came to the surgical room at Dr. HOSPITAL. Suhatman MARS to perform a caesarean section. Based on the pre-operative physical examination carried out, it was found that the patient's physical status was ASA II. In this patient, regional anesthesia will be planned. The patient has a history of hyperthyroidism since ± 10 years ago and regularly takes Thyrozol once a day during pregnancy and controls it with a specialist in Internal Medicine. Theoretically, this shows that the suspected condition of the patient is hyperthyroidism and if continued it could lead to Graves' disease, toxic nodular goiter, subclinical hyperthyroidism, or other conditions that lead to hyperthyroidism so surgery must be carried out taking this situation into account.

Abstract

Kava (Piper Methysticum) is a plant traditionally used in the South Pacific for its sedative and anxiolytic effects. Its active compounds, kavalactones, are known to have potential as natural anesthetics, with a mechanism of action similar to benzodiazepines and general anesthetics. This study aims to evaluate the effectiveness of Kava as an anesthetic through a systematic literature review and meta-analysis. Literature searches were conducted in PubMed, Scopus, and Cochrane Library using relevant keywords. Meta-analysis results indicate that Kava significantly enhances anesthetic effects compared to placebo, primarily through modulation of GABA-A receptors. However, further research is needed to determine the optimal dosage and long-term safety.
