Abstract

Currently, the prevalence of degenerative diseases in Indonesia is increased. The degenerative and chronic diseases need life-long treatment as well as changes in life style. On the other hand, long-life treatment using drugs will develop risks of adverse drug reactions or the possibilities of drug-drug interactions. In these circumstances, the role of a pharmacist is important to providing drug information and counselling, and patient education. However, there is evidence that pharmaceutical care in almost all pharmacies (drug dispensaries) in Indonesia is still “drug-oriented”. Drug information is not adequate and is provided by pharmacy assistants (technicians). In order to implement pharmaceutical care practice that meets the “Standard Competence for Pharmacy Practice” as well as consumer’s expectation, we need to know what is the exactly consumer’s expect in pharmaceutical care, particularly in drug information, and how is the commitment of the pharmacists to provide drug information of degenerative and chronic diseases in pharmaceutical care. For those reasons, a field survey has been carried out in Jakarta, Yogyakarta and Makassar to explore the consumer/patient’s opinions and needs for pharmaceutical care. A focus group discussion has also been done to explore the pharmacist commitment to provide drug information in pharmaceutical care practice. As a result, there is an indication of a discrepancy between the consumer/patient’s need for drug information and the commitment of pharmacists to provide drug information. Consumers have need for more drug information for all aspects including adverse drug reactions, drug interaction, what to do if adverse reaction occurred, duration of any treatments etc., not only drug indication and administration. On the other hand, pharmacists still need more knowledge and continuing education, particularly in pharmacotherapy and pharmacology of drugs used for degenerative and chronic diseases.

Abstract

The Act of The Republic of Indonesia Number 7 of 1996 on Food mentioned that every label and or advertisement concerning food to be sold must mention information concerning the food correctly and not misleading. The government shall regulate, supervise and the measure which are needed, in order that an advertisement concerning food which is sold does not contain information which may be misleading. This cross-sectional study was conducted to Identify legislation related to food advertisement. Primary data collection by means of indepth interviews and Round Table Discussion (RTD), as well as secondary data collection concerning institusional report and monitoring documentation has been used. The data were analysed qualitatively. The findings indicate that monitoring of food advertisement only at the provincial level. Monitoring of Processed foodstuff advertisement by testimony in broadcast media, inset in the event program at television and advertising on the Internet is not optimal and it’s required cross-sectoral coordination.

Abstract

Pharmacy is a place where a pharmacist provides pharmaceutical services. To improve services, it is necessary to carry out an evaluation of patients’s or consumers’ satisfaction on pharmaceutical services.This research aimed to assess the satisfaction of the patients covered by national health insurance programme (JKN) and to find out the relationship of patients’ characteristics and their satisfaction. This research used a cross sectional survey design, conducted for patients visiting pharmacy to redeem the prescribed drug in Tangerang Selatan city, Serang district, Bekasi city, Bogor district, Yogyakarta city, Bantul district, Solo city, and Sragen district. The sample was 152 adult JKN patients chosen by accidental sampling. Data were collected using self assessment questionnaire analyzed using chi square analysis. This study found that the highest level of dimension of pharmaceutical services satisfaction was empathy and the lowest one was assurance, in term of the availability of drugs and the number of drugs that was covered by JKN scheme. Among 5 characteristics (i.e: age, sex, marriage status, education, and occupation), only occupation variable that had significant correlation with satisfaction. The pharmacy management should communicate with the JKN provider regarding the kinds of drugs covered and concern with the drugs availability in the pharmacy.

Abstract

Nosocomial infections is an important issue around the world. Hospitals are required to provide qualified, efficient and effective service to ensure patient safety. The Ministry of Health has revitalized prevention and control of infection program (PPI) in hospitals which is one of the cornerstones towards patient Safety. The purpose of this study is to identify the preparedness of hospitals to implement the PPI program. The data source is Health Facility Research of 2011 which was done by National Institute of Health Research and Development. The aspects examined include facility, infrastructure, human resource, organization, guidelines, compliance in prescribing, the availability of clean water and hospital sewage treatment. The results show that many hospitals are not yet ready to conduct PPI, especially in terms of infrastructure, clean water sterilization and processing waste, mostly hospitals of classes C and D. Sewage treatment is important in the control or prevention of spread of antimicrobial resistance. This program gives many benefits especially in preventing the occurrence of total resistance or back to the era before antibiotics. The PPI program does require a large fee such that hospital management often disapproves, although the result of available cost analysis indicates that PPI is highly cost-effective.

Abstract

A descriptive study to identify the preparedness of pharmacist in community pharmacies to cope with globalization impact was conducted in 2009. This cross-sectional study was done in DKI Jakarta, Bali and Maluku. Informants were purposively determined involving pharmacists from schools of pharmacy, Indonesian Pharmacist Association (IAI), community pharmacies, Provincial and District Health Offices, as well as comunity pharmacy owners. Primary data were collected through in-depth interviews and observation using check-list in community pharmacy. Data were analyzed descriptively and qualitatively using triangulation method. Results of the study show that according to Health Offices and the Indonesian Pharmacist Association, pharmacists were not adequately prepared and pharmacists in stand alone community pharmacy are less prepared than those in a network or franchise pharmacy. Licensed pharmacists of network community pharmacy in the metropolis are going to prepare themselves to face the new patient-oriented paradigm and to meet the standard of pharmacy service, whereas stand-alone community pharmacy still prioritized fast service and lower drug price

Abstract

Lack of information or inappopriate of information on medicine and traditional medicine caused unsuitable utilization of them. The objective of this study was analyzing the regulation of the medicine and traditional medicines information announcement and the implementation of the relevant institutions as well, even in the central, provincial and district level. Cross sectional study with qualitative approach due to the regulation of medicine and traditional medicine information announcement, as well as the related institutions. The study carried out at 2009, in Jakarta, Yogyakarta, Denpasar, Medan and Banjarmasin. The samples ofthis study were pharmacists as officers on The National Agency of Drug and Food Control (Badan POM), Provincial Health Offices, District Health Oflices, pharmaceutical companies, hospitals, pharmacies, and other relevant institutions. Data collecting has been conducted by depth interviews, focus group discussions, while the secondary data has been collected by multiplies of the document needed. Descriptive and qualitative analyzed the data, and final results completed by round table discussion. NADF C announce the information of medicine and traditional medicine to the community and pharmaceutical companies. Provincial Health Office and District Health Ojfice announce the information to the community by socialization, and announce directly to the patients in Primary Health Care, While the pharmaceutical companies provide their own products information. Besides, the hospitals and pharmacies provide the medicine information to the patients; Announcing the information to the community by the government has not been optimal yet to compensate traditional medicine advertizing, since the advertizing services to the community has not been used eflectively; The Indonesian Consumer Organization is one of the institution that participate in announcing information to the community. Besides, the pharmaceutical colleges and pharmacies has also provide information to the community.

Abstract

Acute respiratory infection (ARI) is a common disease in the community. Riskesdas 2018 stated that ARI prevalence was 4.4% and the highest was in 1-4 years old children (8%). The capitation payment system in clinics collaborated with BPJS Kesehatan demands the physician to prescribe as effective and as efficient as possible. On the other hand, the capitation tariff obtained by clinics is considered as too low, thus constrains of the prescription leading to irrational prescribing is likely to occur. This study analyses further the prescribing pattern for ARI children in clinics and its rationality based on the difference in source of funding. A cross sectional research using retrospective method was conducted. Data were gathered from 409 medical records and or patients’ prescription of children between 1-12 years old and diagnosed as having non pneumonia ARI during 1st January to 30th November 2019. Result showed that non pneumonia ARI patients were mostly boys (54,3%) aged 1-5 years old (6.0%). The average number of items for BPJS patients was 3,45, the percentage of generic prescribing was twofold higher for BPJS group (63.94%), the percentage of essential medicine prescribing was 63.96% while the percentage of antibiotic use was lower (48.50%). Dosage propriety for BPJS patients was 70.80% which was slightly higher than non-BPJS group. Overall, the prescribing indicators and dosage properness for BPJS patients were better than non-BPJS patients.

Abstract

Undang-undang Republik Indonesia No. 36 tahun 2009 tentang kesehatan dan peraturan pemerintah No. 51 tahun 2009 menyatakan bahwa tenaga kesehatan harus mempunyai kualifikasi minimum yang ditetapkan oleh pemerintah. Studi kualitatif secara potong lintang pada tahun 2010 untuk mengidentifikasi kualifikasi apoteker rumah sakit dalam memenuhi persyaratan tersebut di Bandung, Yogyakarta dan Surabaya. Data dikumpulkan dengan wawancara mendalam terhadap 10 orang apoteker dari enam rumah sakit dan empat orang direktur/wakil direktur rumah sakit, masing-masing satu orang apoteker dari enam perguruan tinggi farmasi, tiga pengurus Ikatan Apoteker Indonesia, tiga dinas kesehatan provinsi dan kabupaten/kota. Observasi praktek kefarmasian dengan menggunakan daftar tilik dilakukan pada tiap rumah sakit dan data sekunder terkait dokumentasi pemantauan dan evaluasi obat, kepuasan pasien, standar operasional prosedur dan kurikulum perguruan tinggi farmasi juga dikumpulkan. Analisis dilakukan dengan metode triangulasi dan hasil menunjukkan bahwa pengelolaan obat dalam hal pengadaan, distribusi dan penyimpanan dilaksanakan dengan baik oleh apoteker rumah sakit. Praktek farmasi klinik dan keselamatan pasien masih sangat terbatas karena alasan sumber daya manusia dan dokumentasi yang memadai. Informasi obat dan konseling kadang dilakukan tanpa fasilitas yang cukup dan apoteker juga terlibat dalam berbagai tim di rumah sakit seperti penanggulangan infeksi nosokomial dan komite farmasi dan terapi. The Indonesian Health Law No. 36 in 2009 and the Government Regulation No. 51 in 2009 state that health-care providers, including pharmacist, shall have minimum qualification set by the government. A qualitative cross sectional was conducted to to identify hospital pharmacist qualification as health care professionals in meeting the requirements was done in 2010 in Bandung, Yogyakarta and Surabaya. Data were collected through indepth interviews with pharmacists involving ten hospital pharmacists and four hos- pital directors/vice directors, six pharmacy colleges, three regional pharmacist associations, three provincial health offices and district health offices and observation of pharmacy practices using check list in each hospital was also conducted. Secondary data concerning documentation of drug monitoring and evaluation, patient satisfaction, standard operating procedure and pharmacy college curricula were collected too. Qualitative analysis was done descriptively using triangulation method. The study shows that drug procurement, distribution and storage, was well-managed by pharmacist. Practice in clinical pharmacy and patient safety was still limited for the reason of human resources and appropriate documentation. Drug information and counseling was sometimes conducted without adequate facilities and pharmacist was involved in various hospital teams like nosocomial infection control and pharmacy and therapy committee.
