Abstract

Abstract The Government Regulation Number 51 on Pharmacy Practice states that dispensing prescriptions in a health facility, including the community health center, must be done by a pharmacist. In 2010 only ten percents of community health centers have a pharmacist. A cross sectional qualitative study has been done to obtain information on the role of pharmacist themselves in practicing in a community health center, and to obtain information on problem and barrier of the role of pharmacist. The study was carried out in Java, namely in cities of Tangerang, Bandung, Surabaya and Bantul District. Informants for in-depth interviews are District/City Health Office staff involved in the pharmacist assignment in a community health center and head of community health center, whereas for focus group discussion we invited pharmacists from District/City Health Office, community health center, school of pharmacy and regional pharmacists association. Descriptive data were analyzed qualitatively using triangulation method and temporary results were reviewed in a round table discussion in Jakarta with DG of Pharmaceutical Services and Medical Devices, Indonesian Pharmacist Association and Provincial Health Office of DKI Jakarta. Results of the study show that: 1. A. pharmacist is not available at all community health centers, as well as non-care community health centers, there are many prescription services performed by non-professional personnel. 2. The role of pharmacist in a community health center covers good drug management just like their job description, especially in prescription dispensing and drug use report. 3. The role of the pharmacist in pharmaceutical services: (a) information carried on the delivery of drugs prescription drugs to the patient, prior to clinic services began, and during a visit to posyandu posyandu toddlers and the elderly, (b) drug counseling is limited given the availability of time and there is no room , (c) visite the patient was done, either with the patients own doctor or maternity hospitalization, (d) home care have not been going well. 4. Issues related to pharmacists in community health centers is the availability and the number does not match to the workload, so that pharmaceutical care have not been going well due to limitations of time and effort. Also there are pharmacists feel less capable in providing drug information to other health professionals, especially medical specialist several health centers, so it is still necessary coaching and training. Keywords: pharmacist, Guideline of pharmaceutical care, community health center, drug management Abstrak Dalam Peraturan Pemerintah RI Nomor 51 tahun 2009 disebutkan pelayanan resep dokter di pelayanan kefarmasian (salah satunya puskesmas) harus dilakukan oleh apoteker. Data tahun 2010 menunjukkan hanya 10% puskesmas yang memiliki apoteker. Tujuan penelitian adalah mendapatkan informasi tentang peran apoteker di puskesmas dan permasalahan dalam pelayanan kefarmasian di puskesmas. Hasil penelitian diharapkan sebagai masukan bagi pihak yang terkait untuk meningkatkan ketersediaan apoteker dalam pelayanan kefarmasian di puskesmas. Penelitian potong lintang (cross sectional) dengan pendekatan kualitatif dilakukan terhadap instansi yang terkait dengan peran apoteker di puskesmas pada tahun 2011. Lokasi penelitian yang dipilih adalah Provinsi Banten, Jawa Barat, Daerah Istimewa Yogyakarta, dan Jawa Timur. Dari masing-masing provinsi diambil satu kota, yaitu Kota Tangerang, Kota Bandung, Kabupaten Bantul dan Kota Surabaya. Informan penelitian untuk wawancara mendalam adalah Dinkes Kabupaten/Kota dan Kepala Puskesmas, sedangkan peserta diskusi kelompok terarah adalah 12 apoteker yang mewakili Dinkes Kabupaten/Kota, puskesmas perawatan, Perguruan Tinggi Farmasi (PTF) dan Pengurus Daerah Ikatan Apoteker Indonesia (PD IAI). Analisis data secara deskriptif kualitatif dengan metoda triangulasi sumber data dan triangulasi metoda pengumpulan data. Hasil sementara disempurnakan dengan Round Table Discussion di Jakarta dengan mengundang nara sumber. Hasil studi adalah sebagai berikut : 1.  Apoteker belum tersedia di semua puskesmas perawatan, apalagi puskesmas non perawatan, sehingga pelayanan resep dikerjakan oleh tenaga non profesional. 2.  Peran apoteker dalam pengelolaan obat umumnya sudah berjalan, khususnya dalam pelayanan obat resep dan pembuatan LP-LPO bulanan. 3.  Peran apoteker dalam pelayanan kefarmasian: (a) informasi obat dilakukan pada saat penyerahan obat resep kepada pasien, sebelum pelayanan puskesmas dimulai, dan pada saat kunjungan ke posyandu balita dan posyandu lansia, (b) konseling obat dilakukan terbatas mengingat ketersediaan waktu dan belum ada ruangan, (c) visite pasien sudah dilakukan, baik dengan dokter maupun sendiri kepada pasien bersalin rawat inap, (d) home care belum berjalan dengan baik. 4.  Permasalahan yang terkait dengan apoteker di puskesmas adalah ketersediaan dan jumlah tidak sesuai dengan beban kerjanya, sehingga pelayanan kefarmasian belum berjalan baik akibat keterbatasan waktu dan tenaga. Juga ada apoteker merasa kurang mampu dalam memberikan informasi obat kepada tenaga kesehatan lain, khususnya dokter spesialis di beberapa puskesmas perawatan, sehingga masih diperlukan pembinaan dan pelatihan.   Kata kunci: apoteker, pelayanan kefarmasian, pengelolaan obat, puskesmas

Abstract

According to National Basic Health Research (Riskesdas) 2013, the prevalence of anemia in pregnant women in urban area is 36.4%. The treatment of anemia in pregnant women is conducted by administering 90 iron tablets during pregnancy. The majority of anemia in pregnant women is mostly caused by lacking of iron (Fe) due to lack of adherence in taking iron tablets. The objective of this research was to assess the effectiveness of leaflet compared to counseling to improve adherence and increasing hemoglobin status of pregnant women with anemia in primary health care in Bogor District. The research design was quasi-experimental by giving counseling as an intervention conducted by pharmacists to 79 pregnant women with anemia in Cibungbulan primary health care and delivering leaflet to 79 pregnant women with anemia in Ciluengsi primary health care Bogor District in 2013. The evaluation was conducted by using MMAS-8 (Morisky Medication Adherence Scale) questionnaires and Hb rate with STAT-Site MHgb equipment a month after the intervention. Data analysis was carried out with Chi-Square test, Wilcoxon test, Mann-Whitney test and multivariate logistic regression analysis. This research suggests that either counseling by pharmacist or leaflet increases adherence to take iron tablets (P < 0.05). The effectiveness of leaflets and counseling to increase adherence to pregnant women with anemia were not statistically different. Pregnant women with anemia who adhere to take iron tablet have their Hb rate improved 3.24 times compared to those who do not adhere to take iron tablet. Pregnant women with anemia who eat food source of heme every day have their Hb rate improved 2.31 times compared to those who do not eat food source of heme every day.   Abstrak Menurut Riset Kesehatan Dasar (Riskesdas) 2013, prevalensi anemia pada ibu hamil di perkotaan sebesar 36,4%. Penanggulangan anemia pada ibu hamil dilakukan dengan pemberian minimal 90 tablet besi selama kehamilannya. Sebagian besar anemia pada ibu hamil di Indonesia disebabkan karena kekurangan zat besi (Fe) akibat ketidakpatuhannya meminum tablet besi. Tujuan penelitian ini adalah menilai efektivitas leaflet dibandingkan konseling terhadap peningkatan kepatuhan minum tablet besi dan kadar hemoglobin (Hb) ibu hamil dengan anemia di dua puskesmas di Kabupaten Bogor. Rancangan penelitian yang digunakan adalah kuasi eksperimen, dengan intervensi berupa konseling oleh apoteker pada 79 ibu hamil dengan anemia di Puskesmas Cibungbulan dan pemberian leaflet pada 79 ibu hamil dengan anemia di Puskesmas Cileungsi Kabupaten Bogor pada tahun 2013. Sebulan setelah intervensi, dilakukan evaluasi untuk menilai peningkatan kepatuhan minum tablet besi dan kadar Hb menggunakan kuesioner MMAS-8 dan pengukuran kadar Hb. Analisis data dilakukan dengan uji Chi-square, uji Wilcoxon, uji Mann Whitney dan uji regresi logistik multivariat. Kesimpulan bahwa konseling oleh apoteker atau pemberian leaflet pada ibu hamil dengan anemia meningkatkan kepatuhan minum tablet besi (P < 0,05) secara bermakna. Efektivitas leaflet dan konseling terhadap peningkatan kepatuhan minum tablet besi pada ibu hamil dengan anemia secara statistik tidak berbeda. Peningkatan kepatuhan minum obat dan makan makanan yang mengandung protein setiap hari dapat meningkatkan kadar Hb secara bermakna (P < 0,05). Ibu hamil dengan anemia yang patuh minum tablet besi, kadar Hb-nya meningkat 3,24 kali dibandingkan yang tidak. Ibu hamil dengan anemia yang makan makanan yang mengandung protein setiap hari, kadar Hb-nya meningkat 2,31 kali dibandingkan ibu yang tidak makan makanan yang mengandung protein.

Abstract

Background: Infant mortality at the age of ≤1 year per 1000 live births is one of the benchmarks for assessing the extent to which the achievement of people's welfare as a result of implementing development in the health sector. This research aimed to identify the factors associated with infant mortality in Ende Hospital. Method: Cross-sectional design was applied in this research. Data retrieval was collected by using medical records in 2017 and 2018. There were 122 mothers had infant mortality aged ≤1 year enrolled in this research with a total population sampling technique. This study used Kendal Tau-b statistical test. Results: The results showed the highest infant mortality occurred at 0-28 days as many as 77,9%. There was a relationship between maternal education (p-value=0.042), asphyxia (p-value=0.014) and infant mortality in Ende Hospital. There was no relationship between maternal age (p-value=0.602), maternal occupation (p-value=0.842), pregnancy danger signs (p-value=0.713), birth weight (p-value=0.587), labor (p-value=0.845), delivery helper (p-value=0.419), place of delivery (p-value=0.354) and infant mortality in Ende Hospital. Health workers need to consider facilities for newborn care, must be able to measure the Apgar Score for asphyxia measurement, and should provide information regarding the importance of conducting ANC examinations to prevent complication during pregnancy and asphyxia.

Abstract

The pharmaceutical services at a dispensary are prescription dispensing, self-medication and drug information. Patient's satisfaction is an essential factor in improving medicines sale at dispensary. The objective of survey is to know the patient's satisfaction who get the prescription service at Kopkar dispensary. Cross sectional study is conducted by collecting data from 100 adult patients who get the prescription service at Kopkar dispensary on November 2003 by questioner. Respondent are selected by the systematic random sampling. Data were analyzed by comparing between the perception of reality service and the perception of need service using correlation index. The result of this study are: (1) The patients have high satisfaction to the empathy and the cleaning waiting room, (2) The patients have moderate satisfaction to the drug service, the cashier service, the drug information, the drug completed, the drug price, the desk in waiting room, the fan and air conditioning service, and (3) the patients have low satisfaction to the television service.

Abstract

Almost all district public hospital already has their own formulary. The aims of study are to obtain percentage of noncompliance with the public hospital formulary, to obtain the average additional cost be paid by outpatients as a result of noncompliance with the hospital formulary, and to obtain the average of the outpatient's ability to pay for treatment. A cross sectional study has been carried out to 120 patients in RSU Kabupaten K and 100 patients in RSU Kabupaten B. Subjects of the study were adult outpatients with TB, hypertension and diabetes. Data were collected by well-trained district public hospital staff in interviewing patients. The questioner was first tried out to patients at RSU Kota Jakarta Timur. Data were analyzed by cost analysis. Results of the study are difference in drug item with formulary in RSU Kabupaten K is 66,7% for TB, 96,6% for hypertension; where as in RSU Kabupaten B 44,8% for TB, 82,3% for hypertension and 76,7% for diabetes. Average additional cost that must be paid by outpatients per encounter in RSU Kabupaten K is Rp 10.060 for TB, Rp 26.552 for hypertension; while in RSU Kabupaten B is Rp 5.818 for TB, Rp 8.956 for hypertension and Rp 15.218 for diabetes. The average outpatient's ability to pay for treatment in RSU Kabupaten K is Rp 19.807 and in RSU Kabupaten B is Rp 15.301, which are both less than outpatient treatment cost per encounter.

Abstract

The self-medication is an effort conducted by the community to cure theirselves using medicine, traditional medicine or others without health proffesional advice. The aims of this study are to know healthy – illness concept, to know local language, symptoms, cause, prevention and curation of headache, fever, cough and common cold, and the self- medication practice on the village community. This study using qualitative design and data was collected by depth interviewing from 12 key informans at Ciwalen village, Warungkondang sub-district, Cianjur district, West Java, in 1998. Key informans are the chief of RT, the chief of RW, the teachers of elementary school, the health cadres, and the housewives. Data were analyzed using triangulation methode and confirmating the interview result to the key informans. The conclussion of this study are The healthy-illness concept does not only physical aspect, but also social culture aspect. The light illness - heavy illness concept depends on the physical condition of patient, the daily activity and the medication. The community use generally local language nyeri sirah for the headache, muriang for the fever, gohgoy for the cought and salesma for the common cold. The cause of illness is commonly their physical environment, include bacteria for the cought. The prevention of illness is generally conducted by avoiding its cause. The self-medication practice generally use the medicine that were bought from the retail at their village, some of them use the traditional medicine. Reason of self-medication practice are light illness, inexpensive, time eficiency, and as a first aid before going to the health proffesional or health center. The self-medication practice is improperly done, because the community mostly bought a small amount of medicine, so that the brochure of the medicine can not be read.

Abstract

In the elderly, the aging process experienced causes a decrease in NREM sleep cycles 3 and 4. As a result of this decrease, it affects the sleep quality of the elderly. Sleep quality shows a person's satisfaction with his sleep, where the person does not show signs of sleep deprivation.

Abstract

Aims: The unprofessional behavior of nurses makes nursing services not provide positive values, which can be influenced by the absence of preceptorship for nurses. The research conducted in June – August 2022 aims to assess the effect of preceptorship program and recruitment process to improve professional nurse behavior. Design: Quasi-experimental study, with intervention group preceptorship program RS Cinta Kasih Tzu Chi and control group RS Mentari. Samples of intervention and control groups were 35 respondents each, using total sampling technique. Data collection tools are questionnaires and pretest - posttest data collection is carried out 4 days after the questionnaires are distributed. Method: Data analysis using Wilcoxon test, Chi-square test and multivariate logistic regression test. Results: The results showed that the behavior of professional nurses increased. There are differences in the behavior of nurses in the intervention and control groups (p value= 0.000). There is the relationship between the recruitment process and an increase in the behavior of nurses (p value = 0.001). Working period (p value = 0.017) and preceptorship program (p value = 0.006) have an effect on increasing the behavior of professional nurses. Conclusion: It is hoped that RS C will hold a preceptorship program and establish regulations, policies, guidelines and guidelines on the application of the preceptorship method to changes in the behavior of professional nurses. Nurses understand and instill the behavior of professional nurses starting from themselves, colleagues, and patient care.

Abstract

This study aims to explore nurses' level of satisfaction with the role of MPPs in patient care at Hospital X in Tangerang Regency. The method used was quantitative - qualitative mixed methods. Data analysis used chi square analysis. The results showed that based on the results of quantitative analysis, it appears that the MPP role has a significant positive influence on nurse satisfaction in carrying out patient discharge planning. The results of qualitative analysis through interviews with informants also support quantitative findings. Informants stated that timely assessment and implementation of continuity of patient care for discharge preparation and evaluation of implementation are important factors that can increase nurse satisfaction. The majority of informants expressed satisfaction with the implementation of the MPP role, although there were some who were dissatisfied, which was also in line with the quantitative results. The conclusion of the study that can be drawn is that nurses' satisfaction with the role of the Patient Service Manager (MPP) at Hospital X Tangerang Regency depends on the effectiveness of coordination and integration of services carried out by MPP. Periodic evaluation of the MPP program is needed to improve nurse satisfaction and patient service quality. Keywords: satisfaction, nurses, MPP role, mixed-methods.

Abstract

The shift in the pattern of non-communicable diseases to a younger age makes prediabetes undetected and tends to be ignored. Monitoring weight is an essential behavior for individuals to reduce the risk of obesity as a predictor of prediabetes. In addition, family support plays a crucial role in maintaining each individual's health in deciding his lifestyle. This study aimed to determine the relationship between self-efficacy weight management (SEWM) and family support on risk factors for prediabetes at the Community Health Center, Batam. The research design is a descriptive correlation with the cross-sectional approach. Fifty respondents were chosen using a purposive sampling technique based on the inclusion and exclusion criteria. Instrument of INA-PRISC questionnaire, Weight Efficacy Lifestyle Short Form Questionnaire, and Perceived Social Support from Family (PSS- Fa) utilized in this study. Based on univariate results, the majority of respondents aged 26-35 years (30%), female (68%), higher education background (78%), obesity (>25) (80%), had a family history of DM (60%), not smoking (70%), physical activity <150 minutes/week (94%), hypertension >160/100 mmHg (44%), moderate risk of prediabetes (48%), low SEWM (70%), and good family support (38%). A bivariate statistical test using Kendall's Tau B revealed there was a significant relationship between SEWM (< 0.05) and family support for risk factors for prediabetes (< 0.05). It is suggested that respondents increase self-awareness in the early detection of prediabetes risk and the family's active role in assisting family members in starting an active and healthy lifestyle.
