Abstract

Patients with cardiovascular diseases such as chronic heart failure may develop cardiac cachexia. This occurs due to various mechanisms associated with pathological processes in the heart. The incidence of cardiac cachexia is also a marker of poor prognosis in patients. This condition is characterized by weight loss in patients and a decrease in muscle mass. We conducted a manual search method to find the topic-related article. A total of 30 relevant works of literature were studied. We aimed to understand the role of cardiac cachexia on the physiological function and its proper management. From our study, it wasfound that systemic inflammation had a role in the development of cardiac cachexia. The presence of cardiac cachexia is associated with the disruption in several systems such as cardiovascular, gastrointestinal, and also the immune system. Strategies for prevention and appropriate management will reduce the incidence of this event. The treatment given can be pharmacological and non-pharmacological. In conclusion, cardiac cachexia impairs the functional capacity of the patient. Proper management can reduce the mortality and morbidity in patients with cardiac cachexia.Keywords: Cardiac cachexia, physiological function, preventive and management

Abstract

COVID-19 telah menjadi pandemi dan pasien paska COVID-19 sering kali menunjukkan gejala yang berkepanjangan termasuk keluhan pada kardiovaskular. Gejala yang memanjang ini dikenal dengan sindrom COVID-19 yang memanjang atau sindrom long COVID. Sindroma tersebut melibatkan jantung dengan gejala nyeri dada, sesak nafas, cepat lelah, berdebar, dan rasa melayang diakibatkan perubahan posisi. Adapun beberapa mekanismenya berupa cedera miokard sekunder akibat miokarditis karena virus atau kerusakan imun, iskemik miokard akibat trombosis mikrovaskuler, dan hipoksemia. Pemeriksaan penunjang perlu dikerjakan untuk menyingkirkan sebab utama keluhan tersebut. Adapun tata laksana untuk sindrom COVID-19 yang memanjang pada kardiovaskular belum ada rekomendasinya. Abstract

Tindakan elektrofisiologi adalah tindakan invasive yang membutuhkan penempatan kateter ke dalam jantung.Tindakan ini dapat mendiagnosis irama jantung abnormal dan mengidentifikasi area jantung yang terpengaruh. Jika penyebab irama jantung abnormal ditemukan, terkadang dokter akan melakukan tindakan ablasi jantung. Ablasi jantung sering disebut juga dengan ablasi kateter, merupakan prosedur yang dilakukan untuk memperbaiki abnormalitas dari kelistrikan jantung dengan merusak sebagian kecil otot jantung yang bertujuan untuk memutus rangkaian listrik yang tidak normal. Tindakan elektrofisiologi dilakukan di ruangan cath lab dan membutuhkan tim khusus yang diantaranya terdiri dari physician, scrub nurse, teknisi kardiovaskuler serta teknisi radiologi. Peran teknisi kardiovaskuler pada tindakan elektrofisiologi yang dilakukan di cath lab adalah sebagai circulator dan monitor.

Abstract

The Hippocratic Oath (HO) is an oath recited by all doctors in the world. HO contains a promise based on ethical principles and medical practice. Adherence to the principles in HO must be upheld by every medical practitioner. However, with the rapid development of the current era, HO is no longer appropriate. Such as the advancement of digital health, the use of artificial intelligence (AI), the assessment of big data in healthcare, the security of digital patient data, patient-based care, and the role of patients in determining the direction of self-medication and health digital virtual services. This transformation demands a change in the general context of HO. This article will briefly discuss about HO in relation to changes in the digital era. Some of HO's inputs on the progress of the digital world today include the use of digital technology in medical practice, the role of patients, patient data privacy, and the use of big data. This article uses a literature review research method with an ethical approach.

Abstract

The incidence of cardiogenic shock (CS) did decrease in the PCI era, but the short-term mortality is still high up to 80%. Cardiogenic shock most commonly occurs as a complication of acute myocardial infarction, but can also result from medication, heart failure, obstruction, or other pericardial and metabolic diseases. The pathophysiology of cardiogenic shock is associated with many etiologies and precipitants. CS needs to be evaluated based on the presence or absence of hypoperfusion and congestion so that further management becomes more targeted. Closer hemodynamic monitoring is needed in CS patients with hypoperfusion. In this review, we will be discussed the hemodynamic findings and evaluation of perfusion in patients with cardiogenic shock.

Abstract

Sindrom Korener Akut (SKA) merupakan penyakit kardiovaskular yang menyebabkan kematian serta perawatan yang tinggi di Rumah Sakit. Pasien yang menderita SKA mendapatkan pengobatan meliputi farmakoterapi dan terapi reperfusi seperti intervensi koroner perkutan primer dan terapi fibronolitik. Setelah perawatan dirumah sakit, pasien melanjutkan perawatan paska SKA dirumah, terdapat beberapa kasus yang seringkali muncul seperti SKA yang berulang maupun resiko-resiko lain yang mengakibatkan pasien dirawat inap kembali. Untuk mendapatkan terapi yang maksimal salah satu faktor pendukung adalah pentingnya peran dokter umum dalam perawatan pasien paska Sindrom Koroner Akut. Artikel ini bertujuan untuk melihat peran dokter umum dalam perawatan paska SKA dengan metode penelitian berupa tinjauan pustaka.

Abstract

Latar belakang: Berdasarkan data dari organisasi kesehatan dunia (WHO) 99% populasi dunia hidup dan menghirup udara dengan paparan polusi yang melebihi batas aman menurut pedoman WHO. Polusi udara dapat berdampak buruk pada kesehatan terutama pada sistem kardiovaskular. Berbagai penyakit kardiovaskular seperti hipertensi, gagal jantung, penyakit jantung koroner, aritmia, thrombosis vena, serta permasalahan pada sistem serebrovaskular seperti stroke diketahui berhubungan dengan paparan polusi udara sehari-hari. Metode: studi ini merupakan sebuah literature review. Penelusuran literatur menggunakan database dari pubmed dan google scholar. Didapatkan 35 jurnal dalam bahasa inggris. Hasil dan kesimpulan: paparan polusi udara dapat berdampak buruk pada kesehatan terutama pada sistem kardiovaskular, baik paparan jangka pendek maupun paparan jangka panjang. Berbagai penyakit kardiovaskular seperti hipertensi, gagal jantung, penyakit jantung koroner, aritmia, thrombosis vena, serta permasalahan pada sistem serebrovaskular seperti stroke diketahui berhubungan dengan paparan polusi udara sehari-hari. Diperlukan upaya yang bersinergis antar negara dan mencangkup setiap lapisan masyarakat untuk menurunkan paparan polusi udara.

Abstract

Deep vein thrombosis (DVT) is part of venous thromboembolism disease dan the incidence increases sharply with age. In European countries, DVT incidence is 70-140 cases/100,000 person-year. Patient with DVT, short-term mortality rates were 2-5% and more frequent in proximal than distal DVT. DVT is a common condition for complication such as post phlebitic syndrome, pulmonary embolism and death. An algorithm strategy combining pre-test probability, D-dimer testing and compressible Doppler ultrasound imaging can be applied for suspected lower extremity DVT. Patients with low pre-test probability and a negative D-dimer test result can be excluded from diagnosis of DVT without the need for Doppler ultrasound imaging. Primary treatment of DVT is anticoagulation therapy, intervention using thrombolysis and vena cava filters are for special conditions. The duration of anticoagulation therapy depends on the risk factors.

Abstract

Hypertension is one of non communicable disease which has a high prevalence in Indonesia. Based on Basic Health Research in 2018 the are N 658.201 Indonesian citizens with hypertension. Hypertension have some of the risk factor include gender, age, and sedentary lifestyle. Physical activity can be a risk factor of hypertension if people do it rarely. This study use a quantitative approach. The research method is using a retrospective cross-sectional study. The study will be using the GPAQ questionnaire and blood pressure measurement. This study determine the relationship between physical activity and blood pressure in patients with essential hypertension. Data analysis of this research is using Spearman test. The result of data analysis showed that there was a significant relationship between physical activity and blood pressure in hypertensive patients at the Ciledug Health Centre p = 0,00, the result of this study have a fairly strong correlation with a negative correlation (r= -0,59). The conclusion of this study is there is a significant relationship between physical activity and blood pressure in patients with essential hypertension at the Ciledug Health Center.

Abstract

Confidentiality of patient personal data is also a patient's right which is regulated in Law number 29 of 2004 and Law number 44 of 2009. Along with the development of technology, some loopholes can be exploited by cybercrime perpetrators. in general, they use malware programs to hack the target's computer system. The Information and Electronic Transactions Law (UU ITE) articles 30 and 31 regulate illegal access to systems belonging to other people or the public. Articles 32 and 34 also regulate illegal access and transactions with electronic documents owned by other people or the public. Violators can be charged using article 48 of the ITE Law. To date, no law specifically regulates the crime of hacking using malware on artificial intelligence technology used in hospitals. This research method is normative juridical with descriptive research methods. The specific legal basis (lex specialis) that regulates the protection of patient personal data concerning the illegal buying and selling of patient data (information business and electronic technology) that uses AI does not yet exist in the law, the closest thing is Article 67 of the Personal Data Protection Law in a general context. In this case, it can result in a legal vacuum, legal uncertainty or uncertainty about statutory regulations, or legal chaos.
