Abstract

Background: Since reformation era in 1998, the activities of the integrated health service post (posyandu) have been declining. In order to revitalize posyandu activities, revitalization strategy was introduced. The purpose of this study was to examine factors associated with posyandu cadre activities in Jember, East Java, including the effectiveness revitalization strategy.Subjects and Method: This was an analytic a cross-sectional study, conducted in Jember, East Java. A sample of 80 integrated health service post (posyandu) cadres was selected for this study. The dependent variable was cadre activity. The independent variables were age, knowledge, education, employment, duration, training, and revitalization strategy. Questionnaire was used to measure variables. Logistic regression was used to analyze data.Results: Duration ≥10 years (OR= 1.53; 95% CI= 0.42 to 5.61; p= 0.524), education ≥high school (OR= 2.35; 95% CI= 0.47 to 11.76; p= 0.299), housewife (OR= 9.58; 95% CI= 1.60 to 57.21; p= 0.013), ever had training (OR= 4.38; 95% CI= 1.21 to 15.77 p= 0.024), good revitalization strategy (OR= 3.71; 95% CI= 1.03 to 13.36; p= 0.045) were associated with increased cadre activity. Age 26-45 years old (OR= 0.35 ; 95% CI= 0.01 to 17.81 p= 0.604), age ≥46 years old (OR= 0.18; 95% CI= 0.01 to 3.66; p= 0.262), low knowledge (OR= 0.80; 95% CI= 0.16 to 4.02; p= 0.786), poor knowledge (OR= 0.40; 95% CI= 0.06 to 2.89; p= 0.367) were associated with decreased cadre activity.Conclusion: Housewife, ever had training, and good revitalization strategy are factors that increase cadre activity, and they are statistically significant. Abstract

Background: Tuberculosis is a communicable disease that remains a major public health issue worldwide. Tuberculosis epidemics have become a primary public health concern for the last few decades. The problem becomes aggrevated due to the emergence of Multiple Drugs Resistant Tuberculosis (MDR-TB). It is estimated there are 6,100 MDR-TB cases annually in Indonesia. Grobogan is one of the 6 districts in ex-residentialPatiin Central Java with the second highest MDR-TB cases after Kudus. Between 2011 and 2016 the annual incidence was 23 cases with case fatality rate of  52.17%. This study aimed to investigate the different roles of MDR-TB patients, families, TB  progammers, and health providers, in the incidence of MDR-TB in Grobogan, Central Java.Subjects and Method: This was an analytic qualitative study with case study approach. The study was conducted in Grobogan, Central Java, from May to June 2017. A total of 26 informants were selected purposively for this study, consisting of 7 MDR-TB patients, 7 family members who served as drug-taking supervisor, 7 TB programmers at Community Health Centers, 1 TB programmer at District Hospital, 3 Community Health Center doctors, and 1 District Hospital doctor. The data were collected by in-depth interview, direct observation, and document review. Interactive analysis was used to analyze data, including data collection, reduction, presentation, and verification.Results: All MDR-TB patientshad favorable perceived susceptibility, perceived severity, and self-efficacy, that drove them to adhere to TB treatment.  Most patients reported that the treatment was beneficial. The existing barrier was not of serious concern that made them to stop treatment.Fa­milies of the patients had positive attitude towards TB treatment so they provided the necessary support in acessing treatment and adherence to treatment. However, TB program management at District Hospital, was sub-optimal as it did not administer appropriately the standard DOTS-TB treatment guideline, despite the existence of TB standard operating procedure. Likewise, TB treatment management by hospital doctors and private practice doctors was inadequate, due to the lack of DOTS-TB training. The lack of adherence in implementing thestandard DOTS-TB treatment guideline was the dominant causal factor for the incidence of MDR-TB in Grobogan district, Central Java.Conclusion: Adequate DOTS-TB treatment management and quality health services at both primary and secondary level facilities are needed in the efforts to prevent MDR TB. It is suggested that the government through the District Health Office hold DOTS-TB promotion program and invest in developing skilled DOTS-TB providers.Keywords:multidrug resistance, tuberculosis, TB management program, adherence.Correspondence: Gunawan Cahyo Utomo. Abstract

Background: Health law number 36 of 2009 article 4 states that everyone has the right to be healthy. The right applies to all people, including the prisoners even though they have broken the law. This study aimed to describe the quality of health of female prisoners in obtaining health services during detention in Semarang's female prison.Subjects and Method: This was a qualitative study with a single fixed case study research strategy. The study was conducted in November 2018. The key informants were 4 female prisoners, 1 health policlinic doctor, and 1 head of a correctional institution.Results: There was a polyclinic unit that provided health services. Prisoners were not sure that health services are often given drugs that do not provide benefits optimally. Health personnel are always prepared to provide health services outside of working hours even though the number of health personnel is limited. Referral is done by conducting a basic examination to obtain an initial diagnosis to determine the next action, referral is done with the permission of members of the correctional observer team (TPP) and the head of the prison then referral can be done at facilities that have collaborated and have MOUs with prison so that prisoners do not pay for treatment. The availability of food and beverages consumed is provided in sufficient quantities. The availability of sports facilities to support health, as well as maintaining personal hygiene of prisoners is carried out by bathing, washing clothes, cleaning beds, and disposing of sanitary napkins in the places provided.Conclusion: The lack of health personnel causes sub-optimal health services. The lack of effectiveness of the drugs causes convictions of prisoners with health services provided in prisons.Keywords: quality of health, female prisonersCorrespondence: Alvy Nur Hidayati. Abstract

Background: Acupuncture services in Indonesia keep developing. Law No. 36 of 2014 concerning health personnel has confirmed the status of acupuncture in Indonesia. The regulation includes acupuncture health personnel in the physical therapy group not as traditional ones. However, there are still scientific publications in 2018 which state that acupuncture services are traditional health services. This study aimed to describe the services of acupuncture and acupuncture health personnel.Subjects and Method: This was a qualitative descriptive one. It was conducted in Surakarta City in November 2018-January 2019. It employed a purposive sampling technique to determine informants. The informants in this study consisted of 5 informants, namely 1 executive board of the Indonesian therapist acupuncture association (HAKTI) center, 1 Central Java HAKTI adminis­trator, 1 Surakarta Ministry of Health Polytechnic lecturer at the Acupuncture Department, and 2 therapists and acupuncture service providers. The data collection in this study was done using the technique of in-depth interviews, observation, and document analysis. The data analysis was conducted using the method of Miles and Huberman.Results: Acupuncture service is an effort to restore non-pharmacological health using therapy or needle stabbing to help smooth the flow of energy and blood so that the body can be healthier by acupuncturists with a minimum of three diploma education. Minister of Health Regulation No. 34 of 2018 is a special regulation on licensing and the practice of therapist acupuncturists. The purpose of this regulation is to provide a legal protection for health care personnel who are acupuncturists who are about to practice, both practice independently and join health care facilities. This regulation is appropriate because it writes health centers as one of the acupuncture services. receiving acupuncture services, until now the acupuncture service has no definite provisions regarding service fees.Conclusion: Acupuncture service is health recovery efforts using needle puncture techniques. Abstract

The aims of the study to describe : ( 1 ) the application use CTL model with audio-visual media to increase motivation study class VIII E of SMPN 1 kedawung District of Sragen ; ( 2 ) the application of learning model use CTL with audio- visual media to enhance the student’s learning achievement class VIII E of SMPN 1 kedawung District of SragenThis study uses a Class Action Research ( CAR ) is a means to investigate , refine and evaluate learning.The proceses are carried out with 4 stages there are, including planning , implementation , observation and reflection . The fourth of stage are forming a cycle , carried out collaboratively. The subjects of this research is grade VIII E students of SMP Negeri 1 Kedawung. The data comes from students and teachers ,there are the student's motivation and student’s achievement in learning activities. The Data collection techniques is done by observation, interviews, document analysis and tests. This research uses descriptive analysis of data and compares the findings with performance indicators.The results showed that: (1) the application of learning models CTL with audio-visual media in teaching social studies can enhance student motivation class VIII E of SMPN 1 kedawung; (2) the application of learning model use CTL with audio-visual media in learning activity can improve student achievement class VIII E SMPN 1 kedawung. The Increased student motivation can be seen from the results of student motivation questionnaire after implementing the action. in the first cycle reaches 75.87%, then the cycle II reached 82.78% and the third cycle increased to 83.80%). While student achievement on the initial conditions shows that the average value of 73.12 with 71.87% classical completeness, after the action on the first cycle increased the average value becomes 73.12 with classical completeness amounted to 71.87%, second cycle increased with an average value of 75.03 with classical completeness 75%, and the third cycle experienced a significant increase in the average value reached 80.12 by 84.38% classical completeness.

Abstract

This article analyzes the existence of Madrasah Mambaul Ulum, HIS Kasatryan and HIS Pamardi Putri in the political constellation of the movements in 1920s.These educational institutions were established in the area of the Kasunanan Palace. Madrasah Mambaul Ulum consisted of elementary school, junior high school and senior high school, catering for children of abdi dalem (servants in the palace) and those of lay people. On the other hand, HIS Kasatryan and HIS Pamardi Putri were elementary schools for children of lower aristocrats. The founders of the institutions gave freedom to the managers to make decisions concerning education (school-based management) in terms of the curriculum, teachers, learning materials, learning strategies, and evaluation system. Even society members were allowed to take part in the implementation (community-based education). The local contents, the Javanese culture and Islam were the subjects that guided the students ethics and moral so that they developed as individuals who were responsible and just and capable of protecting their fellow people.
