Abstract

ABSTRACT Acute heart failure is a clinical syndrome with a complex of symptoms that is typical shortness of breath at rest or during activity. Acute heart failure is mostly caused by coronary heart disease. Coronary heart disease is a heart failure disease with narrowed coronary arteries can be caused by the buildup of a layer of fat on the walls of the coronary arteries. The purpose of this study was to determine the initial management of patients with acute heart failure due to coronary heart disease in the Integrated Heart Service Emergency Room, Sanglah Hospital. This study is a descriptive study using a cross sectional research method on a sample of 43 people who came to the PJT Emergency Room at Sanglah Hospital with a diagnosis of acute heart failure due to coronary heart disease in 2019-2020. The data that has been obtained were then analyzed using SPSS version 24. The results showed that most of the patients 30 sample (69.77%) were male compared to women, there were 13 sample (30.23%). The majority of patients with acute heart failure due to coronary heart disease come to the Emergency Room with complaints of shortness of breath. Initial treatment given in the Emergency Room included intravenous diuretic Furosemide with a dose range of 20-80 mg/IV (93.02%), Beta blockers (2.35%), Nitroglycerin (13.95%), Digoxin (13.95%), and Dobutamine (30.23%). Most of the patients in this study were given Furosemide as an initial treatment according to the PERKI 2020 guidelines which suggest the administration of Furosemide as a treatment for fluid congestion in acute heart failure. Some patients require inotropic support such as Dobutamine which is adjusted according to individual clinical criteria. Supporting examinations were carried out on all study samples (100%) including electrocardiogram (ECG) and chest X-ray examinations. Abstract

Background: Recent studies have identified that chronotropic incompetence is correlated with poor cardiometabolic health and systemic inflammation that results in exercise intolerance, impaired quality of life and death due to cardiovascular disease (CVD). Unfortunately, there’s still paucity of data regarding cardiometabolic factors associated with chronotropic incompetence. The purpose of this study was to identify the cardiometabolic factors associated with chronotropic incompetence. Methods: This study was a cross-sectional retrospective study using cardiac treadmill stress test data at Sanglah General Hospital from May 2018 - May 2020 and 136 patients were enrolled. Data analysis used SPSS version 21. Pearson chi-square test was used to compare categorical variables based on cardiometabolic risk factors in chronotropic incompetence. Results: Patients were divided based on the characteristics of age, gender, smoking status, body mass index, coronary artery disease, heart failure, hypertension, dyslipidemia, type 2 diabetes mellitus (T2DM), the levels of HbA1C, total cholesterol, LDL, HDL, and triglyceride. In this study, it was found that T2DM (PR 2.29; 95%CI 1.16–3.37), HbA1C (PR 3.13; 95%CI 2.31-4.22), dyslipidemia (PR 1.773; 95%CI 1.170–2.687), high total cholesterol (PR 2.396; 95%CI 1.650-3;481), and high LDL level (PR 1.853, 95%CI 1.229-2.794) were significantly associated with chronotropic incompetence (all p-value <0.05), while other factors were not significantly related. Conclusion: Chronotropic incompetence can impair quality of life and contribute to cardiovascular mortality. However, T2DM, high HbA1C, dyslipidemia, high total cholesterol and LDL levels were found to be associated with chronotropic incompetence. This may contribute to higher cardiovascular risk attributed to those factors.

Abstract

This research outlines an analysis of Psychological Well-Being in children who are in the final stages of primary school. The study used a literature review method with the aim of investigating and analysing a deeper understanding of aspects of psychological well-being in children of this age. Through analysing a number of literature sources, this study identified key factors that play a role in influencing psychological well-being in children in the late primary grades. These factors include, but are not limited to, social interactions at school, social support from family and peers, emotional development, adjustment skills and a supportive learning environment. The results of the analyzes suggest that psychological well-being in high school children is closely related to various psychosocial and environmental factors. Moreover, a better understanding of the relationship between these aspects and children's psychological well-being can provide valuable insights for educators, parents and mental health professionals in developing appropriate strategies and interventions to support positive psychological development in children at the primary school level. In conclusion, this study highlights the importance of a holistic understanding of the factors that influence psychological well-being in children in the upper primary grades. The implications of this study are expected to provide a foundation for the development of educational approaches and interventions aimed at improving the psychological well-being of children at this level.

Abstract

The results of the Computer Based National Assessment (ANBK) indicate that students continue to experience difficulties in reading literacy, underscoring the need for more interactive digital-based learning media innovations. This study aims to develop open-source website literacy to enhance students' reading literacy, focusing on criteria such as feasibility, practicality, and effectiveness. The research method uses the ADDIE development model. The subjects in this study were divided into 2 two groups in the feasibility test and the practicality test. Data were collected through various methods, including both non-test and test-based approaches, using questionnaires and multiple-choice questions. The feasibility test was conducted using the BSNP instrument, the practicality test was assessed with the User Experience Questionnaire (UEQ), and the effectiveness test was evaluated through a one-group pretest-posttest design. The results showed that open-source website literacy had a feasibility test with an average score of 3.82 (feasible category). The practicality test by teachers obtained a score of 2.46 (very practical), while students gave a score of 1.75 (practical). The effectiveness test, using the SPSS 26 t-test, yielded significant results. (2-tailed) = 0.000 < 0.05, indicating a statistically significant increase in students' reading literacy scores from pretest to posttest. The study's conclusion demonstrates that open-source website literacy is a feasible, practical, and effective approach to improving the reading literacy of fifth-grade elementary school students. The implications of this study confirm that integrating technology into literacy learning can increase student engagement and provide a more engaging learning experience.
