Abstract

Instrumen endodontik patah merupakan salah satu komplikasi yang paling sering terjadi pada perawatan saluran akar. Pengambilan intrumen yang patah dari saluran akar merupakan prosedur yang sulit, terutama ketika menembus hingga periapikal. Laporan kasus ini bertujuan menyajikan penatalaksanaan bedah pengambilan instrumen endodontik patah pada periapikal gigi molar pertama maksila. Seorang pria berusia 29 tahun dirujuk ke departemen konservasi gigi karena mengeluhkan sakit yang tumpul dan intermiten pada gigi molar pertama kiri atas dan bertambah sakit ketika menggigit. Pasien menjelaskan telah menjalani perawatan saluran akar yang tidak lengkap sekitar 2 bulan yang lalu. Pemeriksaan klinis menunjukkan terdapat tumpatan sementara pada permukaan oklusal molar pertama kiri atas dan respon positif pada pemeriksaan perkusi. Berdasarkan pemeriksaan radiograf periapikal, terdapat sebuah segmen file patah yang ekstrusi keluar dari akar mesio bukal dengan obturasi saluran akar yang tidak hermetis pada saluran akar. Pengambilan instrumen patah dilakukan dengan bedah periapikal (teknik retrograde), selanjutnya dilakukan reseksi akar mesiobukal. Penatalaksanaan instrumen endodontik patah hingga daerah periapikal dengan bedah periapikal merupakan pilihan perawatan yang menunjukkan hasil dan prognosis yang baik.

Abstract

Introduction: Functional crown lengthening is one of the most common surgical procedures that facilitating restorative treatment. It was done on teeth with inadequate clinical crowns in the presence of deep and subgingival pathologies. Inadequate clinical crowns defined as tooth with less than 2 mm cervico-incisal of sound. Case Report: The 32 years old female patient complained on broken restoration on upper left anterior tooth since one week ago. He also complained about upper right anterior tooth that turned brown. The tooth had received root canal treatment with direct composite restoration since 3 years ago, but the restoration on tooth 11 and 21 was broken. The remaining crown on tooth 11 and 21 was less than 2 mm. The periapical radiograph examination showed tooth 11 and 21 was non-hermetic obturation. Functional crown lengthening and root canal treatment was performed on teeth 11 and 21, with porcelain crown restoration and fiber post. Conclusion: Functional crown lengthening result affects the quality of post retreatment restoration. The success of functional crown lengthening is marked by no recurrent gingival hyperplasia after functional crown lengthening.

Abstract

Introduction: Hemisection is sectioning and removal of one root and its crown portion on multi-rooted teeth. Hemisection denotes a conservative treatment option for large bifurcation perforation, loss of periodontal attachment and was performed to maintain the original teeth structure. This case report aims is to present the management of hemisection of a mandibular first molar with crown rehabilitation. Case and Management: A 16-year-old male complained of pain while biting on his right lower teeth. Clinical examination showed deep caries on the occlusal surface, the tooth has a positive response on percussion and palpation tests. The mobility test was positive on the mesial root and crown. Based on periapical radiographic examination, there was a radiolucent feature extended to the bifurcation area. The diagnosis was pulp necrosis and symptomatic apical periodontitis accompanied by bifurcation perforation. Root canal treatment and hemisection were performed to remove infected tissue, and the remaining part of the teeth was restored with a porcelain-supported fixed prosthesis. Conclusion: Hemisection can be a suitable conservative treatment for teeth with extensive damage and has a good long-term prognosis.

Abstract

ABSTRAK Kesehatan gigi dan mulut merupakan pintu  utama yang mempengaruhi kesehatan sistemik secara keseluruhan. Seperti kita ketahui gigi berfungsi untuk pengunyahan, berbicara (fonasi), estetika dan perlindungan terhadap jaringan pendukung gigi. Upaya peningkatan kesehatan gigi dan mulut  dilakukan agar masyarakat dapat tetap tersenyum, gigi tetap sehat dan berfungsi dengan baik. Mengajak masyarakat untuk selalu menjaga kebersihan mulut, meningkatkan kualitas kesehatan gigi dan mulut, mengajak masyarakat untuk selalu kontrol ke dokter gigi minimal 6 bulan sekali dan memberikan edukasi tentang kesehatan gigi dan mulut kepada masyarakat. Pada kegiatan ini dilakukan perawatan dalam upaya preventif  meliputi penyuluhan, edukasi dan sikat gigi masal kepada anak-anak Taman Kanak-kanak, Sekolah Dasar, dan SLB. Upaya kuratif dengan melakukan perawatan gigi sederhana seperti penambalan gigi pada gigi berlubang, scaling pada  peradangan pada gusi yang ringan dan sedang, serta pencabutan gigi anak pada gigi goyah dan gigi dewasa yang sudah tidak dapat dipertahankan kembali. Upaya preventif berupa sikat gigi masal dilakukan pada anak-anak dengan sebaran taman kanak-kanak 25%, sekolah dasar 43%, PAUD 18% dan anak SLB 14%. Upaya kuratif yang dilakukan dalam bidang bedah mulut berupa pencabutan gigi tanpa komplikasi sebanyak 8,5%, konservasi gigi meliputi penambalan gigi dengan resin komposit sebanyak 25.3% , bidang periodonsi berupa pembersihan karang gigi sebanyak 48,1%, perawatan pada anak berupa topikal aplikasi dan fissure sealant sebanyak 20,8% dan perawatan lain-lain seperti premedikasi dan dental health education (DHE) sebanyak 5,9%. Kegiatan Bulan Kesehatan Gigi dan Mulut Nasional yang dilakukan di RSGM, UMY mampu melakukan upaya preventif berupa penyuluhan sejumlah 125 peserta dan sikat gigi masal sejumlah 277 siswa dan upaya kuratif dengan berbagai perawatan sebanyak 1207 pasien. Kegiatan ini mampu memecahkan masalah kesehatan gigi dan mulut pada masyarakat Yogyakarta.  ABSTRACT Oral health is the main entry that affects overall systemic health. As we know, teeth function for mastication, speech (phonation), esthetics, and protection of the supporting tissues of the teeth. This effort is made so people can keep smiling because their teeth are healthy and functioning correctly. This activity aims to invite the public always to maintain oral hygiene, improve the quality of dental and oral health, invite the public to control the dentist at least once every six months continuously, and provide education about dental and oral health to the public. In this activity, preventive maintenance was carried out, including counseling, education, and mass tooth brushing for children in Kindergarten, Elementary Schools, and Special Schools. Curative efforts by carrying out simple dental treatments such as dental fillings for cavities, scaling for mild and moderate inflammation of the gums, and removing children's teeth for loose and adult teeth that can no longer be maintained. Preventive efforts in the form of mass toothbrushes were carried out on children with a distribution of 25% kindergartens, 43% elementary schools, 18% PAUD, and 14% SLB children. Curative efforts were made in the field of oral surgery in the form of a tooth extraction without complications as much as 8.5%, dental conservation including dental fillings with composite resin as much as 25.3%, in the field of periodontics in the form of cleaning tartar as much as 48.1%, treatment in children in the form of topical applications and fissure sealants as much as 20.8% and other treatments such as premedication and dental health education (DHE) as much as 5.9%. During the National Dental and Oral Health Month activity at the Academic Dental Hospital, UMY carried out preventive as and curative efforts with various treatments for as many as 1207 patients. This activity can solve dental and oral health problems in Yogyakarta.

Abstract

Background: The polishing procedure was an important step in composite resin restoration as it reduces restoration surface roughness. Smooth and well-polished restoration surfaces can increase microhardness and minimize discoloration on the tooth surface. The study aims to examine the differences of one-step polishing systems on surface roughness, microhardness, and color stability of bulk-fill composite resin. Method: The samples were bulk-fill composite resin moulded with 4 mm of diameter and thickness (n=30) for microhardness (n=15) and surface roughness (n=15) test and moulded with 6 mm of diameter and 4 mm of thickness for color stability (n=15) test. The samples were divided into three groups consisting of 5 samples for each variable and were polished using PoGo, OptraPol, and OneGloss. Color stability test samples were immersed in tea solution for 7 days. Color stability measurement was done by comparing the results of color measurements using a spectrophotometer before and after immersion. Microhardness was tested using Vickers Microhardness Tester, while surface roughness was tested using the Stylus Profilometer Fowler Surfcorder SE1700. Result: The data analysis showed that PoGo produced the smoothest surface roughness significantly (p=0.006), the highest microhardness significantly (p=0.002), and the lowest color change significantly (p = 0.027) Conclusion: PoGo produced lowest surface roughness and color change, and highest microhardness compare with OptraPol and OneGloss.

Abstract

Caries are still a significant problem in oral and dental health worldwide, including Indonesia. Various efforts must continue to be made to reduce the incidence of caries, both promotive and preventive, as well as curative and rehabilitative. Based on the Riset Kesehatan Dasar 2018, 36.71% of the people of Padang City have cavities/injured/toothache. Of the youth aged 15-24, 37.45% had cavities/injured/toothache. This program aims to get an overview of caries classification and provide appropriate treatment. The overview of caries classification can become data on the pattern of caries development so that strategies for dealing with earlier caries can be carried out appropriately, effectively, and efficiently. The implementation of the present program was carried out through a thorough and comprehensive examination of the oral cavity; then, treatment was carried out according to the complaints and cases found. The post-treatment evaluation was carried out by distributing questionnaires to participants afterward. Based on the depth of caries treated, 57.14% were dentin caries. Caries classification based on G.V. Black showed that Class I cavity was the highest incidence (64.29%). Mount and Hume's classification presented class 1.3 (39.28%) as the highest, while based on the ICDAS classification, it was class D3 (46.44%). The post-treatment evaluation showed that all participants (100%) stated that the condition of the teeth improved and that there were no complaints/discomfort after treatment.
