Abstract

Objective: Determining the survival rate of early stage cervical cancer patients after radical hysterectomy. Method: A retrospective cohort study was conducted on 24 earlystage cervical cancer patients who had performed radical hysterectomy in Prof. dr. R.D. Kandou Hospital Manado during the period between January 2008 and December 2010. We used Kaplan-Meier methods to observe the survival rate. Result: The mean age of patients in this study was 47.8 (33-63) years old with a median of 45 years old. The largest proportion was less than 50 years old (66.7%) and stage IIA cervical cancer (66.7%) as the severity of cancer. Most histopathology type was the squamous cell carcinoma (50.0%). About 70.8% patients did not have lymph nodes metastasis and 62.5% patients did not receive adjuvant therapy. This study revealed that 1-year, 2-year, 3-year, 4-year, and 5-year survival rate were 100.0%, 100.0%, 95.8%, 83.0%, 70.8%; respectively. Conclusion: The survival rate of early stage cervical cancer afte[Indones J Obstet Gynecol 2016; 4-3: 164-169] Keywords: cervical cancer, radical hysterectomy, survival rate

Abstract

Objective: To determine the sensitivity and specificity of neutrophil - lymphocyte ratio compared with platelet - lymphocyte ratio in predicting ovarian malignancies. Methods: This was an observational analytic study with diagnostic test design on 37 ovarian tumor patients who were planned to perform laparotomy in Prof. Dr. R.D. Kandou Hospital Manado and network hospitals in Manado from March to May 2016. We took blood samples to assess the complete blood count. Results: During the period, we obtained the mean age of 28.1 years old (range 18-71 years old). A total of 13 samples showed malignancy which the cystadenocarcinoma mucinosum (27.0%) as the most common histopathological types. Sensitivity and specificity value of neutrophil - lymphocyte ratio were 92.3% and 95.8% with 2.47 as the cut-off point. Meanwhile, the sensitivity and specificity of platelet - lymphocyte ratio were 61.5% and 75.0% with 152.86 as cut-off point. Conclusion: Both neutrophil - lymphocyte ratio and platelet - lymphocyte ratio are associated with ovarian malignancy. The sensitivity and specificity of neutrophil - platelet ratio show better prediction for ovarian malignancies. Abstract

Objective: To determine the ratio of serum endothelin-1 levels between severe preeclampsia and normotensive pregnancy. Methods: Observational analytic study using cross-sectional. Sixteen woman with normal pregnancy and sixteen others with severe preeclampsia who met the inclusion and exclusion criteria, were tested for ET-1. The serum was analyzed at Prodia Laboratory, Manado. The ET-1 level was examined using ELISA (R&D Systems, Inc., Minneapolis, MN 55413, USA). The data obtained was analyzed using SPSS software version 20.0.and discussions were held using the existing literature theory. Results: The mean and median levels of endothelin-1 plasma in patients with severe preeclampsia is 2:46 ± 1:44 pg/ml, 1:09 ±  0:26 pg/ml, whereas in normotensive pregnancy is 1:03 ±  0:26 pg/ml, 1.95 ± 1:44 pg/ml with p < 0:05 (0000). Conclusion: There was a significant difference between endothelin- 1 level in severe preeclampsia and normotensive pregnancies. Abstract

Objective: To determine the effect of hysterectomy on bladder function pre- and post-radical hysterectomy in early stage of cervical cancer. Method: This study was a pre-post intervention study. Data were collected through questionnaires from women who had radical hysterectomy and total hysterectomy in Prof. Dr. R. D. Kandou Manado general hospital and other networking hospitals since January 1st, 2014 to November 31st, 2014. We analyzed the data using Wilcoxon and Mann-Whitney statistical test. Result: There were each 18 respondents for the radical and total hysterectomy group in Prof. Dr. R. D. Kandou general hospital and networking hospitals. The age distribution of radical hysterectomy was 41-45 years old for 44.4%. The parity was dominated by second parity for 38.8%. In pre- and post-surgery, there were significant differences for urinary incontinence disorder (p=0.003), emptying disorder (p=0.008), urinary pain (p=0.034), and total urinary disorder (p=0.001). While, between radical and total hysterectomy, there was no significant difference in bladder function (p>0.05). Conclusion: There is an association before and after surgery to urinary function. However, there is no association between the radical and total hysterectomy group. [Indones J Obstet Gynecol 2016; 4-2: 97-100] Keywords: bladder function, cervical cancer, radical hysterectomy

Abstract

Objective: To know the relationship between obesity and gynecology cancer. Method: This study use case control study design for 250 gynecology patients (125 controls and 125 cases) in Prof. Dr. R. D. Kandou Manado Hospital from 1 July to 30 November 2015. The data was collected by measuring Body Mass Index (BMI) and filing out selfadministered questioners. Result: From the 250 subjects, the study group (125 subjects), 72 subjects have obesity (57.6%) and 97 subjects have multiple parities (77.6%) with 58 subjects diagnosed with cervical cancer (46.4%). In the control group (125 subjects), 71 subjects have normal weight (56.8%) and 67 subjects have multiple parities (53.6%) with 64 subjects diagnosed with ovarium cysts (51.2%). Using multivariate logistic regression, the overweight and obese subjects have 7 folds higher risk to develop gynecology cancer compared to those with normal or underweight subjects. Those with multiple parities and grande multipara subjects have 3 folds higher risk to develop gynecology cancer compared with those who are nullipara and primipara. Conclusion: A significant correlation is found between obesity and gynecology cancer using multivariate logistic regression analysis (p=0.000, OR=6.9 (95% CI = 3.62-13.13).
