Abstract

EFFECT OF EARLY MOBILIZATION TO URINE RETENTION EVENT POST HERNIOTOMY OPERATION IN RSUD PROF DR SOEKANDAR MOJOSARI By :Lutfi Wahyuni, Binarti Dwi Wahyuningsih Hernia is a problem that can be found in the case of surgery and became the most cases of surgery after appendicitis. Emergency emergency cases can occur if the hernia is sedentary where there has been a blockage in the food channel which eventually the blood vessels caught should be surgery called herniotomy. Many problems arise from surgery and the risk of anesthesia (spinal anesthesia), including the presence of urinary retention events. This study aims to identify the incidence of urinary retention in postoperative herniotomy patients after early mobili- tation. The design used in this study was pre-experimental with a one-shot case study design. The population in this study were all patients who had undergone herniotomy surgery at 2nd grade inpatient room in RSUD Soekandar Mojosari in February 2017 as many as 27 people. Samples were taken using consecutive sampling technique. Data analysis using descriptive analysis. The result of the research showed that in stage 1 of 30 respondents (95%) who experienced urine retention decreased to 9 respondents (30%) in phase 2 subsequently decreased to 2 respondents (6,6%) in stage 3 and at stage 4 there were no respondents experience urinary retention. There is an effect of early mobilization on the incidence of urinary retention with different responses in each respondent. Early mobilization is influenced by physical factors, emotions and age and developmental status. Abstract

ABSTRACT Chronic kidney disease is a condition when kidney function begins to decline gradually. The condition of the decline in kidney function occurs due to many factors, one of which is because the kidney damage that has occurred intensely for many years. Patients with chronic renal failure need a management or hemodialysis therapy, which is the cleansing process of blood from waste substances, through a screening process outside the body using an artificial kidney in the form of a dialysis machine. Hemodialysis therapy is needed in the management of patients with chronic renal failure who cannot be treated again (terminal). The success of hemodialysis is based on various elements, one of which is compliance with fluid restrictions. Limitation of fluid intake in patients with chronic renal failure is very important. The provision of structured education can be given to patients with chronic kidney failure who are undergoing hemodialysis therapy. Education structured nutrition and electrolyte (ESNE) is a structured health education on nutrition and electrolyte management for chronic renal failure patients undergoing hemodialysis. ESNE is done to improve patients' understanding of diet and electrolyte compliance to prevent an increase in IDWG and to improve patient compliance with fluid restrictions. The stages of service activities that have been carried out are to explain fluid requirements and fluid restriction in patients with chronic renal failure using the Education structured nutrition and electrolyte (ESNE) method, the next recourse is an evaluation after continuous or ESNE administration of respondents given treatment and respondents who were not treated. Abstract

Cardiac arrest is a condition where the heart is unable to pump blood at all as a result of abnormal heart rhythms that are not effective. Response time is the speed and accuracy of service time needed by patients to get help in accordance with the emergency of the disease since entering the emergency room. Good response time for patients is <5 minutes if it passes it can result in the lives of patients in danger and can even cause death. The purpose of this study is to determine the relationship of response time to the emergency actions of cardiac arrest patients. The design in this study was an analytical correlation using a cross-sectional approach. The population in this study were all nurses in the IGD and ICU in Sakinah Mojokerto Hospital and Hasanah Mojokerto Hospital in 52 nurses. The sampling technique used is total sampling. The instrument used was a stopwatch and SOP observation sheet for handling Cardiac arrest patients. The results showed that most nurses had response time ≤ 5 minutes as many as 35 respondents (67.3%) and the accuracy of emergency actions in cardiac arrest patients according to SOP were 40 respondents (76, 9%). The faster the nurse performs the response time it is expected that the more appropriate in performing emergency actions in cardiac arrest patients

Abstract

Chronic kidney disease is a failure of kidney function to maintain metabolism, and balance of fluid and electrolytes due to the progressive destruction of kidney structure. Increased fluid volume must be limited. If there is excess can cause edema, tightness and decreased consciousness. Therefore, patient motivation is needed to control changes of Interdialytic Weight Gain (IDWG). The aim of this study is to prove the relationship of patient motivation with changes of IDWG in CKD patients. The design of this study was correlational analysis with a cross-sectional approach. The sampling technique used non-probability sampling type purposive sampling, 40 CKD patients undergoing hemodialysis therapy at the RSUD Bangil were enrolled in this study. Data collection was using a questionnaire. Data analysis was using Spearman rho statistical test, which showed a significance value of 0,000 that was smaller than alpha (0.05), so it can be concluded that there was a significant relationship between patient motivation and IDWG changes of CKD patients undergoing hemodialysis therapy. Strong motivation to undergo hemodialysis is important because hemodialysis has become a necessity for him, a treatment must be undertaken by patients with chronic kidney disease.

Abstract

Chronic kidney disease (CKD) is a long-term disease where the kidneys have decreased function. In women who experience prolonged CKD, other problems will arise, namely Menstrual abnormalities. CKD patients who have just undergone hemodialysis therapy are prone to stress, if not treated immediately it will disrupt the menstrual cycle. This research design uses correlation analytic with cross sectional approach. The population in this study were 49 women who suffered from CKD in the hemodialysis unit of RSUD Prof. Dr. Soekandar. Sampling using non-probability sampling technique Consecutive sampling type within a period of 3 weeks. The sample in this study amounted to 43 people. The instruments used in this research are DASS questionnaire and Menstrual abnormalities checklist sheet. Data analysis using logistic regression test. The results showed that 20 respondents (46.51%) had CKD >4 years. For stress levels, it was found that 17 respondents (39.53%) were at mild stress levels. And 39 respondents (90.70%) experienced Menstrual abnormalities. The results of the logistic regression test showed that the p-value = 0.048. It can be concluded that there is a long-standing relationship between chronic kidney disease and stress with menstrual abnormalities in the hemodialysis unit of RSUD Prof. Dr. Soekandar. Women who suffer from CKD in their bodies will experience hyperprolactinemia which will disrupt the menstrual cycle. A person who has just been diagnosed with CKD is prone to stress, which stress will stimulate the release of Corticotropic Releasing Hormone (CRH). Hyperprolactinemia and CRH can affect the performance of Gonadotropin-releasing hormone (GnRH) which lead to menstrual abnormalities.

Abstract

When Isoman This community service activity aims to increase public knowledge about positive thinking, healthy food consumption, and the application of prone position as an effort to recover while undergoing Covid-19 self-isolation. This community service is located at the Wates Health Center, Mojokerto City. The method used in counseling with an educational approach through the stages of (1) pretest (2) educational counseling on the  2P1F method (3) Post-test or evaluation. This activity was carried out in December 2021. The counseling activity contained material presentations related to the  2P1F method, previously respondents were given questionnaires related to positive thinking, food, or food that is good for consumption when exposed to Covid-19 and prone position. The result is that 7 respondents (35%) have good knowledge, 10 respondents have sufficient knowledge, 10 (50%) and 3 (15%) respondents have less knowledge. Furthermore, the material exposure is given by explaining the  2P1F method during isomanism, then respondents are asked to fill out the questionnaire again and the results are an increase, namely about 17 respondents (85%) have good knowledge and 3 respondents (15%) have sufficient knowledge. This proves that the explanation of the  2P1F method is useful for increasing knowledge of how to build positive thinking when exposed to Covid-19, consuming good food to increase immunity and help the healing process, and prone position that are useful for increasing oxygen levels in the blood.
