<![CDATA[Dalam aktivitas supply chain selalu berpotensi untuk timbul risiko, oleh sebab itu manajemen resiko sangat diperlukan untuk penanganan resiko. Pada perusahaan yang memproduksi mesin cetak plastic seperti CV Surya CIP, dalam aktivitas supply chain nya memiliki peluang untuk timbul resiko. Tujuan penelitian ini adalah mengidentifikasi berbagai resiko yang terjadi di CV Surya CIP beserta penyebabnya, menentukan nilai severity dan occurance dari setiap resiko yang terjadi dan menentukan strategi mitigasi resiko yang tepat untuk menanggulanginya. Metode yang digunakan didalam penelitian ini adalah House of Risk Model dengan menggunakan matriks Agregat Risk Potential (ARP) yang dapat mengidentifikasi bobot penyebab dan mitigasi resiko yang paling besar. Metode ini terdiri dari 2 fase yaitu fase pertama untuk mengidentifikasi risiko dan agen risiko, yang kemudian dilakukan pengukuran tingkat severity dan occurance serta penghitungan nilai ARP. Sedangkan fase kedua yaitu penanganan risiko. Dari hasil penelitian menunjukkan bahwa terdapat 21 kejadian resiko dan 31 agen resiko, yang berpotensi terjadi di CV Surya CIP. Dari hasil tersebut kemudian terpilih 2 agen resiko yang menjadi penyebab utama munculnya kejadian resiko yaitu A3 (Kelangkaan bahan baku), A17 (Kualitas bahan baku buruk) dan A28 (Konsumen tidak bisa membayar order). Strategi mitigasi resiko yang digunakan untuk menangani kedua agen resiko tersebut adalah mengembangkan SOP untuk tahap seleksi supplier. In the supply chain activities always has the potential of risk, therefore risk management is necessary for handling the risks. At the company that produces plastic molding machine such as CV Surya CIP, in its supply chain activities have a chance to arise risks. The purpose of this study is to identify the various risks that occur in the CV Surya CIP and its causes, severity and determine the value of occurance of any risks incurred and determine appropriate risk mitigation strategies to mitigate them. The method used in this study is the House of Risk Model using matrix Aggregate Risk Potential (ARP), which can identify the causes and mitigation weights greatest risk. This method consists of two phases: the first phase to identify risks and risk agency, which then measuring the severity level and occurance as well as the calculation of the value of ARP. While the second phase which is handling the risks. The results showed that there were 17 occurrences of risk and 31 agents of risk, which could potentially occur in CV Surya CIP. From these results then elected two agencies risk becoming major cause of the incident, they are the risk of A3 (scarcity of raw materials), A17 (poor quality of raw materials) and A28 (Consumers can not pay order).
