Abstract

Background: Dental treatment that is often done is the tooth extraction procedure. The treatment and the use of sharp instruments such as syringes, pliers, bein during extraction can cause anxiety. Anxiety will be responded by several changes in the body such as the increasing of MAP, pulse rate, and respiratory rate. Handling anxiety can be done with aromatherapy, one is lemon aromatherapy.Aim: To determine the effect of lemon aromatherapy on anxiety levels and the correlation of anxiety levels with the results of MAP, pulse rate, and respiratory rate in tooth extraction patients at RSND SemarangMethod: Experimental study with pretest and posttest control group design with sampling simple random to get 40 respondents that indicated as extraction teeth patient. Respondents were divided into 2 groups: 20 control respondents and 20 intervention respondents. Anxiety level was measured by the Modified Dental Anxiety Scale (MDAS) questionnaire. Data that was not distributed normally were the difference in pulse rate and the difference in respiratory rate, while the results of the homogeneity test obtained data that were not homogeneous were the difference between the MDAS questionnaire and the difference in the MAP. The rest of the mentioned data above were all distributed normally and homogeneous. Analysis of differences in anxiety levels between before and after lemon aromatherapy was given using paired t-test, analysis of differences in anxiety levels between the control group and the intervention group using the independent t-test, and to analyzing the correlation of anxiety levels with the results of MAP, pulse rate, and respiratory rate using the Spearman test..Results: There was a significant difference in anxiety scale between before and after lemon aromatherapy was given p = 0,000135 (p <0.05) and between the control group and the intervention group p = 0.000002 (p <0.05). There was a positive correlation between anxiety levels with the results of MAP, pulse rate, and respiratory rate.Conclusion: Anxiety of tooth extraction patients shows a decreased after inhaling lemon aromatherapy. When someone is anxiety, there will be an increase in MAP, pulse rate, and respiratory rate.Keywords: Anxiety, tooth extraction, lemon aromatherapy, MAP, pulse rate, respiratory rate.

Abstract

Background: Spinocerebellar ataxia (SCA) is a late onset neurodegenerative disorder in which coordination and balance are affected. Although many international guidelines have been established regarding presymptomatic testing, it is still a grey area in Indonesia. We report two large families with advanced stages of SCA who underwent presymptomatic genetic testing in children along counseling process.Case presentation: Thorough examination was performed, including pedigree construction, physical and neurological examination, gene mutation analyses for patients, and presymptomatic testing for family members, including children. SCA3/MJD1 gene mutation analysis was done in both cases, and a full penetrance CAG repeat expansion was found in both affected patients. Two different outcomes were observed in the offspring, who were both children. The risk and consequences of positive results had been explained in a counseling session to family members, who decided to keep the information until the child would have reached legally adult age.Conclusions: In developing countries such as Indonesia, problems arose due to ethical issues, knowledge of genetic diseases, and inaccessible molecular diagnostics. Culture, religion and tribe diversities may create additional challenges. These cases emphasize the need for careful consideration of presymptomatic testing in children, especially in complicated situations where psychological and ethical issues should be addressed.

Abstract

Background: Peripheral neuropathy can be caused by diabetes mellitus, nutritional deficiencies, entrapment or Carpal Tunnel Syndrome (CTS), and idiopathic.Objective: To determine the therapeutic efficacy of Vitamin B1, B6, and B12 forte in relieving symptoms of peripheral neuropathy.Methods: This was pre- and post-experimental study involving patients with moderate peripheral neuropathy (Toronto Clinical Neuropathy Score [TCNS] ≥6 and Michigan Neuropathy Screening Instrument [MNSI] ≥7) taken from outpatient neurological clinic of Dr. Kariadi Hospital, Semarang. The patient was prescribed Vit B1 100 mg, B6 100 mg, and B12 5000 mcg once daily for 2 months. Evaluation of the numerical pain rating scale in the form of VAS and Total Symptom Score (TSS) was conducted at the first and second month. The VAS score difference test was conducted with the Wilcoxon test and TSS with the Post Hoc test and considered significant if p <0.05.Results: There were 30 patients aged 18 - 65 years, consisted of 70% female and 30% male. The etiology of peripheral neuropathy were idiopathic (40%), CTS (26.7%), DM (23.3%), and HNP (10%). There were significant differences of the VAS scale  and TSS at every evaluations.Conclusion: Administration of VitB1, B6, and B12 forte relieved symptoms of moderate peripheral neuropathy with improvement of VAS and TSS scores
