Abstract

ABSTRACT This research to determine the knowledge and attitudes of health workers regarding reporting and reporting systems of medication errors has been carried out. Medication errors is any event that can cause or resulted  in improper health services or harm to patients that actually can be avoided. This research is a qualitative descriptive study using in-depth interviews with data analysis conducted using thematic analysis. The results showed that the health workers at Monompia General Hospital GMIBM Kotamobagu did not understand the reporting of medication errors, decision making to report medication errors was determined by the severity of the result of errors and lack of reporting due to fear of disciplinary action and fear of losing their jobs. Monompia General Hospital GMIBM Kotamobagu City continues to strive to improve the quality of health services, but the limitations of facilities and infrastructure as well as human resources were the obsracles. Keywords : Knowledge, Medication Errors, Reporting System ABSTRAK Telah dilakukan penelitian untuk mengetahui pengetahuan dan sikap tenaga kesehatan mengenai pelaporan dan sistem pelaporan medication error. Medication error merupakan setiap kejadian yang dapat menyebabkan atau berakibat pada  pelayanan kesehatan yang tidak tepat atau membahayakan pasien yang sebenarnya dapat dihindari. Penelitian ini merupakan penelitian yang bersifat deskriptif kualitatif menggunakan wawancara mendalam dengan analisis data yang dilakukan menggunakan tematik analisis. Hasil penelitian menunjukan bahwa tenaga kesehatan  di RSU Monompia GMIBM Kota Kotamobagu belum memahami mengenai pelaporan medication error, pengambilan keputusan untuk melaporkan kesalahan pengobatan ditentukan oleh tingkat keparahan hasil dari kesalahan dan minimnya tingkat pelaporan disebabkan karena tenaga kesehatan takut tindakan disiplin serta takut kehilangan pekerjaan. RSU Monompia GMIBM Kota Kotamobagu terus berupaya untuk meningkatkan mutu pelayanan kesehatan, tetapi keterbatasan sarana dan prasarana serta sumber daya manusia menjadi kendala tesendiri. Kata kunci : Pengetahuan, Medication error, sistem pelaporan

Abstract

ABSTRACT Pharmaceutical care are activities that aim to identify, prevent and resolve drug related problems (DRPs). In the process of pharmacy services, especially in patients who receive many types and amounts of drugs is the risk of medication errors. The more risk factors you have, the more likely you are to get coronary heart disease, causing the complexity of the therapy given. This study aims to determine the characteristics of patients and the percentage of Drug Related Problems (DRPs) at the administration stage of coronary heart disease patients at the inpatient installation of Prof. RSUP DR. R. D. Kandou Manado for the period December 2017 - April 2018. This research was a descriptive study with prospective data retrieval of 38 inpatient CHD medical record data that met the inclusion criteria. The results of the study based on the characteristics of CHD patients showed that the number of male patients was higher than that of females as many as 29 patients (78.38%) and the highest number of patients in the elderly group (60-69) were 15 patients (39.47%). (5-10) types of drugs prescribed in 22 patients (57,90%) and the highest class of drugs in 38 patients (100%) cardiovascular therapy classes and circulatory drug. Percentage of DRPs based on the indication without therapy (10.34%), therapy without indication (10.34%), low drug dose (79.31%), high drug dose (0%), non-compliance (0 %). Keywords: CHD, Drug Related Problems (DRPs), Administration ABSTRAK Pelayanan kefarmasian merupakan kegiatan yang bertujuan untuk mengidentifikasi, mencegah dan menyelesaikan masalah terkait obat/Drug Related Problems (DRPs). Dalam proses pelayanan kefarmasian khususnya pada pasien yang menerima banyaknya jenis dan jumlah obat merupakan resiko terjadinya kesalahan pengobatan (medication errors). Semakin banyak faktor resiko yang dimiliki, semakin berlipat pula kemungkinan terkena penyakit jantung koroner menyebabkan kompleksnya terapi yang diberikan. Penelitian ini bertujuan untuk mengetahui karakteristik pasien dan presentase Drug Related Problems (DRPs) pada tahap administration pasien penyakit jantung koroner di instalasi rawat inap RSUP Prof. DR. R. D. Kandou Manado periode Desember 2017 - April 2018. Penelitian ini merupakan penelitian deskriptif dengan pengambilan data secara prospektif terhadap 38 data rekam medik pasien PJK rawat inap yang memenuhi kriteria inklusi. Hasil penelitian berdasarkan karakteristik pasien PJK menunjukkan jumlah pasien laki-laki lebih banyak dibandingkan perempuan yaitu sebanyak 29 pasien (78,38%) dan jumlah pasien terbanyak pada kelompok usia lansia (60 – 69) sebanyak 15 pasien (39,47%). Hasil penelitian terkait menunjukkan bahwa mayoritas pasien PJK menerima menerima (5 – 10) jenis obat yaitu sebesar 22 pasien (57,90%) dan golongan obat yang  paling banyak digunakan oleh pasien PJK yaitu golongan obat dengan kelas terapi kardiovaskular dan golongan obat yang mempengaruhi darah sebanyak 38 pasien (100%). Presentase DRPs indikasi tanpa terapi sebesar (10,34%), terapi tanpa indikasi sebesar (10,34%), dosis obat rendah sebesar (79,31%), dosis obat tinggi sebesar (0%), ketidakpatuhan (0%). Kata kunci : PJK, Drug Related Problems (DRPs), Administration

Abstract

Prescription can reduce the administrative / prevent dangerous medication errors, withadverse effects on patients by the drugs used during the management of health care. This studyis aimed to evaluate the administrative prescriptions by examining the possibility of medicationerrors by a Pediatricians at three pharmacies in the city of Manado. This is a descriptive studyconducted a retrospective review of patient prescriptions. The method of sampling took randomreceipts using the Slovin formula. 12,225 receipts was vised in the three pharmacy and found338 sheets sample of prescription from period of January to December 2011 then processed andevaluated administrative completeness. Administrative completeness prescription written by apediatrician at the Manado City Permit Practice (11.37%), Address patients (53.7%), Gender(0%), weight loss (27.5%), Age (78 , 3%), date prescription (98.4%) and to mark the R /, DoctorName, Address Doctor, Initials Doctor, Patient Name, Drug Name, Dosage, Number of Drugs,How to use a clear (100%). From these data we can conclude the potential occurrence ofmedication errors and can not find any recipes that meet the screening criteria for theprescription paperwork based on the Decree of the Minister of Healthno.:1027/MENKES/SK/1X/2004.Keywords: Administrative Complete Receipts, Receipts Pediatrician, Medication Error
