Abstract

Latar Belakang. Penemuan kasus dan pengobatan dini hipotiroid pada anak batita sangat penting karena keterlambatan hal tersebut menyebabkan kelainan intelektual dan atau fungsi neurologis yang menetap. Penegakan diagnosis hipotiroid lebih akurat dengan pemeriksaan laboratorium, tetapi akan mahal dan kurang praktis jika di lakukan di lapangan. Instrumen skrining hipotiroid dapat digunakan sebagai alternatif untuk mendeteksi hipotiroid pada anak batita. Instrumen seharusnya mudah, murah, dan memberikan hasil diagnostik yang dapat diandalkan. Tujuan. Tujuan penelitian untuk mendapatkan sensitifitas dan spesifisitas instrumen skrining hipotiroid. Metode. Penelitian observasional dengan desain uji diagnostik ini dilakukan di kabupaten Magelang, Wonosobo, Temanggung, Purworejo, Situbondo dan Jember, selama 10 bulan. Variabel yang dianalisis adalah 26 gejala hipotiroid pada anak batita. Baku emas berdasarkan pemeriksaan laboratorium kadar TSH dan fT4. Analisis untuk mendapatkan nilai sensitifitas dan spesitifitas dilakukan tabulasi silang dengan tingkat kemaknaan 5%. Hasil. Nilai sensitivitas dan spesifisitas instrumen skrining hipotiroid (ISH) dengan cut off >5 dibandingkan dengan baku emas rendah (Se 26,3, Sp 90,3; area under curve (AUC) 58,3%). Kekuatan hubungan antara skor ISH dengan cut off >5 dibandingkan baku emas adalah OR 3,329 (1,621-6,835; 95% CI). Uji multivariat mengoreksi probabilitas anak batita yang mempunyai skor ISH >5 untuk menjadi hipotiroid 2,253 (1,011-5,022 95% CI) dan terjadi peningkatan area under the curve (AUC) menjadi 70%. Kesimpulan. Instrumen skrining hipotiroid pada anak batita kurang sensitif dan kurang spesifik untuk mendiagnosis hipotiroid.

Abstract

ABSTRAK Perlindungan wanita hamil dari kekurangan iodium merupakan salah satu kebijakan perbaikan gizi di Indonesia. Kekurangan iodium pada wanita hamil selain berdampak buruk pada ibu juga mempengaruhi pertumbuhan dan perkembangan janin terutama organ otak. Di Indonesia saat ini, sumber utama asupan iodium berasal dari konsumsi ikan laut dan garam konsumsi beriodium. Akan tetapi tidak semua wanita hamil dapat dengan mudah mendapatkan iodium yang cukup selama kehamilannya. Data proporsi penduduk Indonesia yang mengonsumsi ikan laut sebesar 42,6% dengan rerata konsumsi ikan laut per orang per hari sebesar 25,5 gram. Garam beriodium sesuai SNI mensyaratkan mengandung 30 ppm iodium (KIO3). Dengan perkiraan konsumsi garam beriodium 10 gram per orang per hari, wanita hamil hanya mendapatkan 178 µg iodium dari kebutuhan yang dianjurkan sebesar 250 µg per hari. Bukti empiris menunjukkan bahwa rata – rata konsumsi garam di Indonesia sekitar 5 – 8 gram per orang per hari sehingga asupan iodium harian wanita hamil semakin jauh berkurang dari kebutuhan. Keadaan ini diperberat dengan cakupan rumah tangga mengonsumsi garam beriodium cukup mengandung iodium selama lebih dari 3 dekade hanya berkisar 60% sampai <80%. Hasil Riskesdas 2013 menunjukkan proporsi rumah tangga mengonsumsi garam beriodium cukup berdasarkan hasil tes cepat sebesar 77,1% (target >90%). Sedangkan indikator kecukupan asupan iodium menunjukkan bahwa median ekskresi iodium urin (EIU) wanita hamil di perkotaan dan perdesaan di Indonesia 163 µg/L (adekuat 150 – 249 µg/L), mendekati batas batas bawah (marjinal). Oleh karena itu, suplementasi iodium pada wanita hamil sebagai alternatif sementara untuk memenuhi kebutuhan iodium wanita hamil perlu dipertimbangkan untuk menggunakan dosis harian 150 µg per hari.

Abstract

Latar Belakang. Anemia karena kekurangan zat besi masih menjadi masalah gizi utama di Indonesia. Kondisi ini dapat menurunkan produktifitas dan gangguan pertumbuhan pada anak usia sekolah. Zat besi merupakan bagian penting dari thyroperoxidase (TPO) yang berperan dalam sintesis hormon tiroid. Tujuan. Mengukur hubungan antara kadar hemoglobin dengan kadar hormon tiroid bebas (free Thyroxine / fT4) pada anak sekolah dasar. Metode. Penelitian cross-sectional dilakukan di daerah perdesaan pegunungan Kabupaten Wonosobo yang mempunyai riwayat daerah endemis Gangguan Akibat Kekurangan Iodium. Sebanyak 141 anak usia sekolah dasar diukur status gizi, kadar hemoglobin dan free Thyroxine (fT4). Besar sampel dihitung berdasarkan Pearson product-moment correlation coefficient. Data dianalisis menggunakan uji statistik korelasi Pearson. Hasil. Sebanyak 47,5% partisipan tergolong pendek, 24,1% menderita anemia, rata- rata kadar hemoglobin dan hormon tiroid bebas berada pada kisaran normal 12,6 g/dL dan 1,5 ng/dL secara berturutan. Terdapat hubungan positif antara kadar hemoglobin dengan kadar hormon tiroid bebas dengan rho sebesar 0,24 (p < 0,05). Kesimpulan. Kadar hemoglobin berhubungan dengan kadar hormon tiroksin bebas (fT4).

Abstract

Women are more at risk to stress in pregnancy than during the puerperium. One method of coping with stress is through physical exercise. Maryam Exercise can be used as an alternative to the development of pregnancy exercises in Indonesia because it covers the holistic aspects and easy to do. It is a combination of basic pregnancy exercises, Islamic prayer (Salat) movements and dhikr.  The purpose of this study was to examine the impact of Maryam exercise on the stress levels and cortisol serum levels among primiparous pregnant women. This study used a quasi-experimental study design, pre-test and post-test with non-equivalent control group. In the third trimester of 40 primiparous women  were randomly assigned into two groups such the experimental group (n=20) and the control group (n=20). The experimental group received the Maryam exercise while the control group received the standard antenatal exercise. There was a significant difference in stress levels between the intervention group and the control group before and after receiving the intervention (p <0.05). The cortisol levels also showed the significance difference between the intervention group and the control group before and after receiving the intervention (p <0.05). The findings of this study suggest that Maryam Exercise has a positive impact on stress levels and cortisol serum levels of primiparous pregnant women.

Abstract

Background: Thick Slab is a technique used in the MRCP examination to acquire the image of the biliary system by using oblique slices in taking some parts of the image at different angles. Image quality with a thick slab technique is considered better than the thin slice since the image visualization of the bile ducts system in various parts appear more clearly. Aims: The purpose of this study is to determine the effect of variation in the number of thick slab on the scan time and image information on the MRCP and determine the most informative image with the most effective scan time as possible. Methods: This study type was an experimental study conducted in St. Elisabeth hospital Semarang. The data were in the form of 90 MRI images of biliary tract of 3 patients with 5 variations of Thick Slab (6, 12, 18, 24, 30). The image assessment was taken by 5 respondents regarding the information of the objects of Right Hepatic Duct, Left Hepatic Duct, Common Hepatic Duct, Pancreatic Duct, Cystic Duct, Common Bile Duct and Gallbladder. Data analysis was conducted by regression test and cross tabulation. Results: The study results showed that there was an effect of varying the amount of thick slab on the scan time. The effect of varying the amount of thick slab on the image information can be seen from the statistical test that there was no effect, however descriptively there was a different on the specific per object. The image of thick slab variation of 12 was an image that had the highest value of information with the mean value of 1,988, the total value of 13.936 and a scan time of 0.46 S so that the most informative image with the most effective scan time was on the thick slab number variation of 12. Conclusion: There was an effect of variation in the number of thick slab on the scan time. More variation of the number of thick slab was followed by an increase in the value of the scan time. But there was no effect of variation in the number of thick slab on the image information, however descriptively there was a different on the specific per object which showed that the highest score for the Right Hepatic Duct, Common Hepatic Duct were in the thick slab of 12 and 30, respectively

Abstract

Background: In the process of work to gain the maximum results, a radiologist needs a viewing box tool to read radiographs. Aims: to create a viewing box tool by using a potentiometer system. Methods: This study used applied research method by creating and using the design of viewing box tool by using a potentiometer system and testing the viewing box tool created by using a Lux meter and 15 respondents consisting of five radiologists and 10 radiographers who should fulfill the questionnaire form. Results: The mean of viewing box illumination reached 220 lux. The results of the questionnaire showed that 100% radiologist gave an A (excellent) and expressed that the viewing box tool created could be used properly and 90% radiographers provided an A (excellent) and expressed that the viewing box tool created could be used properly, while 10% radiographer gave a value of B (moderate). Conclusion: viewing box tool created could be used properly and obtained optimal results as a tool in reading radiographs. Potentiometer system contained in the viewing box was very helpful in reading radiographs because it allowed to adjust the light intensity according to user needs.

Abstract

Background: The main idea of designing an assisting tool for immobilization in pediatric thorax and abdomen examinations is that there are radiographer’ s difficulties while performing radiographs, especially pediatric thorax and abdomen. Aims: This study aims to design of appliance assist for immobilization thorax and abdomen examination on pediatric patients. Methods: This study was an exploratory experiment by designing, applying and testing the construction of an assisting tool for immobilization in thorax and abdomen examination on pediatric patients. The methods used in data collection were interview and observation. Function test results were analyzed based on the check list of the respondents regarding the feasibility of the tool. Results: an assisting tool for immobilization in thorax and abdomen examination on pediatric patients was designed in examination table-shaped made of basic materials such as acrylic, hollow steel and Eser steel plate. Function test on the work of the assisting tool was performed by 6 respondents who applied it to the patient. Based on the function test results, there was of 89,5% respondents who stated that the tool for immobilization in thorax and abdomen examination on pediatric patients could help the performance of the radiographer in positioning and reducing the patient’s movement and there was no artifact in the radiographs. Conclusion: Function test results showed that the tool design could reduce the movement of patients and may replace the function of others to resist the movement of the patient. An assisting tool for immobilization in thorax and abdomen examination on pediatric patients should be improved further especially in choosing the material used, so that the toll will be lighter and can better reduce the patient’s mobility and the size should consider the average height of the pediatric patients.

Abstract

Fatsups and BLADE sequences are used to reduce artifacts and clarify anatomical images. Basedon theoretical studies, the STIR, BLADE sequences, the addition or subtraction of parameters, and theaddition of artificial intelligence used still have a weakness, namely increasing the scanning time to belonger. Another technique that can be used and sequences and parameters on MRI is the denoisingKalman filter technique in the Matlab (Matrix Laboratory) program. The denoising technique is appliedafter the scanning process. Denoising will not increase the MRI scanning time. This systematic reviewaims to know the technical application of denoising Kalman filter for reduction artifacts on MRIexamination. The search was done using google scholar, WILEY, IEE Explore, SPRINGER,PERPUSNAS, and Scopus in English with 2004-2020 articles period. The keywords are MRI artifact,reducing artifacts, and the Kalman filter algorithm. A review of 4 articles of filter Kalman intervention onMRI Brain, MRI Abdomen, and MR Cardiac shows that the Kalman filter is good enough to reduceartifacts and improve anatomical information. The Kallman filter could reduce flow artifacts, improveimage quality and clarify anatomical images on MRI.
