Abstract

Heart failure is a major cause of cardiovascular morbidity and mortality worldwide despite advances in prevention and management. Recent clinical trial findings have shown that sodium-glucose co-transporter 2 inhibitor (SGLT2i) produces effects other than lowering blood glucose levels, i.e., exhibits beneficial cardiovascular effects. The purpose of this paper is to determine the role of SGLT2i in heart failure patients with low ejection fraction (HFrEF) so that it can be used as a guide for future management. This study is a narrative review by searching on Google Scholar through several keywords. The period including articles obtained is articles from 2014 to 2022. The diuretic effect of SGLT2i can cause an increase in cardiac preload. The reduction in cardiac afterload likely occurs through a decrease in blood pressure and arterial stiffness resulting in increased endocardial blood flow. SGLT2i has a beneficial effect on the heart remodeling process. SGLT2i can prevent heart failure by increasing the production of ATP from the oxidation of ketone bodiesSGLT2i in heart failure patients can reduce blood pressure, maintain a decrease in eGFR and reduce the risk of acute kidney injury (AKI) in patients. SGLT2i is very effective in the treatment of HFrEF patients. Because of these advantages, further research is needed in Indonesia in order to determine the effectiveness and impact of giving SGLT2i to HFrEF patients in Indonesia.

Abstract

Latar belakang: Gagal jantung merupakan beban penyakit yang tidak kunjung berkurang dari dulu hingga sekarang. Penuaan dan pertumbuhan populasi menyumbang dalam peningkatan jumlah kasus gagal jantung. Isi: Selama proses remodeling jantung pada penyakit gagal jantung, terjadi perubahan metabolism jantung dengan peningkatan glikolisis yang signifikan disertai penurunan oksidasi asam lemak. Adenosin trisofat (ATP) yang dihasilkan dari glikolisis saja menyumbang hanya sedikit dari total ATP yang dikonsumsi dalam jantung orang dewasa normal. Mikronutrien berperan penting dalam perubahan transpor elektron di mitokondria. Mikronutrien (termasuk koenzim Q10, zinc, tembaga, selenium, dan besi) diperlukan untuk mengubah makronutrien menjadi ATP secara efisien. Kesimpulan: Defisiensi mikronutrien tertentu dapat memperburuk kondisi gagal jantung, dan sebaliknya, defisiensi mikronutrien mungkin dapat dijadikan sebagai target terapi yang baru, meskipun masih dibutuhkan banyak penelitian lebih lanjut perihal tersebut.

Abstract

Sepertiga dari semua pasien dengan gagal jantung mengalami anemia, dan kehadirannya dikaitkan dengan keparahan gejala, peningkatan angka rawat inap, dan peningkatan angka kematian. Etiologi anemia bersifat multifaktorial, kompleks, dan bervariasi antar pasien. Penyebab utama terjadinya anemia pada gagal jantung adalah faktor inflamasi. Kadar hemoglobin berkorelasi negatif dengan penanda inflamasi seperti tumor necrosis factor-alpha, interleukin-6, dan protein C-reaktif, yang semuanya meningkat pada gagal jantung. Pengobatan anemia pada pasien gagal jantung dengan agen pemicu eritropoiesis telah dievaluasi secara intensif selama beberapa tahun terakhir. Sayangnya, agen ini tidak meningkatkan hasil dan dikaitkan dengan risiko efek samping tromboemboli yang lebih tinggi. Oleh karena itu, penggunaan ESA (Eritropoiesis-Stimulating Agents) untuk pengobatan anemia pada gagal jantung tidak dapat direkomendasikan. Terapi lain sedang dipelajari dan termasuk agen yang menargetkan reseptor eritropoietin, jalur hepsidin, atau penambah zat besi. Artikel ini berfokus pada patofisiologi anemia pada gagal jantung, evaluasi efektivitas modalitas terapi yang tersedia, dan terapi yang sedang diteliti yang dapat direkomendasikan sebagai pengobatan anemia pada gagal jantung

Abstract

The high prevalence of heart disease makes dentists often meet patients with this condition. Cardiac disorders commonly encountered in dental practice include hypertension, ischemic heart disease, cardiac arrhythmias, infective endocarditis, and arrhythmias. These patients required special consideration and adequate understanding of the cardiovascular conditions necessary to provide safe and effective dental care. Based on the heart condition, proper modification in dental care is very important. This article will discuss common heart conditions, how these diseases can affect dental care and prophylactic management and treatments that can be done to provide safe and optimal dental care.

Abstract

Cardiorenal syndrome is a disorder that involves the heart and kidneys, acute or chronic dysfunction of one organ can induce acute or chronic dysfunction of other organs. Renal dysfunction associated with acute heart failure, and vice versa, causes considerable morbidity and mortality. Until recently, there has been no consistent and effective strategy in the management of patients with cardiorenal syndrome. Generally, only done by reducing the volume of fluid so as not to worsen kidney function. Early identification leads to more effective treatment and shorter hospitalization. A multidisciplinary approach involving cardiologists and renal specialists is essential.

Abstract

Exercise training is an essential additional non-pharmacological method to treat heart failure patients that has been shown to have significant impacts on mortality, morbidity, exercise capacity and quality of life. Various type of training are developed to resolve the needs of heart failure patients, including those who want to participate in sport activities. It is critical and challenging to prescribe personalized exercise training program perfectly, therefore, this review provides brief overview of the advantages and recommendations to prescribe individualized type of exercise training including aerobic, resistance, respiratory, aquatic and competitive sport activities for patients with heart failure. A comprehensive electronic search was conducted using PubMed, Google Scholar, and ScienceDirect. The search was limited to English-language publications from 2004 to 2021. Hand searching of relevant journals and reference lists was also performed. Exercise training should be component of a comprehensive heart failure care approach. Each patient with heart failure may have a different endurance to tolerate the same relative training. Abstract

Abstract Mesenteric ischemia is a rare etiology of abdominal pain caused by impaired blood flow to the mesenteric circulation. This condition has acute and chronic variations and is associated with high mortality and morbidity. Early diagnosis is needed to determine management and prevention efforts towards a worse prognosis. The main principles of management of mesenteric ischemia are the restoration of bowel perfusion and resection of the necrotic bowel. This review will discuss the basic materials, diagnosis, and patient management strategies based on the conditions and causes of mesenteric ischemia.
