Abstract

Corneal perforation may be associated with prolapse of ocular tissue. We present a rare case reportan11 months-old female presented prolapsed of right eye. She was earlier diagnosedwithright corneal ulcer anddescemetocele. Physicalexamination revealed perforatedcornea.On ultrasound examination showedvitreous hazzinessand membranous opacity assumed due to inflammation. Based on examinationswe suspectedmalignancy process and performed enucleation withautogenous Dermis Fat Graft (DFG) which harvested onthigh region.Histopathology examination revealed chronic inflammation process.Hence, determining the causative factors of the prolapsed eye is the key for better management and prognosis.

Abstract

Introduction: Adnexa foreign body is one of the classifications of extraocular foreign body. Visual prognosis depends on the zone of injury, type, and size of the foreign body and subsequent complications. Case Presentation: A 5-year-old girl came to the outpatient clinic with her parent with chief complaints of swelling and pain in the lower eyelid in the left eye after she had a motorcycle accident with her parent (single accident) five months ago. At the time, the patient was hit by a piece of mica from a broken helmet in her left eye, and the patient complained that her left eye was red swollen and that she had difficulty opening her eyes and; when the midwife then referred her to an ophthalmologist in Jember and referred to Soetomo with diagnosis conjunctival tumor in LE. Visual acuity RLE 5/5, and in the conjunctiva, there was hyperemia and foreign body like glass or mica in fornix inferior. In the CT scan, there was foreign body or corpus alienum with size 1.05 x 0.5 x 2.1 cm in cutan subcutan left inferior eyelid until ciliary body and swelling in tissue around corpus alienum. The patient was the plan to extract the foreign body. Conclusion: Adnexa foreign body is one emergency case in ophthalmology. The extraction technique depends on the object’s composition and size because it impacts the step of the operation technique.

Abstract

Introduction : Facial nerve palsy is a condition that affects various organ including the eyes. We report a case of facial nerve palsy with eye manifestations in one patient that requires immediate action to save the cornea. Case Illustration : A 63-year-old male presented to our OPC with the primary complaint of right eyelid inability to close since September 2021after brainstem tumor excision, which had worsened since May 2022. Visual acuity: 2 meters counting fingers; IOP: 11 mmHg. Anterior segment examination revealed lagophthalmos (8 mm) in the right eye, a pinch test result of 10mm (horizontal laxity), conjunctival hyperemia, conjunctivalization of the cornea, a fluorescein test result of 7 x 4 mm, and band keratopathy. The wrinkles on the patient's right face disappeared. There was numbness, and the smile was asymmetrical. Patient underwent right eye modified lateral tarsal strip and recesses levator 8mm. The patient's eyelids can close tightly almost symmetrically with the left eye and no ptosis after operations. One week after operation, patient feels more comfortable, and the conditionof the cornea has improved. The facial nerve is vulnerable to trauma because of its path from the brainstem through the temporal bone to the facial muscles. In conditions of irreversible facial nerve palsy, managements are focus to overcome the complications. Modified lateral tarsal strips and levator recesses are relatively simple and inexpensive techniques that can help to overcome lagophthalmos. Discussion : Conclusion : Ocular manifestations can occur in patients with facial nerve paresis. Modified lateral tarsal strips and recesses levator eyelids can help protect the cornea.

Abstract

Introduction : Kissing nevus is a rare form of congenital nevus that occurs on upper and lower eyelid of one eye, forming a large nevus when the eyelids are closed. This condition causes functional and aesthetic problems thus the reconstruction is essential. Case Illustration : A 23-years old female presented to our ophthalmology outpatient clinic with complain of having painless, black colored, non-progressive nevus over left upper and lower eyelids since birth that cosmetically unpleasing. The dark brown-black colored was sized 55x30 mm with hypertrichosis covering approximately two third of both left upper and lower eyelid margins and extending to cheek. The entire nevus was excised and reconstructed by supraclavicular full thickness skin graft, lateral canthopexy and canthoplasty. Three weeks after operation showed satisfactory results. Histological examination revealed intradermal nevus pigmentosus. Discussion : The reconstruction of eyelid after excision remains a challenge because of its specialized function and cosmetic significance. Ideally, the reconstruction should provide a stable eyelid margin, protect the ocular surface, and sustain mobility. Free skin grafts harvested from the control lateral eyelid, postauricular, supraclavicular, and upper arm region have been the most common reconstructive approach. The supraclavicular area is a reliable donor option to cover large head and neck defects due to its similarity in colour, thickness, and flexibility. Conclusion : Free supraclavicular graft is safe, reliable and cosmetically acceptable choice for reconstruction head and neck defects.

Abstract

Introduction: Ankyloblepharon filiforme adnatum (AFA) is a rare congenital condition marked by one or more bands of tissue adhering partially or entirely to the ciliary margins of the upper and lower eyelids at the gray line. This case study illustrates the ease of treating the condition. A general ophthalmologist could do that. Case Presentation: A one-month-old infant was admitted to the outpatient clinic. Since birth, he could not open his eyes due to upper and lower eyelid adhesions. There was also a coloboma in the one-third lateral upper eyelid in the right eye. The evaluation of other body systems was normal. The infant was diagnosed with type I AFA and underwent surgery under general anesthesia. After crushing the band with the clamp, the bands of tissue were excised with the blade. The coloboma was reconstructed by applying primary closure and pentagonal shape incision, then sutured with 8-0 vicryl. The eyelids were separated with an aligned lid margin. Conclusions: Simple surgical procedures are used to treat congenital ankyloblepharon. It depends on the nature and extent of the adhesions along the lid margin. To reduce the risk of amblyopia, early treatment should be initiated immediately.

Abstract

Introduction: An unusual type of congenital nevus known as "kissing nevus" develops on one eye's upper and lower lid and becomes enormous while the eyelids are closed. Reconstruction is complex since it creates both aesthetic and functional issues. Case Presentation: A 23-year-old female patient attended our ophthalmology outpatient clinic stating that she had a painless, non-progressing black nevus covering her left upper and lower eyelid since birth. The dark brown-black, 55 mm in size, with hypertrichosis that extended to the cheek and covered about two-thirds of the left upper and lower eyelid edges. A free supraclavicular skin transplant restored the complete nevus shortly after removal. We performed canthoplasty, lateral canthopexy, and full-thickness graft procedures. Three weeks following the procedures, the result was satisfactorily proved. Through histopathological examination, intradermal nevus pigmentosus was identified. Conclusions: A secure, trustworthy, and aesthetically pleasing option for head and neck deformities reconstruction involves a free supraclavicular graft.

Abstract

Introduction: Eyelid reconstruction in cicatricial ectropion caused by facial burns represents a surgical challange. Surgical management consists in correcting the lid defect often associated with a skin graft and lid tightening. Case Description: a 18-years-old man was referred because suffered cicatricial ectropion on superior-inferior eyelid of right eye after got burn injury 4 months before come to our outclinic, with cicatricial tissue on 50% on right face after facial burns. Ocular examination revealed a visual acuity 20/20 on both eyes, lagophthalmos but a good Bell’s phenomenon, and diffused ectropion on the eyelid of right eye. Reconstruction was made a release contracture tissue, lid tightening and full thickness skin grafts from donor site which recommended from supraclavicular 60 x 30 mm for superior eyelid and 50 x 40 mm for inferior eyelid, place the graft and fix it with a bolster and Frozt suture. We follow up the patient, 1 day, 1 week, and 6 months post operation. Discussion: Major issues when addresing cicatricial ectropion followed by severe burns are : right donor site selection, slightly oversized graft allowed for some contraction and full eyelid closure. For further plan is consultation to plastic surgery to manage other cicatricial tissue of part of body.   Conclusion: Reconstruction for diffused ectropion by using full thickness skin graft and lid tightening are better option for correction of cicatricial ectropion.
