Abstract

Abstract: This study aimed to determine the relationship between visual acuity and degree of Non Proliverative Diabetic Retinopathy (NPDR) in patients with type 2 diabetes mellitus (T2DM). This study was conducted in Retina Subdivision Ophthalmology Department Prof. Dr. R. D. Kandou Hospital, Manado, North Sulawesi, Indonesia. Samples were 354 eyes. Visual acuity examination was performed on all patients diagnosed with NPDR by using a Snellen chart at 6 meters distance. NPDR degree was graded according to the International Clinical Diabetic Retinopathy Disease Severity Scale of the American Academy of Ophthalmology. Correlation analysis between visual acuity and the NPDR degree was done by using Kruskal Wallis test which showed a P value of 0.185 (> 0.05). Conclusion: Visual acuity had no significant relationship to the degree of NPDR.Keywords: visual acuity, NPDR, T2DMAbstrak: Penelitian ini bertujuan untuk mengetahui hubungan antara tajam penglihatan dengan derajat Non Proliverative Diabetic Retinopathy (NPDR) pada penyandang diabetes melitus tipe 2 (DMT2). Penelitian ini dilakukan di Poliklinik Mata Subdivisi Retina RSUP Prof. Dr. R. D. Kandou Manado, Provinsi Sulawesi Utara, Indonesia. Sampel penelitian berjumlah 354 mata. Pemeriksaan visus dilakukan pada semua pasien yang terdiagnosis NPDR dengan menggunakan Snellen chart pada jarak 6 meter. Penilaian derajat NPDR berdasarkan International Clinical Diabetic Retinopathy Disease Severity Scale dari American Academy of Ophthalmology. Analisis hubungan visus dengan derajat NPDR dilakukan dengan uji statistik Kruskal Wallis yang menunjukkan nilai P = 0,185 (>0,05). Simpulan: Tidak terdapat hubungan bermakna antara tajam penglihatan (visus) dan derajat NPDR.Kata kunci: visus, NPDR, DMT2

Abstract

Abstract: Macular hole (MH) up to now has an unknown exact etiology or mechanism behind its formation. It can be formed post cataract surgery with phacoemulsification technique with a relatively small probability. We reported a male of 74 years old who came with diminished visual acuity of his left eye three months after cataract surgery with phacoemulsification procedure. In ophthalmologic examination we found the visual acuity of the right eye was 6/60 and of the left eye was 2/60. Fundoscopy of the left eye revealed a depression in fovea region with estimated size of 500 μm. Optical coherence tomography (OCT) examination resulted in intraretinal fluid with multiple cyst appearance in retinal layers associated with retinal thickening. Based on anamnesis, physical examination, and imaging examination, the diagnosis of this patient was stage 3 macular hole with vitreomacular traction. Management of this patient was pars plana vitrectomy (PPV). One week after PPV, the patient gained a remarkable visual acuity improvement (6/12), albeit, there was still slight metamorphosia.Keywords: macular hole, cataract surgery, phacoemulsification Abstrak: Macular hole (MH) hingga saat ini masih tidak diketahui penyebab pastinya ataupun mekanisme dasar terjadinya MH. Penyakit ini juga dapat terbentuk pasca operasi katarak dengan teknik fakoemulsifikasi meskipun dengan probabilitas yang relatif kecil. Kami melaporkan sebuah kasus MH pada seorang laki-laki berusia 74 tahun dengan keluhan pandangan kabur pada mata kiri 3 bulan setelah operasi katarak (teknik fakoemulsifikasi). Pada pemeriksaan oftalmologi didapatkan tajam penglihatan mata kanan 6/60 dan mata kiri 2/60. Pada pemeriksaan lanjut mata kiri menggunakan funduskopi ditemukan cekungan pada bagian fovea dengan ukuran sekitar 500 μm. Pemeriksaan dengan OCT mendapatkan intra-retinal fluid dengan gambaran kista multipel pada lapisan retina disertai penebalan retina. Berdasarkan anamnesis, pemeriksaan fisik, dan imaging, diagnosis pada pasien ini ditegakkan sebagai macular hole stadium 3 dengan vitreomacular traction pada mata kiri. Tata-laksanayang dilakukan ialah vitrektomi pars plana (VPP). Satu minggu setelah VPP, tajam penglihatan pasien membaik secara drastis menjadi 6/12 walaupun masih terdapat metamorphopsia ringan.Kata kunci: macular hole, operasi katarak, fakoemulsifikasi

Abstract

Background: Most patients who use non steroid anti-inflammatory drug (NSAID) in long-term or high-dose will experience NSAID gastropathy that appears as dyspepsia complaints. The aim of this study is to assessthe efficacy of nabumeton and incidence of dyspepsia as side effect of treatment using nabumeton compared to meloxicam in patients with pain due to osteoarthritis. Methods: Sixty patients with pain due to osteoarthritis at internal medicine clinic in Koja Hospital were included in this study and were given meloxicam or nabumeton within February 29th  to March 25 in double-blind manner. Primary disease pain degree and heartburn pain degree were evaluated three days after administration of the drug and a week was added if necessary. The statistical analysis used were student t-test to compare changes in pain scores between the 2 groups, and Chi-square test to assess differences in the number of dyspepsia in both groups.Results: Pain scale reduction in meloxicam group was 0.57 ± 0.67 points, which is less than 1.27 ± 0.74 points in nabumeton group. In addition, the average recurrence of heartburn in the meloxicam was 18 out of 30 (60%) patients in total, while the average recurrence of heartburn in nabumeton was 2 out of 30 (60%) patients.Conclusion: The use of nabumeton pain medication is better in terms of efficacy and side effects of heartburn compared to meloxicam.Keywords: osteoarthritis, joint pain, meloxicam, nabumeton

Abstract

Background: Macular thickening that occurs in diabetic retinopathy has been known as one of the primary causes of vision loss functions in patients with diabetic retinopathy. In order to prevent further complications due to macular thickening, Optical Coherence Tomography (OCT) has been used as a diagnostic tool in diabetic retinopathy. This study aims to evaluate the importance of OCT in promoting early detection in retinal thickening as diagnostic tools and proper exams to diminish the chance of visual acuity related to diabetic retinopathy. Method: A case-control observational study was conducted among 22 subjects with Diabetes Mellitus Type-2 (DM-2) from the medical record of the Ophthalmology-Vitreoretinal Division at Prof. dr. R.D. Kandou State General Hospital, Manado from March-December 2018 period. They were divided into 11 patients with mild and moderate non–proliferative diabetic retinopathy (NPDR) and 11 non-diabetic patients’ group. OCT was used to evaluate the macular thickness in both groups. Data were analyzed using SPSS version 20 for Windows. Result: Most of subjects were female (59.1%), age 45-50 years (31.8%), ≥ 10 years duration of DM-2 (63.6%), refractive errors of 0.00-(-3.00) and 0.00-(+3.00) (33.3%), and HbA1c levels ≥6.5 % (54.5%). There was a statistically significant difference of the central macular thickness between NPDR (257.32±76.94 µm) and non-NPDR patients (213.14±19.23 µm) (p=0.006) by using OCT. Conclusion: Optical Coherence Tomography (OCT) could be used to compare the significant differences of macular thickness between diabetic patients without proliferative diabetic retinopathy at Prof. Dr. R D Kandou General Hospital, Manado, Indonesia. Abstract

Sub-internal limiting membrane (sub-ILM) hemorrhage, a rare ocular condition, poses a challenge in ophthalmology due to its association with severe visual impairment. This case report presents a 27-year-old male with a sub-ILM hemorrhage following intense physical activity. Despite unsuccessful Nd:YAG laser treatments, vitrectomy with internal limiting membrane (ILM) peeling resulted in a remarkable improvement in visual acuity from hand movement to 6/6 within the first day post-surgery. The fundus photograph and OCT findings illustrated restored macular anatomy, highlighting the positive outcomes of surgical intervention.

Abstract

Introduction: This retrospective study evaluates the impact of triamcinolone-assisted vitrectomy on intraocular pressure (IOP) in patients undergoing vitreoretinal surgery, with particular focus on comparing outcomes in patients with and without lens surgery. Methods: Medical records of patients treated at R.D. Kandou Hospital from May 2019 to May 2024 were reviewed, and pre- and postoperative IOP values were analyzed. A total of 35 patients met the inclusion criteria. Results: The results show a significant reduction in IOP at all postoperative intervals, with mean differences of 11.12 mmHg at 1 day, 8.60 mmHg at 1 week, 13.23 mmHg at 1 month, and 16.91 mmHg at 3 months postoperatively. Analysis revealed no significant difference in IOP outcomes between phakic and pseudophakic patients. Conclusion: These findings indicate that triamcinolone-assisted vitrectomy effectively lowers IOP, regardless of lens status.

Abstract

Introduction : To report a stargardt disease case Case Illustration : A 14 year young boy presented with chief complaints of blurry vision in both eyes since childhood. Progressive loss in his ability to differentiate between faces, colors and point out details of objects. On ocular examination, his visual acuity was 1/60 with retinoscopy value revealed +2.00 Dsph and was unable to read plates of Ishihara chart in both eyes. On indirect ophthalmoscopy optic disc were normal with sharp margins and orange color and absent foveal reflex in both eyes. Numerous small whitish flecks were seen around the macula in both the eyes with an ill-defined circular lesion was seen at the macula with beaten bronze appearance. Supportive therapy such as correction with low vision aids or the provision of sun protective glasses can help in the daily lives of patients. Discussion : Stargardt disease is an inherited macular degeneration that typically presents in the first two decades of life. The prevalence is in the region of 1 in 8–10,000 individuals. Study reported an annual incidence of 0.127 per 100,000. This case was also rarely reported in Indonesia. The age of onset and rate of progression vary greatly, with most experiencing symptoms in their teens or earlier. Conclusion : Stargardt disease case has been reported in a 14 year young boy. Progressive vision loss can be the only vision complaint from the patient. Supportive therapy such as correction with low vision aids or the provision of sun protective glasses can help in the daily lives of patients.
