Abstract

Abstract

Background.Obesity is one of Cardiovascular risk factor. Obesity caused increase in heart mass due to cellular hypertrophic and metaplastic proses, independently of hemodynamic factors such as blood pressure. There were hyperleptinemia in obesity due to the selective Leptin resistency in central nervous system and peripheral organs. Study with cultured rat cardiomyocyte have shown hypertrophy and hyperplastic effect of Leptin to cardiomyocyte. Some clinical studies have shown correlation between circulating Leptin level with Left Ventricle Mass in hypertensive and insulin resistance men.Objective. This study aimed to elaborate the correlation between circu-lating Leptin level with Left Ventricle Mass in normotensive men.Methods. A cross sectional study was performed with normotensive men, which included 40 obese normotensive and 40 normoweight men. All patients underwent physical and laboratory assessment and examination of Left Ventricle with echocardiography. The circulating Leptin level were determined by ELISA method. The Leptin level were expressed as median (25th percentile; 75th percentile)Results. The circulating Leptin level were significantly different between the obese normotensive and the lean group. The Left Ventricle Mass in Obese increased, although have not fulfilled the criteria for Left Ventricle Hypertrophy. There were significant correlation between Left Ventricle Mass with BMI (r = 0,711; p<0,001) and waist circumference (r = 0,732; p<0,001). Respectively, there were significant correlation between Left Ventricle Mass Index with BMI (r = 0.541; p<0.001) and waist circumfer-ence (r = 0,558; p<0.001. There were significant correlation between circulating Leptin level with Left Ventricle Mass (r = 0,510; <0,001) and Left Ventricle Mass Index (r = 0,414; p<0,001) in obese men.Conclusion.Circulating Leptin level is correlated with Left Ventricle Mass in normotensive men.

Abstract

Background.Obesity is associated with increase arterial stiffness and el-evated plasma endothelin-1 concentration. However, there is still conflict-ing data regarding the effect of obesity on arterial stiffness and plasma endothelin-1 concentration.Objective.The purpose of the current study was to assess this effect and analyze whether there was correlation between arterial stiffness indices and plasma endothelin-1 concentration.Methods.The design of the study was cross sectional study that compare and correlate arterial stiffness and plasma endothelin-1 concentration be-tween obesity group (n=40) and normoweight group (n=40). Obesity was defined as body mass index = 25 kg/m2. Arterial stiffness was as-sessed by measuring carotid-femoral pulse wave velocity (PWV) dan ß stiffness index (ß) non-invasively using ultrasound method. Endothelin-1 was measured by ELISA.Results.There was no significant difference regarding PWV between obesity and normoweight group (mean ± SD: 809.44 ± 137.77 versus 850.96 ± 211.60 cm/s, p=NS), but ßwas significantly higher in obese group (8.79 ± 3.15 versus 7.28 ± 1.96, p=0.012). PWV and ßcorre-lated significantly with age (PWV: r=0.446, p<0.001, ß: r=0.354, p=0.001), but only ßcorrelated with body mass index (r=0.282, p=0.011) and waist circumference (r=0.312, p=0.005). There was no significant difference between obesity and normoweight group regard-ing plasma endothelin-1 concentration (0.94 ± 0.26 versus 0.95 ± 0.18 pg/dl, p=NS). There are no significant correlations between PWV/ßand plasma endothelin-1 concentration.Conclusions.These findings suggest that the effect of obesity on arterial stiffness is not uniformly seen throughout all arterial region. Obesity im-pact on carotid artery is greater than its impact on aorta, and this impact on arterial stiffness is not mediated by endothelin-1.

Abstract

Abstract

Kawasaki disease is an acute, self -limited vasculitis of unknown etiology that occurs predominantly in infants and young children. The major sequele of Kawasaki disease are related to the coronary arterial system. Cardiac imaging is a critical part in evaluation of all patients with suspected Kawasaki disease. Multi Detector CT (MDCT) provides a safe non-invasive approach to accurately delineate coronary artery anatomic structure. We report a case of a 6 years old boy with history of Kawasaki disease. Multi detector CT scan showed giant aneurysm at proximal LAD, proximal RCA and medium aneurysm at proximal LCX. Patient then treated with the anticoagulant therapy.

Abstract

Congenital heart disease (CHD) occurs in about 6 to 8 of 1000 live births with the increasing prevalence can be attributed to major improvements in diagnosis and treatment. Imaging has an important role in diagnosis of CHD. It outlines anatomy and physiology, helps to refine management, evaluates the consequences of interventions and helps guide prognosis. However, no single available imaging modality fulfills these roles for all patients and diseases. Therefore, assessment for CHD must involve multimodalities that can be used in a complementary fashion, and that together are sensitive, accurate, reproducible, and cost effective, whilst minimizing harm.In previous years, the diagnosis and the treatment of congenital malformations have often depended on cardiac catheterization. In many institutions, cardiac catheterization still remains the gold standard against which other modalities are measured. In the past decade, however, imaging methodologies have increasingly shifted toward the use of less invasive and noninvasive techniques. Although echocardiography is the most commonly used imaging modality for diagnosis and follow-up of subjects with CHD, the evolution of cardiovascular magnetic resonance (MR) imaging and increasingly computed tomography (CT) does offer new ways to visualize the heart and the great vessels.

Abstract

Background: Modified Blalock-Taussig shunt (MBTS) is considered as a simple procedure but has a considerable operative mortality rate. Patient’s characteristics who underwent MBTS in Indonesia is quite different than other country. There was no predictor of operative mortality has been identified in Indonesian. Objectives: To compare mortality rate based on age criteria and to identify mortality and morbidity predictors after MBTS procedure. Methods: A retrospectively cohort study was conducted on 400 patients who underwent MBTS at National cardiovascular center Harapan Kita (NCCHK) between January 2013 and december 2017.Results: There were 32,1% death at age ≤ 28 days, 19,9% at age 29-365 days, 3,6% at age 366-1825 days and 8% at age > 1825 days. Body weight < 3 kg, haematocrite level > 45% before procedure and activated partial thromboplastine time level (aPTT) < 60 seconds were operative mortality  predictors. Postoperative morbidity rate was 32,9%. Packed red cell  transfusion (PRC) more than 6 ml/kg, mechanical ventilator use before procedure, prostaglandin E1 use before procedure, aPTT level less than 60 seconds after procedure were identified as postoperative morbidity predictors. Conclusion: Operative mortality rate significantly different among age criteria but it was not proven as an operative mortality predictors. Body weight < 3 kg increase mortality rate and haematocrite level higher than 45% and aPTT level less than 60 seconds decrease mortality rate. Postoperative morbidity predictors were PRC transfusion more than 6ml/kg, mechanical ventilator use before procedure, prostaglandine E1 use and aPTT level less than 60 seconds.

Abstract

Background: Atrial septal defect (ASD) potentially causes pulmonary hypertension (PH) and increased pulmonary vascular resistance (PVR). In previous studies, pulmonary Doppler notch was evident in patients with high PVR. There was a distinct notch pattern in different types of PH. This study aims to examine whether echocardiographic notch pattern could estimate PVR in secundum ASD patients. Methods: Cross sectional study was conducted in secundum ASD patients ≥18 years old who underwent clinically indicated right heart catheterization. Association of notch presence and PVR index (PVRi) and correlation of notch ratio (NR), a parameter of notch location, and PVRi were analyzed. The best cut off of NR value was obtained using the receiver operating characteristics curve. Results: Among 60 patients, the notch was present in 50 patients (83%) and significantly associated with PVRi ≥6 WU.m2 (p<0.0001). Notch ratio and PVRi had a moderate negative correlation (r=-0.410, p=0.003). Receiver operating characteristics curve with area under curve 0.709 showed that cutoff NR value of 1.635 was a good discriminator for PVRi ≥6 WU.m2, with 70% sensitivity, 70% specificity, 56.8% positive predictive value, and 14.9% negative predictive value. Patients were then classified into three groups of notch pattern based on its presence and location, which was significantly associated with PVRi (p<0.0001). Conclusions: Pulmonary Doppler notch pattern is associated with PVRi in secundum ASD patients. This may be used clinically to estimate PVR to aid patient selection for further invasive investigation. Abstract

Background: Heart valve disease is still a significant health burden in the world, including Indonesia. The postoperative outcome of mitral valve surgery is influenced by many things, including decreased right ventricular (RV) function, which is the most common complication. Several studies have shown that decreased RV function after mitral valve surgery is associated with long-term outcomes. TAPSE is a routine and easy measurement of RV systolic function. A decrease in TAPSE after cardiac surgery is common because of the effects of pericardiotomy, and does not necessarily reflect a decrease in RV ejection fraction (RVEF). Regardless of whether postoperative TAPSE values indicate right ventricular systolic function or only due to the effects of pericardiotomy, it is still not clear whether postoperative TAPSE values have a prognostic value to long-term mortality after mitral valve surgery. Therefore, the objective of this study is to obtain information regarding the relationship of TAPSE echocardiographic parameters after mitral valve surgery with long-term mortality. Methods: This is a retrospective cohort study, looking at the effect of TAPSE on outcome after mitral valve surgery. The analysis starts from the starting point of the study when the patient was discharged alive from the hospital after mitral valve surgery (operation period January 2016 – February 2017) to the end point of the study, which was June 30th, 2021 and the observed outcome was mortality from any cause. Results: Of the 266 study subjects, 11 subjects died within 4-5 years after mitral valve surgery, the mortality is 4%. Bivariate analysis was performed on several factors and no relationship was found between the analyzed variables and mortality. Conclusion: There is no relationship between mortality and TAPSE after mitral valve surgery. This article has a related Erratum.

Abstract

According to the World Health Organization (WHO) in 2015, 70% of cardiac deaths were caused by coronary heart disease (CHD). Based on WHO data in 2017, 17.5 million deaths were recorded, equivalent to 30% of the total deaths in the world caused by coronary heart disease. Coronary heart disease is a disorder of heart function caused by plaque that accumulates in arterial blood vessels so that it interferes with the supply of oxygen to the heart tissue. This causes reduced blood flow to the heart muscle and oxygen deficiency occurs. In more serious circumstances, it can result in a heart attack. Risk factors for coronary heart disease include age, gender, hypertension, cholesterol, heredity, diabetes mellitus, obesity, dyslipidemia, smoking and lack of physical activity. If a person's chances of suffering from coronary heart disease can be predicted early based on the existing risk factors, then the mortality rate of coronary heart disease can be suppressed. The objective of this study is to build a model that can predict the possibility of a patient suffering from coronary heart disease. The study used the Fuzzy Logistic Regression model. This model was used to maximize prediction results in which data size was limited. The least square method was used to estimate the value of the parameter. We obtained from National Cardiovascular Center Harapan Kita, Jakarta. Evaluation with the Mean Degree of Membership method showed that the model built was feasible and good enough to predict coronary heart disease. By using the confusion matrix, the accuracy of the prediction model is 80.00%, with a specificity of 42.85% and a sensitivity of 100%.
