Abstract

ABSTRACTIntroduction: anal sphincter injury is one of the complications that often occur in vaginal delivery. The impact of anal sphincter injury after vaginal delivery can reduce quality of life by causing various complaints such fecal incontinence, voiding disorders, sexual dysfunction, perineal pain and rectovaginal fistula. Assessment of risk factors is important to watch out for patients who are more prone to injury to the anal sphincter. This study aims to obtain a scoring system of various antenatal risk factors that can be used to predict the incidence of obstetric anal sphincter injury. Methods: This study is a diagnostic test that uses predictive factors for various risk factors for anal sphincter injury in vaginal delivery. The research was carried out in maternity service at Dr. Cipto Mangunkusumo General Hospital and Fatmawati General Hospital in the period February - July 2016. Analysis of the data in this study uses SPSS version 21.0. The analysis test used in this study is the calculation of odds ratio, logistic regression test, ROC curve analysis test to assess sensitivity and specificity of the scoring system. Results: Nulliparous, estimated fetal weight ≥ 3,500 gr, perineum length ≤ 25 mm, estimated head circumference ≥33.5 were risk factors for obstetric anal sphincter injury (OR> 1; p <0.05). Through the scoring modeling system all risk factors can be converted into scores 1. The probability values of scores 1, 2, 3, 4 are 7%, 30%, 70% and 90% respectively. The sensitivity and specificity of the scoring system developed were 61.3% and 91.3% with a cut-off score of 2.Conclusion: By knowing the prediction score before delivery, action can be taken to prevent the possibility of sphincter injury in obstetrics.

Abstract

Electromagnetic stimulation (EMS) has emerged as a potential alternative for managing urinary incontinence in women. However, research directly comparing EMS to Kegel exercises in cases of postpartum stress urinary incontinence (SUI) is limited. The aim of this study was to assess the effectiveness of EMS (improvement of the symptoms, incontinence severity and pelvic floor muscle strength) and patient compliance with the therapy in postpartum women with SUI. A single-blind randomized clinical trial was conducted involving postpartum women diagnosed with SUI at least three months after delivery. The EMS group received the therapy three times a week for five weeks, while the Kegel group was instructed to perform daily exercises for eight weeks. Improvement of the symptoms and incontinence severity were evaluated using the Urogenital Distress Inventory-6 (UDI-6) and a 1-hour pad test, respectively, while pelvic floor muscle strength was measured with a perineometer. Both groups showed significant improvements in UDI-6 scores, 1-hour pad test results and pelvic floor muscle strength compared to before treatment. However, the EMS group had significantly greater muscle strength than the Kegel group (16.5 vs 8.0 cmH2O, p=0.006). The UDI-6 scores, 1-hour pad test results and patients’ compliance were not significantly different between EMS and Kegel groups. EMS demonstrated a greater ability to enhance pelvic floor muscle strength than Kegel exercises. These findings suggest that EMS may be a more effective option for enhancing pelvic floor muscle strength in postpartum women.

Abstract

Background: Cervical dysgenesis is an extremely rare type of Mullerian anomalies causing obstructive symptoms. Conservative surgery of neocervix may be related to reocclusion of the genital tract and further increase the risk of reoperation. The presence of uterine malformation may also complicate this condition and lead to challenges for normal anatomy restoration. Case presentation: We present three cases of primary amenorrhea and obstructive symptoms caused by reocclusion following cervicovaginal conservative surgery. A neovagina was created using amniotic graft, followed by the insertion of intrauterine catheter device using a syringe barrel to maintain canalization and prevent reccurent occulsion. Double approach technique (laparotomy hysterotomy and transvaginal approach) was used in the two cases and one case underwent transvaginal surgery. Three months follow-up revealed normal menstruation and rather satisfying anatomical outcome. Clinical discussion: Treatments should aim to relieve obstructive symptoms, improve function, and preserve fertility. Conservative surgical approach to manage cervical dysgenesis warrants challenges in preventing reocclusion. Surgery mainly consists of creating neocervix, neovagina if needed, and maintaining the continuity of the genital tract. Conclusion: Intrauterine catheter device using a syringe barrel is a simple and feasible option with satisfactory anatomical as well as functional outcome to manage cervical dysgenesis

Abstract

AbstractObjective: To evaluate the clinical characteristics of patients with Pelvic Organ Prolapse (POP) in national referral hospitals and analyze the relationship between age, parity, and prolapse severity. Methods: This medical record–based retrospective study included 353 POP patients from 2023–2024. Variables assessed were age, parity, prolapse severity (POP-Q), sexual activity status, and type of therapy received. Correlation analysis was performed using Spearman’s rank correlation with SPSS version 26. Results: Most patients (81.3%) were postmenopausal, and 64.6% were multiparous. The majority presented with stage IV prolapse (34.8%), and operative therapy was the primary treatment choice (89.5%). Significant associations were observed between age and prolapse severity (? = 0.208, p < 0.001) and between parity and prolapse severity (? = 0.215, p < 0.001). These findings indicate that increasing age and higher parity are significantly associated with more severe POP, although the correlation strength was weak. Conclusions: POP was most commonly found in postmenopausal women, with higher severity among older and multiparous patients. These findings highlight the importance of early screening and preventive strategies to reduce POP progression. Further prospective multicenter studies are needed to evaluate the long-term impact of POP on quality of life and to compare the effectiveness of conservative versus operative therapies. Keywords: conservative therapy, degree of prolapse, parity, Pelvic Organ Prolapse, reproductive age, surgical intervention.

Abstract

Objective: This systematic review and meta-analysis aimed to evaluate the effectiveness of laser therapy in improving urinary and sexual function in women with SUI.Methods: The literature was selected using the Preferred Reporting Items for Systematic Reviews and Meta-analyses (PRISMA) protocol. We did literature searches in several databases, including Science Direct, Pubmed, Google Scholar, Cochrane, ProQuest, and Scopus, from inception until January 2025. Quality assessment was performed using the Cochrane Risk of Bias Tool. Mean differences (MD) with 95% confidence intervals (CI) were calculated using a random-effects model.Result: There were ten journals with 598 participants that met the criteria. The results of the pooled analysis showed a statistically significant improvement in post-laser therapy pad weight (MD = -0.49; 95% CI: -0.97 to -0.01; p = 0.0435). Despite the non-significant differences and heterogeneity found in the meta-analysis of sexual function outcome (MD = -0.49; 95% CI: -0.97 to -0.01; p = 0.0435), the individual studies stated that the sexual function was improved in women with SUI post-laser therapy.Discussion: Laser therapy, particularly CO₂ and Er:YAG modalities, appears effective in enhancing sexual function in women with SUI. Variability in protocols and outcome measures among studies may contribute towards heterogeneity. Most trials had small sample sizes and short follow-ups.Conclusion: Laser therapy shows promise for improving urinary and sexual function in women with SUI, warranting further robust research. Penilaian kualitas dilakukan menggunakan Cochrane Risk of Bias Tool.

Abstract

Pelvic organ prolapse (POP) is a common pelvic floor disorder that adversely affects women’s physical, psychological, and social well-being. Globally, approximately 30% of women may experience some degree of POP, with reported prevalence in Asian countries ranging from 3% to 50%. Limited hospital-based epidemiological data are available in Indonesia. This study aimed to describe the demographic and clinical profile of POP patients at a national tertiary referral center. A retrospective descriptive study was conducted using medical records of patients diagnosed with POP at the Urogynecology Clinic of Cipto Mangunkusumo General Hospital between 2019 and 2025. Variables analyzed included age, prolapse  severity, dominant compartment, parity, urinary disorders, type 2 diabetes mellitus, and obesity. Data were presented descriptively with 95% confidence intervals (CI). A total of 384 patients were identified, with a mean age of 57.86±12.41 years (95% CI 56.61–59.10). Most patients were aged >50 years (74.0%; 95% CI 69.6–78.4). Mild POP was observed in 52.1% and severe POP in 47.9%. Based on the compartment with the highest prolapse degree, cystocele was identified in 39.3% of cases, apical prolapse in 14.3%, and rectocele in 7.6%, while 38.8% had no single predominant compartment. A history of childbirth was documented in 83.8%, with parity 1–3 being most common (49.7%). Stress urinary incontinence (27.3%) was the predominant urinary disorder. Type 2 diabetes mellitus was found in 11.7%, and obesity in 34.0% of patients with available BMI data. In conclusion, POP patients were predominantly women aged >50 years, most with a history of childbirth. Prolapse severity was nearly evenly distributed between mild and severe cases; cystocele was the most common highest-degree compartment, followed by apical prolapse and  rectocele, with a substantial proportion of patients showing multicompartment involvement. Stress urinary incontinence was the predominant urinary disorder, and obesity and type 2 diabetes mellitus were the most common metabolic comorbidities.

Abstract

Introduction: Mayer-Rokitansky-Küster-Hauser (MRKH) syndrome is a rare congenital anomaly of the female reproductive system, characterized by the absence of the uterus and upper vagina. Neovaginal reconstruction is the primary treatment to enable normal sexual function. Two commonly used techniques are the laparoscopic Davydov procedure and the use of Small Intestine Submucosa (SIS) grafts. Methods: A systematic review and meta-analysis were conducted on studies comparing these two surgical techniques in MRKH patients. Literature searches were performed through multiple international databases. Inclusion criteria involved both observational and experimental studies reporting anatomical and sexual function outcomes postoperatively. Statistical analysis was performed to compare the outcomes of both techniques. Results: The review found that both techniques are effective in achieving satisfactory anatomical and functional neovagina creation. However, differences were observed in parameters such as duration of molding use, vaginal length, and FSFI domain scores. The laparoscopic Davydov procedure showed advantages in anatomical outcomes and shorter molding duration, while SIS grafts demonstrated better scores in some subjective aspects of sexual function. Conclusion: Both the laparoscopic Davydov procedure and SIS grafts are effective in neovagina creation for MRKH patients. The Davydov technique offers superior anatomical results and reduced postoperative dilation time, whereas SIS grafts may provide improved sexual desire, arousal, and satisfaction. Further studies with larger and more diverse populations are needed to validate these findings.

Abstract

Introduction: Cervicovaginal agenesis is rare female congenital anomalies. The patients usually present with primary amenorrhoea, cyclical abdominal pain, or primary infertility. A conservative surgical approach has been proposed to alleviate obstructive symptoms, construct, and maintain the patency of the genital tract, while still preserving fertility. Case Report:  A Ms. 14 year old was referred with primary amenorrhea and cyclical abdominal pain for 5 months. Physical examination and ultrasound revealed cervical dysgenesis with proximal vaginal dygenesis. Cervical canalization using double approach (laparotomy and trasnvaginally) was done to restore the normal anatomical integrity, maintain the functional cervical canal with a theoretical advantage of protection from ascending infection, and preservation of fertility. Discussion: Obstructed menstrual blood, causing symptoms such as primary amenorrhea, could be a sign of female congenital anomalies. Hysterectomy has been the mainstay treatment for many decades, due to the risk of reoperation. Advances in surgical technique of conservative treatment marked the overall success rate up to 60% with few cases are reported to have pregnancy. Conclusions: Double approach technique could be a feasible option for conservative surgery in cervicovaginal agenesis.
