Abstract

PT Hino Motor Manufacturing Indonesia is a manufacturing company which it fields are assembly the components, assembly the bus and truck vehicles and export some spare parts. As a manufacturing company that assembly large vehicles, there are several potential hazard that can be fatal if it becomes an accident. Since 2014, Safety department of PT HMMI has set zero accident target, but it turns out that the number of work accidents that occure have an increasing trend until 2018. Department with the highest number of work accident is Vehicle Medium department because most of the assembly activities are conducted in there. The purpose of this study is to identify the causes of work accidents and potential hazards, so work accidents that have occured can be handled properly and not occure agan in the future, whereas the potential hazard can also be prevented before it becomes an accident. In order to identify the causes of work accidents and potential hazard in comprehensive way, the methods applied are Failure Mode and Effect Analysis (FMEA) and Fault Tree Analysis (FTA). The result of FMEA implementation is the priority sequence of failure mode which has the highes Risk Priority Number (RPN) value, the work accidents type are struck down, pinched and slashed. Then that three failure modes will be analyzed again using FTA so the causes of the work accident can be traced to it’s root. Based on the comparison between the causes of work accidents from this study and the result of the investigation that have been done in PT HMMI, there are several causes of work accidents that missed during the investigation and has not been handled properly. Suggestion for handling the causes of work accident are given to decreace the risk of work accidents and to seek the zero accident target. PT Hino Motors Manufacturing Indonesia (HMMI) merupakan perusahaan manufaktur  di bidang industri perakitan komponen, perakitan kendaraan bus dan truk serta ekspor suku cadang. Sebagai perusahaan manufaktur yang memproduksi kendaraan besar, terdapat beberapa potensi bahaya yang dapat berakibat fatal jika menjadi sebuah kecelakaan kerja. Sejak tahun 2014, departemen Safety PT HMMI telah menetapkan target zero accident, namun ternyata jumlah kecelakaan kerja yang terjadi mengalami tren peningkatan hingga tahun 2018. Departemen dengan jumlah kecelakaan kerja tertinggi adalah departemen Vehicle Medium karena sebagian besar kegiatan perakitan dilakukan di departemen Vehicle Medium. Tujuan dari penelitian ini adalah untuk mengidentifikasi penyebab kecelakaan kerja dan potensi bahaya, sehingga kecelakaan kerja yang sudah terjadi dapat ditangani dengan baik dan tidak terulang kembali di masa depan sedangkan potensi bahaya yang ada juga dapat dicegah sebelum menjadi sebuah kecelakaan kerja. Agar pengidentifikasian penyebab kecelakaan kerja dan potensi bahaya dapat dilakukan secara komprehensif, metode yang diterapkan adalah Failure Mode and Effect Analysis (FMEA) dan Fault Tree Analysis (FTA). Hasil dari penerapan FMEA merupakan urutan prioritas dari failure mode yang memiliki nilai Risk Priority Number (RPN) tertinggi yaitu jenis kecelakaan kerja tertimpa, terjepit, dan tersayat.  Selanjutnya ketiga failure mode tersebut kembali dianalisis menggunakan FTA sehingga penyebab kecelakaan kerja dapat ditelusuri hingga ke akarnya. Berdasarkan perbandingan dari penyebab kecelakaan hasil penelitian dengan hasil investigasi yang telah dilakukan di PT HMMI, terdapat beberapa penyebab kecelakaan kerja yang terlewatkan selama investigasi dan belum ditangani dengan baik. Usulan untuk penanganan diberikan guna mengurangi risiko terjadinya kecelakaan kerja dan mengupayakan target zero accident.

Abstract

The retail industry engaged in the pharmaceutical sector such as pharmacies sells various types of medicinal products and medical chemicals with fluctuating demand characteristics. This is due to the emergence of diseases that cannot be predicted with certainty. Inventory management is an important aspect to ensure the availability of medicines when needed while still being able to minimize losses both from retained capital on goods and potential expiration. Medicinal products themselves have a certain shelf-life, so products that have expired cannot be used again and cause losses to company. The form of inventory management policy is the order lot size, time between orders and reorder points. To determine the size of the order lot, in addition to the expiration aspect, the limitations of the warehouse as a place of storage and purchase discount factors need to be considered. The existence of a discount policy from suppliers can be used to minimize purchasing costs which are a component of inventory costs. The form of the discount policy that is generally given is the all-unit discount, but for certain types of products the policy given is an incremental discount. The purpose of this study is to build a multi-item probabilistic inventory model by considering the expiration factor, warehouse capacity constraints, and purchasing discount policies. This research will compare two discount policies, namely incremental discount and all unit discount. Based on the results of the sensitivity analysis, it is known that the model is sensitive to the parameters of unit discount provisions, good goods fraction, and holding costs.

Abstract

The characteristics considered in this study are probabilistic demand, perishable products, and warehouse constraints for multi-item inventory models. This condition occurs in several industries that consider perishable factors and warehouse constraints, namely companies that produce food, food sales agents, and retail that sell goods to end customers. The Karush-Kuhn-Tucker Condition approach was used to solve the warehouse capacity problem to find the optimum point of a constrained function. The results of the developed inventory model provide two optimal ordering times, namely ordering time-based on warehouse capacity and joint order time, and the two ordering time values ​​will be compared to determine which ordering time is optimal. In addition, the sensitivity analysis to the model was done to analyse the total inventory costs in a planning horizon, the time between goods ordering from one cycle to the next cycle, and the number of items that will expire. The parameters to be changed for the sensitivity test were warehouse constraint, a fraction of good condition goods, holding costs per unit per period, and all unit discount factors. The sensitivity analysis was done to see the behaviour of the total cost, time to order changes, and the quantity of perished products. The result of model testing and sensitivity analysis showed that total cost, based on joint order, is sensitive to the fraction of good condition products, discount, and holding cost.  The joint order was not sensitive to the warehouse capacity. In general, the model was perceived as able to describe the behaviour of the model components.

Abstract

Abstrak— Dalko Motor adalah sebuah usaha di bidang otomotif yang berlokasi di Musi Banyuasin, Sumatera Selatan. Perusahaan ini menghadapi berbagai tantangan dalam pengelolaan persediaan gudang karena belum adanya sistem pencatatan yang terintegrasi. Ketiadaan sistem ini mengakibatkan ketidaksesuaian stok, kehilangan data, serta memperlambat proses pengecekan barang. Masalah yang sering terjadi meliputi overstock, kehabisan stok, sulitnya melacak asal pemasok barang, serta waktu pelayanan yang tidak efisien. Kondisi ini berdampak negatif pada kepuasan pelanggan dan efisiensi operasional bengkel. Penelitian ini bertujuan untuk merancang dan mengembangkan sistem informasi pencatatan persediaan gudang berbasis website di Dalko Motor menggunakan metode Waterfall. Sistem ini mencakup fitur pemantauan stok secara real-time, penerapan Reorder Point, pengelolaan barang masuk dan keluar, manajemen data pemasok, serta pembuatan laporan riwayat barang. Tujuan dari penelitian ini menghasilkan rancangan sistem informasi persediaan gudang sparepart mobil pada Toko Dalko Motor dengan penerapan Reorder Point yang dapat membantu user dalam mengelola pencatatan persediaan gudang serta mengawasi pengelolaan barang yang terjadi pada Dalko Motor. Dengan implementasi sistem ini, Dalko Motor diharapkan dapat mengoptimalkan manajemen persediaan, mempercepat proses pemesanan, serta mendukung pengambilan keputusan yang lebih baik dan efisien. Abstract

PT ABC merupakan perusahaan distribusi produk pestisida dan alat penunjang pertanian yang menghadapi permasalahan dalam optimasi rute distribusi. Dengan 62 titik konsumen yang tersebar di Provinsi Lampung dan 8 unit kendaraan CDD berkapasitas 5 ton, rute distribusi saat ini ditentukan berdasarkan pembagian wilayah regional tanpa mempertimbangkan jarak tempuh dan kapasitas kendaraan secara optimal. Hal ini menyebabkan tingginya biaya transportasi, terutama biaya bahan bakar, yang sering kali melebihi anggaran. Penelitian ini bertujuan untuk merancang rute distribusi produk pestisida dengan menggunakan software OR-Tools yang mengimplementasikan model Capacitated Vehicle Routing Problem (CVRP). Metode ini memungkinkan perhitungan rute yang lebih efisien dengan mempertimbangkan kapasitas kendaraan dan jarak tempuh minimal. Hasil penelitian menunjukkan bahwa dengan menggunakan model CVRP, rute distribusi yang diusulkan dapat mengurangi jarak tempuh rata-rata hingga 47%. Selain itu, biaya bahan bakar berkurang sebesar 53%, yang berdampak langsung pada penurunan biaya operasional distribusi. Optimasi ini memberikan solusi signifikan bagi PT ABC dalam mengelola distribusi dengan lebih efisien, meningkatkan efektivitas pengiriman, dan mengurangi biaya secara keseluruhan. Abstract

PT XYZ is a manufacturing company that produces various furniture products. The company has two product categories, first category A (as standard products) with production system is make to stock the second is category B (custom products) with production system is make to order. Production time products category B is more than longer than production time product category B. Production department’s PT XYZ arise problem when face high demand. Limitations of production’s resources cause bottleneck in lines production and inability to meet demand. In addition, inaccuracies in allocating limited resources will causing descent of throughput. Optimization of limited resources needs to be done to reach the maximum throughput. This research was conducted to find out the production resources that become constraintss in fulfilling demand, the number of each type of product must be produced at the existing capacity to achieve optimal throughput and identify and analyze solutions to problems of limited production resources. The results of the study conclude that the production resources that become constraintss are man power and man hours. The maximum throughput that can be achieved by PT XYZ by optimizing the existing available resources is Rp. 819,148,490, and there are two types of products that cannot be fulfilled. Increased production capacity can be done with several alternatives, such as over time hours, out sourcing operators (freelancers), subcontracting of production, adding man power with recruitment of new operator and their combinations. The optimal throughput is Rp 1,056. 763,100 could be obtained from using alternative resource from recruiting new operator and combination with over time capacities.
