Abstract

Background: Endotracheal suctioning is often performed by nurses and beneficial for critically ill patients. Suctioning is essential for removing secretions, maintaining airway patency and prevent unexpected complications. Open suctioning is performed by disconnecting patients with the ventilator. Suctioning not only removed secretions in the airway but also oxygen. Suctioning must be done correctly, safely, effectively and efficiently to prevent unexpected events in critically ill patients. Objective: This study aimed to determine differences on cardiopulmonary parameters after open suction in critically ill patients Methods: The study design was comparatif cross sectional analytic approach, using one group pre test and post test. The sample of the research amounted to 34 people, using purposive sampling technique. Catheter size 14 Fr on ETT number 7 mm used in this research to performed endotracheal suctioning. Cardiopulmonary parameters (Heart rate/ HR, respiratory rate/RR, oxygen saturation/SpO2, systolic blood pressure/ SBP and diastolic blood pressure/DBP) measured by pulse oxymeter and bedside monitor before suction and immediately thereafter. Results: The results showed increase average heart rate 6.412 (from 106.62 into 113.03), Respiratory rate has increased 4.971 (from 20.62 into 25.59), SpO2 decreased 1.68 (from 99.09 into 97.41), and systolic blood pressure increased 5.71 (from 118.29 into 124.00) after performed open suction. The results of paired t-test statistical analysis (for RR, HR) obtained a < 0.05 (0.000), whereas Wilcoxon statistical analysis (for SpO2, SBP) obtained a < 0.05 (0.000 and 0.001). So it can be said that the difference cardiopulmonary parameters was statistically significant after perfomed open suction in critically ill patients. Keywords: Endotracheal suctioning, cardiopulmonary parameters, critically ill patients.

Abstract

Background: Oxygen administration is given to patients have oxygen supply deficit during observation due to trauma or respiratory disorder. Oxygen may be administered through a low flow method, i.e. nasal cannula. The use of non humidifier nasal cannula may cause the incidence of nasal mucosa irritation. Administering oxygen through non humidifier nasal cannula should be stopped when the oxygen therapy is more than 5 litre per minute. Objective: To investigate the correlation between the duration of non humidifier nasal cannula oxygen use and the incidence of nasal mucosal irritation at ICU in Panembahan Senopati Hospital Bantul. Method: This was an observational study which used a quantitative method, with correlational design and cross sectional approach. Samples were purposively selected comprising as many as 31 people. Data were obtained from observation sheet and analyzed with Fisher’s Exact test. Result: The use of non humidifier nasal cannula oxygen was mostly in short duration (58.1%). The majority of patients did not encounter the incidence of nasal mucosal irritation (96.8%). The result of Fisher’s Exact test showed p=0.419 >0.05. Conclusion: There was no significant correlation between the duration of non humidifier nasal cannula use and the incidence of nasal mucosal irritation in the ICU patients at PanembahanSenopatiHospital Bantul. Keywords: Duration of use, oxygen, non humidifier nasal cannula, nasal mucosal irritation

Abstract

Background: Triage is the process of determining the priority of patients’ treatment based on these verity of their condition. Triage at the Emergency Room is needed to manage the flow of patients coming to the Emergency Room through the sorting of patients according to their critical condition. Staff doing triage are those who have got certificate of training on the management of emergency patients or Basic Trauma Cardiac Life Support (BTCLS). Staff must have good knowledge in implementing triage. Objectives: To identify knowledge and skills of staff in implementing triage and strength of correlation between level of knowledge with skills of staff in implementing triage. Methods: The studi was descriptive analytic with crossectional approach. Samples were purposively selected, consisting of 20 respondents. Data were obtained through questionnaire and observation sheet and analyzed using Kendall Tau. Results: The result of the studi showed the majority of staff had good knowledge (70%) and skills (85%). The result of Kendall Tau test showed score of significance was 0.025 (sig<0.05) with correlation coefficient 0.450 which meant that strength of correlation between level of knowledge with skills in the implementation of triage was average. Conclusion: There was significant correlation between level of knowledge with skills of staff at the Emergency Room of Wates Hospital with average strength of correlation. Keywords: Knowledge, skills, triage

Abstract

Background: Patients with critical conditions in the ICU depend on a variety of tools to support their lifes. Patients’ conditions and and their unstable hemodynamic are challenges for nurses to perform mobilization. Less mobilization in critical patients can cause a variety of physical problems, one of them is cardiorespiratory function disorder. Objective: to investigate differences in heart rate (HR) and respiratory rate (RR) before, during, and immediately after early mobilization. Methods: This study employed quasi experiment with one group pre and post test design. Twenty four respondents were selected based on the criteria HR <110 / min at rest, Mean Arterial Blood Pressure between 60 to 110 mmHg, and the fraction of inspired oxygen <0.6. Early mobilization was performed to the respondents, and followed by assessments on the changes of respiratory rate and heart rate before, during, and immediately after the mobilization. Analysis of differences in this study used ANNOVA. Results: Before the early mobilization, mean RR was 22.54 and mean HR was 78.58. Immediately after the mobilization, mean RR was 23.21 and mean HR was 80.75. There was no differences in the value of RR and HR, before and immediately after the early mobilization with the p-value of 0.540 and 0.314, respectively. Conclusions: Early mobilization of critical patients is relatively safe. Nurses are expected to perform early mobilization for critical patients. However, it should be with regard to security standards and rigorous assessment of the patient's conditions. Keywords: Early mobilization, critical patients, ICU
