Abstract

Di negara miskin, sekitar 25-50% kematian wanita usia subur disebabkan hal berkaitan dengan kehamilan. Data diDinas Kesehatan Kota Metro pada tahun 2016 Cakupan kunjungan ibu hamil K4 tertinggi di Ganjar Agung danMulyojati yaitu (100%), terendah di Karang Rejo yaitu (97,6%), dan Sumber Sari Bantul berada di urutan ke tujuhyaitu (99,1%). Tujuan dari penelitian ini untukmengetahui gambaran kualitas pelayanan antenatal di wilayah kerjaPuskesmas Sumbersari Bantul Kota Metro Tahun 2016. Metode penelitian yaitu Deskriptif. Populasi pada penelitianini adalah ibu hamil diwilayah kerja Puskesmas Sumbersari Bantul Kota Metro Tahun 2016 dan sampel berjumlah180 ibu hamil. Cara ukur yang digunakan buku KIA, alat ukur berupa lembar checklist kemudian dianalisa secaraunivariat dengan distribusi frekuensi. Hasil didapatkan bahwa kunjungan antenatal K4 sesuai standar 121 ibu(67,22%), penimbangan berat badan sebanyak 180 ibu (100%) pengukuran tinggi badan sebanyak 148 ibu (82,22%),pengukuran tekanan darah sebanyak 180 ibu (100%), Pengukuran LiILA sebanyak 159 ibu (88,33%), PengukuranTFU sebanyak180 ibu (100%), skrining TT sebanyak 119 ibu (66,11%), Pemberian tablet Fe diberikan 90 tabletsebanyak 101 ibu (56,11%), pemeriksaan presentasi janin sebanyak 180 ibu (100%), pemeriksaan DJJ sebanyak 178ibu (98,89%), Pelaksanaan temu wicara sebanyak 180 ibu (100%), Pelayanan tes laboratorium rutin yang tidaklengkap sebanyak 143 ibu (79,44%), laboratorium khusus (HbsAg) tidak dilakukan sebanyak 115 ibu (63,89%)Tatalaksana kasus dengan kondisi tidak berisiko sebanyak 124 ibu (68,89%). Kesimpulan penelitian kualitaspelayanan antenatal diwilayah kerja Puskesmas Sumbersari Bantul kota Metro tahun 2016 menunjukkan bahwasebagian besar ibu hamil melakukan ANC sesuai standar, dilakukan penimbangan BB, pengukuran TB, LiLA, TFU,skrining TT, diberikan tablet Fe <90 tablet, dilakukan pemeriksaan leopold, DJJ, temu wicara, tes laboratorium rutintidak lengkap, HbsAg tidak dilakukan, tatalaksana kasus tidak beresiko. Sehingga disarankan untuk dapatmeningkatkan serta mempertahankan kualitas pelayanan antenatal, dengan mengerahkan ibu hamil untuk melakukankunjungan antenatal sesuai dengan standar minimal yaitu 4 kali selama kehamilan ditempat pelayanan kesehatan,serta mengerahkan petugas kesehatan terutama bidan dalam memberikan pelayanan sesuai dengan standar pelayananyang memenuhi komponen 10T pada setiap kunjungan ibu hamil.

Abstract

Postpartum mothers often experience changes both physiologically and psychologically, one of which is a change in body reaction due to uterine contractions. This causes discomfort in the mother, especially in the abdominal area due to uterine contractions called afterpains. Pain due to uterine contractions (afterpains) requires treatment to be minimized or suppressed as little as possible. As a midwife who provides loving care for mothers, the pain felt by the mother needs to be intervened, one of which is by providing effleurage massage. To determine the effectiveness of effleurage massage on afterpains in multiparous postpartum mothers on day 1 with normal delivery at Medirossa 2 Cibarusah Hospital.: Quasi-experimental with a pretest-posttest one group design. The sample in this study were all multiparous postpartum mothers on day 1 with normal delivery who met the inclusion criteria in November to December 2023 as many as 32 people, the sampling technique was total sampling. The research instrument used a Numeric Rating Scale (NRS) pain scale observation sheet. The statistical test used was the Wilcoxon test. There was a decrease in pain intensity after effleurage massage intervention where before effleurage massage the median value was 4 (minimum 1, maximum 7) and the pain intensity after effleurage massage the median value was 2 (minimum 0, maximum 4). From the results of the Wilcoxon test, p.value = 0.000 (α <0.05) was obtained. There is effectiveness of effleurage massage on afterpains in multiparous postpartum mothers on day 1 with normal delivery at Medirossa 2 Cibarusah Hospital in 2023. It is hoped that midwives can apply effleurage massage as one of the techniques to relieve afterpains in postpartum mothers with normal delivery.

Abstract

According to (Jakarta Provincial Health Office, 2023) the coverage of exclusive breastfeeding in DKI Jakarta province in 2022 was 50.11% and the coverage rate of exclusive breastfeeding in South Jakarta in 2022 was 50.32%, although the coverage is large, there are quite a lot of mothers who have not given exclusive breastfeeding to their children. One of the factors that causes the failure of the implementation of exclusive breastfeeding is because in the first 3 days or in the early period after delivery, breast milk has not come out so that it becomes a reason for mothers and families to give formula milk (Aprilianti, 2019). This study aims to determine the effect of a combination of oxytocin massage, lactation massage and acupressure on breast milk production in postpartum mothers aged 0-3 days at TPMB Yevi Yesevi, AMd. Keb South Jakarta in 2023. The benefits of this study can provide knowledge and insight to the community in performing massage related to breast milk production. This study uses a quantitative research method, with a pre-experimental design and one group pretest and post-test technique. The population in this study were postpartum mothers aged 0-3 days at TPMB Yevi Yesevi, AMd.Keb. This study used a non-probability sampling procedure with a purposive sampling technique with a sample size of 30 respondents. Data analysis used univariate and bivariate analysis using the Wilcoxon Test. The results of the study on postpartum maternal breast milk production 0-3 days before the combination of oxytocin massage, lactation massage and acupressure mean 0.61, median 0.35, and standard deviation 0.6502. Postpartum maternal breast milk production 0-3 days after the combination of oxytocin massage, lactation massage and acupressure mean 2.6, median 2.60 and standard deviation 1.3564. The Wilcoxon test analysis obtained a Z value of -4.579 and an asymp sig. (2-tailed) value of 0.000 smaller than the alpha level of 5% (0.05) so Ha is accepted and H0 is rejected, so it can be concluded that there is an effect of the combination of oxytocin massage, lactation massage and acupressure on breast milk production in postpartum mothers 0-3 days.
