Abstract

Pneumonia merupakan salah satu penyakit infeksi yang mengenai saluran pernapasan bawah ditandai dengan batuk dan sesak napas, hal ini diakibatkan oleh adanya agen infeksius seperti virus, bakteri, mycoplasma (fungi), dan aspirasi substansi asing yang berupa eksudat (cairan) dan konsolidasi (bercak berawan) pada paru-paru. Pneumonia merupakan salah satu penyakit menular yang memiliki tingkat kematian tinggi baik dialami kelompok lansia atau anak-anak. Untuk itu, kasus ini menarik untuk dibahas agar dapat meningkatkan pengetahuan masyarakat dan menjadi pedoman dalam pencegahan penyakit tersebut. Metode dalam penerapan kasus tersebut berupa metode wawancara, observasi. Selain itu, hal tersebut didukung pula dengan metode studi dokumentasi dan studi kepustakaan dalam membandingkan dengan literature atau jurnal terpercaya. Hasil : manifestasi yang timbul pada kasus didapatkan semua sesuai dengan literature, selain itu penentuan diagnose keperawatan juga disesuaikan dengan literature yang didapat. Namun dalam penentuan perencanaan dan tindakan keperawatan ditentukan berdasarkan kondisi pasien sehingga dapat dilakukan dokuemntasi dari setiap tindakan yang diberikan.

Abstract

The anxiety of health workers, especially nurses during a pandemic, is an important problem, especially for nurses, whose interaction intensity with patients is quite high. One of them is in the hemodialysis unit, where the average patient is an outpatient with more interaction between parties outside the hospital, so the risk of carrying the virus is quite high which will be at risk of transmitting it to the hemodialysis nurse. The average hemodialysis patient is an outpatient so that contact with outsiders is quite high which will be at risk of carrying the virus. The aim of this study analyzing spirituality with the anxiety level of nurses when caring for hemodialysis patients infected with Covid-19. The design in this study was cross sectional which was carried out on 19 hemodialysis nurses who treated Covid-19 patients with a total sampling technique. Data was collected using two instruments, namely the Spiritual Well-Being Scale (TSWBS) and the Beck Anxiety Inventory (BAI). The results of the analysis show that there is no relationship between spirituality and the anxiety level of nurses when caring for hemodialysis patients who are infected with Covid-19 with a p value of 0.340. Suggestions for further research increase the number of samples and analyze other factors that influence nurse anxiety such as family support.

Abstract

Introduction: The incidence rate of chronic kidney failure in Indonesia in 2013 was 2.0% per mil and in 2018 it increased to 3.8% per mil. Objective: The purpose of this study is to analyze the determinants of rejection of hemodialysis in Chronic Kidney Failure patients. Method: This research is a quantitative study with cross sectional approach. The population in the study were all kidney failure patients in August, amounting to 149 people. Sampling with total sampling in accordance with inclusion criteria, there were 10 patients who refused hemodialysis. Results: The results of a study with 10 patients who refused hemodialysis, 6 people (60%) covered by insurance, 5 people (50%) had kidney failure due to diabetes mellitus, 6 people (60%) patients who refused hemodialysis had poor drinking patterns, 6 people (60%) patients who reject hemodialysis have bad psychological conditions, 5 people (50%) patients who reject hemodialysis have poor family support and 6 people (60%) have poor health service support. Conclusion: The majority of hemodialysis rejection in patients with chronic kidney failure is influenced by drinking patterns, psychological conditions, family support, and poor health service support. This can be a consideration of nurses and other health teams in preparing patients and their families to make decisions according to conditions or needs of patients with chronic kidney failure. Health services can provide counseling services related to the importance of dialysis for patients with late stage chronic kidney failure, so patients choose dialysis therapy as kidney replacement therapy.

Abstract

Chronic kidney disease (CKD) can develop into end stage renal disease (ESRD), requiring hemodialysis therapy. However, this can cause complications such as intradialytic hypertension (IDH), defined as a pre-HD blood pressure >140/90 mmHg or a post-HD blood pressure > 130/80 mmHg. Therefore, the combination of PMR with mozart classical music is recommended. This study aims to analyze the effectiveness of this combined therapy on blood pressure. The research design is quasi experimental using descriptive tests, paired t tests and independent t tests. With a purposive sampling technique, a sample of 38 respondents was divided into intervention and control groups in the hemodialysis unit of RSUD Tarakan. The results showed a significant effect on SBP (p value <0.001) and DBP (p value <0.001) in the intervention group, SBP (p value <0.001) and DBP (p-value = 0.003) in the control group, and significant differences in post-dialysis SBP (p-value = 0.001) and DBP (p value <0.001) between the two groups. In the intervention group, there was a decrease in SBP and DBP, whereas in the control group, there was an increase in SBP and DBP. Therefore, this combined therapy can be a complementary treatment for CKD patients with IDH to stabilize blood pressure, especially during intradialysis.
