Abstract

The purpose of this research is to analyse factors contributing to the boarding time in emergency department Awal Bros Pekanbaru Hospital. Variables had been analyzed are triage system, payment pattern, arrival mode, amount of diagnostic examination, amount of consulting doctors, and patients per day. This is a quantitive research, retrospective, using a cross sectional design. Waiting time conceptual model or Input/Throughput/Output model was the framework used in this study, which focusing on input and throughput process. The total sampel was 374. Data collection was using the research instrument and emergency department performance report. Data analysis was using a cross-tabulation or chi-square for bivariate analysis, and using logistic regression for multivariate analysis. The results of this bivariate analysis showed that among all varaibles, factors that contributing to boarding time was triage system ( p value= 0,001), payment pattern (p value = 0,017), amount of diagnostic examination (p value=0,001) and amount of consulting doctor (p value = 0,041). Meanwhile from the multivariate analysis showed that variables that contributing the boarding time was triage system (p value = 0,023) and amount of the diagnostic examination (p value = 0,001)

Abstract

The current Covid-19 pandemic has restricted people's movements to access health services. This has resulted in reduced access to tuberculosis treatment and has resulted in increased mortality. The government has advocated using digital tools and technologies such as telemedicine for patient care. This research aims to find out how telemedicine is applied to tuberculosis services. This study uses the Systematic Literature Review methodology with scientific article publication data sources from the Google Scholar, PubMed, ScienceDirect, and ProQuest databases. From the 6440 articles obtained at the start of the search, in the end, 18 articles met this study's inclusion and exclusion criteria. Results: The implementation of telemedicine in tuberculosis services is carried out using DOT (Direct Observation Treatment) therapy using Video or vDOT (Video Direct Observation Treatment), while other telemedicine activities are carried out via direct telephone, sending voice text messages periodically for patient reminders, vDOT is carried out online synchronous via uploads or asynchronous via video recording. The telemedicine platform is helpful for health workers in tackling and preventing tuberculosis which requires intensive monitoring of patients so that patients can be monitored regularly, reducing drug withdrawal patients and increasing the success of tuberculosis treatment. There are opportunities for the development of telemedicine applications in Indonesia that focus on the use of vDOT.

Abstract

Wilayah Pasifik Barat (WPR), dengan karakteristik geografis yang menantang berupa kepulauan dan daerah terpencil, menghadapi kesenjangan akses layanan kesehatan yang signifikan. Telemedicine muncul sebagai solusi potensial, dengan akselerasi adopsi yang dipicu oleh pandemi COVID-19. Namun, implementasi yang berkelanjutan dan berkeadilan di kawasan ini masih menghadapi berbagai hambatan. Tinjauan scoping ini bertujuan untuk memetakan tren, tantangan, dan implikasi kebijakan telemedicine di negara-negara terpilih WPR dengan menganalisis 12 artikel penelitian yang relevan dari rentang tahun 2020-2025. Hasil tinjauan mengidentifikasi tren positif berupa diversifikasi model layanan (seperti internet hospital di China dan evolusi menuju virtual care di Australia) serta sikap yang umumnya positif dari tenaga kesehatan. Namun, tantangan mendasar tetap ada, meliputi kesenjangan infrastruktur digital (konektivitas internet), kerangka regulasi yang belum matang (terutama mengenai praktik dokter-pasien langsung, perlindungan data, dan pembiayaan), serta keterbatasan klinis akibat tidak adanya pemeriksaan fisik. Implikasi kebijakan yang diusulkan menekankan pada perlunya penguatan regulasi yang adaptif, investasi agresif dalam infrastruktur dan literasi digital, serta adopsi model layanan hibrida (hybrid care) yang memadukan layanan virtual dan tatap muka. Kesimpulannya, meskipun menjanjikan, telemedicine di WPR memerlukan pendekatan sistemik yang berfokus pada kesiapan sistem kesehatan, kolaborasi regional, dan kebijakan inklusif untuk mewujudkan potensinya dalam memperluas akses layanan kesehatan yang adil dan berkelanjutan.
