Abstract

Background: Patient safety is one of the most important dimensions in quality of healthcare. Nurses are the key in care delivery, their compliance toward Patient Safety policy cannot be overlooked. Studies evaluating determinants of nurse’s compliance to Patient Safety policy is still lacking. Aims: To evaluate nurse’s attitude toward Patient Safety policy using Health Belief Model (HBM). Methods: An analytical cross-sectional study was carried on in Bali Royal Hospital, Indonesia, on December 2016. An anonymous, self-administered questionnaire was used to collect the data on socio demographics, knowledge, and attitude toward Patient Safety policy. Attitudes are specifically assessed using HBM. Results: A total of 124 nurses are included in this study. Mean age of the study population was 29.7±6.7 years. Mean score for knowledge about Patient Safety policy was 12.68±3.40 from 18.0- point scale. Nurses perceived a low risk of non-compliance (mean perceived risk 8.07±1.89) and a low barrier to comply with Patient Safety policy (mean perceived barrier 8.66±1.78). Knowledge and attitudes toward Patient Safety policy did not differ by sex and educational background but it did differ according to unit of workplace. Age and length of work correlated positively to knowledge (Spearman’s r=0.439, p-value 0.000; and r=0.400, p-value <0.001, respectively). A positive and moderate correlation was observed between knowledge vs. perceived severity (r= 0.394, p-value <0.001) and knowledge vs. perceived benefit (r=0.422, p-value < 0.001). There was a strong and positive correlation between perceived severity and perceived benefit (r= 0.725, p-value <0.001). Conclusion: Nurse’s attitudes toward Patient Safety policy were generally good and correlate positively with knowledge. The use of HBM provides an increased understanding of how individual perceptions can be influenced to improve nurse’s engagement in promoting safer health care.

Abstract

Background: Patient safety is a serious issue in a healthcare organization. Building a patient safety culture is the first step in the improvement of safety. As patient safety culture is an inherent component of organizational culture; therefore, a study of organizational culture is required to develop it. This study aims to analyze the patient safety culture and identify the profile of the organizational culture in the hospital to find strategies to improve the patient safety culture.Methods: This study was used the quantitative study using a cross-sectional research method, followed by a qualitative method to get descriptive information. This research conducted in Jambi Public Hospital, Indonesia with 128 respondents. Data were obtained from a survey by the HSOPSC (Hospital Survey on Patient Safety Culture) questionnaire and OCAI (Organizational Culture Assessment Instrument. Focus Group Discussion was conducted to identify obstacles and expectations in the implementation of patient safety culture. Results: The highest positive perceptions are organizational learning and continuous improvement (90.6%) and teamwork within units (78.91%). While "staffing" (39.25) and "non-punitive responses to errors" (38.54 %) are the weakest dimension of patient safety. The most dominant type of organizational culture, followed by a hierarchy with a minimal score difference. This cultural assessment serves as a guide for changes with a quality strategy of clan culture and hierarchy culture were decided in the development of patients safety culture in this hospital.Conclusion: The patient safety culture belongs to the moderate culture category, the reporting culture dimension is the weakest. A management?s commitment and staff empowerment, as well as system development, are all needed on the development of a patient safety culture. Further research is required to investigate various factors to develop the reporting culture.

Abstract

Background: Recently medical tourism is a new form of niche tourism market, which has grown rapidly. The term of Medical Tourism describes tourists who travel to foreign countries to obtain health services and facilities such as medical, dental, surgical care, and so forth. It is necessary to hold a marketing strategy that is able to increase tourist interest in carrying out medical tourism activities in efforts to develop medical tourism. Therefore, the aim of this study was to analyze marketing strategies for medical tourism programs in hospitals. Methods: A systematic review through marketing strategy journals of medical tourism was conducted. Research articles accessed from the internet through databases namely: EBSCO, Emerald, Science direct, Neliti, and Research gate with the keywords medical tourism and medical tourism marketing strategy. The literature analysis from 2 journals with mix methods design, a journal with descriptive qualitative design, a journal with descriptive study design, a journal with SWOT analysis design, a journal with desk research methods design, and 2 journals with data analysis design. Results: The 11 articles that are considered to be in accordance with the objectives of the study. From 11 articles, then screening again based on feasibility according to inclusion and exclusion criteria obtained 8 articles for further review. This systematic review shows that the marketing concepts of segmentation, targeting, and positioning could compete competitively with other hospitals. The marketing strategy of a health service was able to increase the interest of tourists in conducting medical tourism activities in a country. Marketing efforts carried out by hospitals focus on domestic and foreign tourists. Conclusion: The existence of a marketing strategy can increase competitiveness between hospitals and increase the interest of domestic and foreign tourists to carry out medical tourism activities competitively.

Abstract

Background: Throughout the world, emergency departments (ED) are characterized by overcrowding and excessive waiting times. Furthermore, the related delays significantly increase patient mortality and make inefficient use of resources to the detriment of the satisfaction of employees and patients. Lean thinking is a philosophy that focuses on eliminating waste or non-value added elements from the processes so that customers are given greater value. Therefore, many Emergency Departments have begun to apply lean as a way to fight against the problems of crowding, delays and medical incidents.Methods: Eligible studies for this systematic review constitute result summarized that lean method implementation was capable of reducing overcrowding in Emergency Departement. The journal employed in this systematic review sourced from ProQuest, Pubmed, and Google Scholar, whereas the analyzed journals were seven journals.Results: From the total of seven journals reviewed, the studies analyzed the result after implementing the lean method in the Emergency Department. In addition, all of the studies were reported successful in implementing lean intervention. Several studies explained that lean method implementation was capable of reducing waiting time, staying length, and the proportion of patients leaving without being seen in the emergency department. One study suggested that reducing radiography transportation time can reduce turnaround time and can help improve the Emergency Department flow.Conclusion: Lean method implementation can contribute to a decrease in waiting time, stay length, and the proportion of patients leaving without being seen. According to the results of the study, the lean implementation identifies that the lean is capable of decreasing waiting times, stay length, and the proportion of patients leaving without being seen. There must be a policy that governs the lean process. Lean can inspired changes to employee roles, staffing and scheduling, communication and coordination, expertise, workspace layout, and problem-solving.

Abstract

Based on data from Indonesia Corruption Watch (ICW) in 2018, health sector corruption cases caused state losses of Rp 3.7 billion. This study aims to measure corruption knowledge and the urgency of anti-corruption education in Faculty of Public Health University of Indonesia (FKM UI). A quick survey was conducted on 198 students, in addition to interviews with experts. The results of the study showed that knowledge about corruption was still low at 64.64 %. There is full support of students (100%) of the importance of anti-corruption education and also from all experts. The support of integration of anti-corruption education with curriculum is delivered by 92, 4% respondent. The discussion, case studies and role-play became the most proposed on learning methods. The low knowledge of anti-corruption reinforces the urgency of anti-corruption education integrated with curriculum. Campus has the opportunity with the availability of expertise and support of infrastructure, in addition to the challenges of adjusting the curriculum.

Abstract

The maternal and child health book is important in conveying information related to maternal and child health, especially covering basic immunization for infants. The use of maternal and child health books is still not optimal, as evidenced by the data on the coverage of kia book ownership in east java province, around 1.7% do not have kia books. The purpose of this study was to analyze the relationship between ownership of the mch handbook which includes recording, education and communication with the use of basic immunization health services. This study used a quantitative descriptive research method, which was conducted in the period october-december 2020, using secondary data from riskesdas 2018. Data analysis included univariate by conducting frequency distribution tests and bivariate analysis with logistic regression tests. The results of the mch book ownership obtained a statistical analysis of p value = 0.245 (p> 0.05), it can be concluded that there is no significant relationship between mch book ownership and the use of basic immunization health services.

Abstract

Latar Belakang: Dalam proses pembangunan kesehatan untuk meningkatkan kualitas hidup masyarakat, Kementerian Kesehatan memutuskan untuk mengeluarkan kebijakan dana BOK (Bantuan Operasional Kesehatan). Dimana anggaran yang digunakan untuk membiayai pelayanan kesehatan masyarakat. Akan tetapi tersedianya dana BOK tersebut dirasakan belum efektif, sehingga belum menekan angka kematian ibu dan anak. Tujuan dari penelitian ini adalah mengetahui implementasi Bantuan Operasional Kesehatan (BOK) dengan cakupan penanganan komplikasi kebidanan di Puskesmas Kota Bekasi tahun 2019. Metode: Penelitian ini merupakan penelitian kualitatif. Pengambilan data dilakukan dengan wawancara mendalam dan telaah dokumen kepada informan yang terlibat pengelolaan dana BOK tahun 2019. Sampel yang terlibat dalam pelaksanaan program bantuan operasional kesehatan di Puskesmas Kota Bekasi terdiri dari sembilan responden. Hasil: penelitian mengungkapkan bahwa perencanaan anggaran sudah sesuai dengan juknis, kebijakan sumber daya manusia belum memadai, jumlah dana sudah mencukupi dan proses akuntabilitasnya memerlukan pengawasan dan penyederhanaan pelaporan. Pada tingkat capaian program penanganan komplikasi kebidanan, cakupan layanan ada yang telah meningkat, tetapi ada beberapa layanan yang tidak memenuhi target Standar Pelayanan Minimal (SPM). Saran: Diharapkan kepada Dinas Kesehatan Kota Bekasi dapat meningkatkan capaian SPM bidang kesehatan di Kota Bekasi dengan mengoptimalkan dana bantuan operasional kesehatan menuju masyarakat kota bekasi yang lebih sehat.

Abstract

Psychiatric disorders that arise due to Covid-19 include anxiety, depression, psychological trauma and the use of drugs to deal with tension caused by the pandemic. A total of 535 respondents (49%) out of 2,364 respondents had thoughts about death. Hospitals must make changes and mitigate, accelerate and adapt faster. This article aims to identify best practice strategies for suicide prevention to improve mental health services in hospitals. This study used queries: "suicide prevention", "covid" and "hospital". The inclusion criteria are English for the issue of 01/01/2020 - 05/10/2020, open access, full-text articles, all types of articles and have keywords. The exclusion criteria were the opposite of the inclusion criteria. The author obtained articles from SpringerLink (13 articles), Pubmed (17 articles), SAGE (4 articles), ProQuest (27 articles), and ScienceDirect (20 articles). Total of 6 articles is excluded because of duplication. 51 articles are excluded because they were not compatible with PICO. Ten articles were excluded because they were a systematic review. The author has 14 articles left for eligibility selection, and all articles are selected for full article review. The recommendation given is to provide comprehensive mental health services in the hospital.

Abstract

Tujuan dari penelitian ini adalah untuk mengetahui (1) implementasi program bantuan operasional kesehatan (BOK) di tingkat input di Puskesmas Kranji, (2) implementasi program bantuan operasional kesehatan (BOK) di tingkat proses di Puskesmas Kranji, dan (3) pelaksanaan program bantuan operasional kesehatan (BOK) di tingkat output di Puskesmas Kranji Penelitian ini dilakukan dengan metode kualitatif. Variabel penelitian meliputi sumber daya manusia, anggaran, pedoman, fasilitas, perencanaan, pelaksanaan, pengawasan dan penilaian. Sampel yang terlibat dalam pelaksanaan program bantuan operasional kesehatan di Puskesmas Kranji terdiri dari 5 responden. Hasil penelitian mengungkapkan bahwa pada level input, kebijakan sumber daya manusia dan buku pedoman teknis target program secara umum belum memadai, jumlah dana perlu dievaluasi dan proses akuntabilitasnya memerlukan pengawasan dan penyederhanaan pelaporan. Pada tingkat proses, implementasi perencanaan harus dilakukan berdasarkan analisis masalah. Pada tingkat output, cakupan layanan ada yang telah meningkat, tetapi ada beberapa layanan yang tidak memenuhi target SPM.Kata kunci : Bantuan operasional kesehatan (BOK); puskesmas; input; proses; output

Abstract

Di dalam penyelenggaraan Asian Games 2018 pada tanggal 18 Agustus – 2 September 2018 lalu, Rumah Sakit Olahraga Nasional (RSON) menjadi salah satu dari 28 rumah sakit rujukan dan telah memenuhi kriteria yang ditentukan oleh Olympic Council of Asia (OCA). Penelitian ini bertujuan untuk mengidentifikasi langkah persiapan rumah sakit rujukan pada kompetisi olahraga berdasarkan pengalaman RSON pada penyelenggaraan Asian Games 2018 sehingga berguna sebagai informasi di dalam penyelenggaraan kompetisi olahraga di masa mendatang. Penelitian ini menggunakan metode deskriptif kualitatif dengan pendekatan model sistem input-proses-output, yang melibatkan 6 unsur manajemen (Man, Money, Material, Machine, Method, dan Market). Penelitian dilakukan mulai dari 5 Mei 2017 – 2 September 2018 yang berlokasi di RSON dengan data primer berdasarkan observasi dan keterlibatan langsung peneliti pertama di lokasi, serta data sekunder dari regulasi internal rumah sakit, laporan keuangan rumah sakit, dan rekam medis pasien. Berdasarkan penelitian didapatkan hasil bahwa unsur kualitas dan kuantitas sumber daya manusia, ketersediaan anggaran, fasilitas ruangan pelayanan khusus, dan penetapan Surat Keputusan (SK) dan Standar Operasional Prosedur (SOP) menjadi unsur input yang penting. Selain itu, rumah sakit juga perlu mempelajari regulasi kompetisi olahraga yang diselenggarakan, jenis dan karakteristik cabang olahraga yang dipertandingkan, karakteristik setiap negara yang berpartisipasi terutama terkait penyakit endemis atau yang berpotensi menyebabkan kejadian luar biasa. Seluruh unsur ini saling bersinergis di dalam proses pemberian layanan rumah sakit rujukan. Dari segi output, RSON telah melayani 29 pasien, dengan mayoritas kunjungan berasal dari Indonesia (22 pasien), mayoritas cabang olahraga yang dirujuk adalah bola tangan (handball) (22 orang), dan diagnosis terbanyak yaitu contusion shoulder dekstra (10%) dan impingement syndrome right shoulder (10%). Adanya variasi jenis dan jumlah angka kejadian cedera atau penyakit, asal negara dan jenis cabang olahraga yang mengalami cedera ataupun penyakit yang dirujuk ke rumah sakit rujukan kompetisi olahraga dapat dipengaruhi oleh lokasi venue yang terdekat dengan rumah sakit rujukan, cabang olahraga yang bertanding di venue tersebut dan tingkat resiko cedera cabang olahraga tersebut, dan faktor lain yang mempengaruhinya.
