Abstract

The implementation of an alignment system in medical records at Puskesmas Kauman is a direct alignment system. The purpose of this study was to determine the alignment system of straight numerical filling in order to improve the quality of health services at Kauman Public Health Center, Ponorogo Regency. This type of research is descriptive. Respondents were 5 counters and medical record officers. Data collection was carried out through observation and interviews. The results showed that the alignment system was well implemented, where patients were quicker to get health services, and officers had no difficulty in finding medical record files. Tujuan penelitian ini yaitu untuk mengetahui sistem penjajaran straight numerical filling guna meningkatkan mutu pelayanan kesehatan di Puskesmas Kauman, Kabupaten Ponorogo. Jenis penelitian ini adalah deskriptif. Responden yaitu 5 orang petugas loket dan rekam medis. Pengumpulan data dilakukan melalui observasi dan wawancara. Hasil penelitian menunjukkan bahwa sistem penjajaran sudah terealisasikan dengan baik, dimana pasien lebih cepat untuk mendapatkan pelayanan kesehatan, dan petugas tidak kesulitan dalam mencari berkas rekam medis. Abstract

Patient satisfaction is the patient's feelings about the performance of the officers in health services in accordance with the level of expectations. The purpose of this study was to determine the satisfaction of BPJS patients in terms of the queue number in the outpatient registration of Darmayu Hospital Ponorogo. This research design is descriptive qualitative. The study population was 50 BPJS patients. The sample was selected by accidental sampling technique. Data were collected through filling out a questionnaire, then analyzed descriptively. Most respondents got a queue number as much as 96% (48 respondents) and respondents who did not get a queue number were 4% (2 respondents). A total of 34 respondents (68%) were satisfied with the registration service according to the queue number, as many as 32% (16 respondents) were very satisfied, while patients who were quite satisfied and unsatisfied were not there (0%). It is recommended that the queue number be given when a new patient arrives by providing a fixed flow and procedure so that it is easier for the patient to get registration services, especially regarding the queue number, as well as the continuous application of SIM-RS in registration services. Keywords: TPPRJ; patient satisfaction; BPJS ABSTRAK Kepuasan pasien merupakan perasaan pasien terhadap kinerja petugas di pelayanan kesehatan sesuai dengan tingkat harapannya. Tujuan penelitian ini adalah untuk mengetahui kepuasan pasien BPJS ditinjau dari nomor antrian di pendafataran rawat jalan RSU Darmayu Ponorogo. Desain penelitian ini adalah deskriptif kualitatif. Populasi penelitian adalah pasien BPJS sejumlah 50 responden. Sampel dipilih dengan teknik accidental sampling. Data dikumpulkan melalui pengisian kuesioner, lalu dianalisis secara deskriptif. Sebagian besar responden mendapatkan nomor antrian sebanyak 96% (48 responden) dan responden yang tidak mendapatkan nomor antrian sebanyak 4% (2 responden). Sejumlah 34 responden (68%) puas dengan pelayanan pendaftaran sesuai nomor antrian, sebanyak 32% (16 responden) adalah sangat puas, sedangkan pasien yang cukup puas dan kurang puas adalah tidak ada (0%). Disarankan agar nomor antrian diberikan pada saat pasien baru datang dengan memberikan alur dan prosedur yang tetap agar pasien menjadi lebih mudah mendapatkan pelayanan pendafataran, kususnya tentang nomor antrian, serta diterapkannya SIM-RS yang berkesinambungan di pelayanan pendaftaran. Abstract

Retention is the activity of shrinking or reducing the medical record file from the storage rack by moving the inactive medical record that is currently on the active shelf to the inactive rack. In implementing retention, Muhammadiyah Ponorogo General Hospital has SOP as a guideline. The purpose of this study was to identify a medical record retention system in terms of standard operating procedures at Muhammadiyah General Hospital Ponorogo. This type of research is descriptive. The population of this study was 5 filing officers, and all of them became members of the study sample (total sampling). Data were collected through observation, interviews and documentation studies, then analyzed descriptively. Based on the results of the study note that retention is not in accordance with the SOP. The implementation of medical record retention is carried out once every 2-3 years. Whereas the policy written in the SOP is 5 years from the time the patient was last served. Retention was carried out faster due to the lack of a place to store medical records in the active filing room. Suggestions for Muhammadiyah Hospital Ponorogo is to add a place to store active medical records and filing officers retention according to the policies that already exist in the SOP. Keywords: retention; medical records; SOUP ABSTRAK Retensi adalah kegiatan penyusutan atau pengurangan arsip rekam medis dari rak tempat penyimpanan dengan cara memindahkan rekam medis yang inaktif yang saat ini berada di rak aktif menuju ke rak inaktif. Dalam melaksanakan retensi, RSU Muhammadiyah Ponorogo memiliki SOP sebagai pedoman. Tujuan penelitian ini adalah mengidentifikasi sistem retensi rekam medis ditinjau dari standar operasional prosedur di RSU Muhammadiyah Ponorogo. Jenis penelitian ini adalah deskriptif. Populasi dari penelitian ini adalah petugas filing berjumlah 5 orang, dan seluruhnya menjadi anggota sampel penelitian (total sampling). Data dikumpulkan melalui observasi, wawancara dan studi dokumentasi, lalu dianalisis secara deskriptif. Berdasarkan hasil penelitian diketahui bahwa retensi belum sesuai dengan SOP. Pelaksanaan retensi rekam medis dilakukan 2-3 tahun sekali. Sedangkan kebijakan yang tertulis di dalam SOP adalah 5 tahun terhitung dari waktu terakhir pasien dilayani. Pelaksanaan retensi dilakukan lebih cepat karena kurangnya tempat penyimpanan rekam medis di ruang filing aktif. Saran bagi RSU Muhammadiyah Ponorogo adalah menambah tempat penyimpanan rekam medis aktif dan petugas filing melakukan retensi sesuai kebijakan yang sudah ada pada SOP. Abstract

The incomplete summary of entry-exit, then as a whole will cause the quality of medical records to be poor and followed by poor quality of the hospital, as a result of items of in-exit summary that are not completely filled out. The research objective was to determine the incompleteness and ambiguity of the DPJP in writing the main diagnoses on the entry and exit sheet in DRMRI at RSU. Muhammadiyah Ponorogo. This research approach is qualitative. The population was 600 sheets in-out DRMRI. The research variables were the incompleteness and ambiguity of the DPJP in writing the main diagnoses on the DRMRI entry and exit sheet at the Muhammadiyah Hospital Ponorogo. The results of the study were as follows: percentage of clear and complete writing was 25 sheets (29.1%), clear and incomplete writing was 19 sheets (22.1%), unclear and complete writing was 10 sheets (11.6%) , writing unclear and incomplete was 32 sheets (37.1%). The percentage of incompleteness and lack of clarity is greater than the percentage of clarity and completeness of writing the main diagnoses on the DRMRI exit sheet at Muhammadiyah Hospital, Ponorogo. Keywords: DRMRI entry-exit sheet; incompleteness and ambiguity; the main diagnosis ABSTRAK Tidak lengkapnya ringkasan masuk-keluar, maka secara keseluruhan akan menyebabkan mutu rekam medis menjadi kurang baik dan diikuti dengan mutu rumah sakit yang kurang baik pula, sebagai akibat dari item-item ringkasan masuk-keluar yang tidak terisi dengan lengkap. Tujuan penelitian adalah mengetahui ketidaklengkapan dan ketidakjelasan DPJP dalam penulisan diagnosa utama pada lembar masuk-keluar dalam DRMRI di RSU. Muhammadiyah Ponorogo. Pendekatan penelitian ini adalah kualitatif. Populasi adalah 600 lembar masuk-keluar DRMRI. Variabel penelitian adalah ketidaklengkapan dan ketidakjelasan DPJP dalam penulisan diagnosa utama pada lembar keluar masuk DRMRI di RSU Muhammadiyah Ponorogo. Hasil penelitian adalah sebagai berikut: persentase penulisan jelas dan lengkap adalah 25 lembar (29,1%), penulisan jelas dan tidak lengkap adalah 19 lembar (22,1%), penulisan tidak jelas dan lengkap adalah 10 lembar (11,6%), penulisan tidak jelas dan tidak lengkap adalah 32 lembar (37,1%). Persentase ketidaklengkapan dan ketidakjelasan lebih besar daripada persentase kejelasan dan kelengkapan penulisan diagnosa utama pada lembar keluar masuk DRMRI di RSU.Muhammadiyah Ponorogo. Abstract

The medical recording profession standard has 2 competency categories, namely core competencies and supporting competencies. This type of research is descriptive, with the aim of knowing the relationship between the performance of officers in the filling room and the competence of medical recorders. The research sample was 5 medical record officers, selected by saturated sampling technique. Data were collected through observation and interviews. The competence of medical record officers in the filling room of Muhammadiyah Hospital Ponorogo is said to be good. Of the 7 required competencies, the officers have understood and mastered 5 competencies. Meanwhile, the performance of medical record officers is categorized as good, which is indicated by a neat appearance and work results in accordance with the job description. Keywords: medical recorder; competence; performance ABSTRAK Standar profesi perekam medik memiliki 2 kategori kompetensi yaitu kompetensi pokok dan kompetensi pendukung. Jenis penelitian ini adalah deskriptif, dengan tujuan untuk mengetahui keterkaitan antara kinerja petugas di ruang filling terhadap kompetensi perekam medik. Sampel penelitian yaitu 5 orang petugas rekam medik, yang dipilih dengan teknik sampling jenuh. Data dikumpulkan melalui observasi dan wawancara. Kompetensi petugas rekam medik di ruang filling RSU Muhammadiyah Ponorogo dikatakan baik. Dari 7 kompetensi yang disyaratkan, yang sudah dipahami dan dikuasai oleh petugas adalah 5 kompetensi. Sementara itu, kinerja petugas rekam medik dikategorikan baik, yang ditunjukkan dengan penampilan yang rapi dan hasil kerja yang sesuai dengan uraian tugas. Abstract

Decentralized storage is a way of storing medical record documents by separating outpatient, emergency care and inpatient medical record documents into one folder, separate room and place. The purpose of this study was to determine the decentralized medical record document storage system in the Tugu Public Health Center, Trenggalek Regency. This type of research is descriptive. The population of this study were 15 inpatients, doctors, and outpatients. The sample was selected by means of total sampling. Data with categorical types were collected through observation and interviews, then analyzed descriptively in the form of frequency and percentage. The results showed that the decentralized storage caused discontinuity of medical record data and increased the workload of outpatient medical record officers. This storage is done because the inpatient and inpatient distances are not close together. Suggestions from researchers are to improve the storage system in order to create good medical records, add medical record staff and seminars or training on medical records. Keywords: decentralization; medical record documents ABSTRAK Penyimpanan secara desentralisasi merupakan cara penyimpanan dokumen rekam medis dengan memisahkan dokumen rekam medis rawat jalan, rawat darurat, dan rawat inap ke dalam satu folder, ruang dan tempat tersendiri. Tujuan penelitian ini adalah untuk mengetahui sistem penyimpanan dokumen rekam medis secara desentralisasi di Puskesmas Tugu Kabupaten Trenggalek. Jenis penelitian ini adalah deskriptif. Populasi penelitian ini adalah petugas rawat inap, dokter, dan petugas rawat jalan yang berjumlah 15 orang. Sampel dipilih dengan cara total sampling. Data dengan jenis kategorik dikumpulkan melalui observasi dan wawancara, lalu dianalisis secara deskriptif berupa frekuensi dan persentase. Hasil penelitian menunjukkan bahwa penyimpanan secara desentralisasi menyebabkan ketidaksinambungan data rekam medis dan meningkatkan beban kerja petugas rekam medis rawat jalan. Penyimpanan ini dilakukan karena jarak rawat inap dan rawat inap yang tidak berdekatan. Saran dari peneliti adalah memperbaiki sistem penyimpanan agar tercipta rekam medis yang baik, penambahan tenaga rekam medis dan seminar atau pelatihan tentang rekam medis. Abstract

Family support, especially the husband during pregnancy, childbirth and the baby care period as well as the recovery of the mother after giving birth can provide enthusiasm and motivation for mothers in facing the process of becoming a mother. This study aims to determine the effect of the active role of the SIAGA husband on the level of anxiety of pregnant women in facing childbirth at RSUD dr. Darsono Kab. Pacitan. The design of this study was cross-sectional. The population of this study were all normal pregnant women in the third trimester at RSUD Dr. Darsono Pacitan. The sampling technique used was accidental sampling, so that the sample size was obtained = 38 people. Data were collected through filling out questionnaires, then analyzed to test hypotheses. Based on the results of data analysis, it can be seen that the active role of SIAGA husbands is 13.9% on the level of anxiety of pregnant women in facing the delivery process. This explains that moral support from the closest person, namely the husband, is inversely proportional to the emergence of mother's anxiety in dealing with the delivery process.Keywords: husband on standby; anxiety; mother ABSTRAK Dukungan keluarga, terutama suami dalam masa kehamilan, persalinan dan masa perawatan bayi serta pemulihan ibu pasca bersalin dapat memberikan semangat dan motivasi bagi ibu dalam menghadapi proses menjadi seorang ibu. Penelitian ini bertujuan untuk mengetahui pengaruh peran aktif suami SIAGA terhadap tingkat kecemasan ibu hamil dalam menghadapi persalinan di RSUD dr. Darsono Kab. Pacitan. Rancangan penelitian ini adalah cross-sectional. Populasi penelitian ini adalah semua ibu hamil normal trimester III di RSUD Dr. Darsono Pacitan. Teknik sampling yang digunakan adalah accidental sampling, sehingga diperoleh ukuran sampel = 38 orang. Data dikumpulkan melalui pengisian kuesioner, lalu dianalisis untuk menguji hipotesis. Berdasarkan hasil analisis data dapat diketahui bahwa peran aktif suami SIAGA bernilai sebesar 13,9% terhadap tingkat kecemasan ibu hamil dalam menghadapi proses persalinan. Hal tersebut menjelaskan bahwa dukungan moril dari orang terdekat yakni suami berbanding terbalik dengan munculnya rasa cemas ibu dalam menghadapi proses persalinan.Kata kunci: suami siaga; kecemasan; ibu

Abstract

Post sectio caesarea (SC) pain is often a complaint for post SC mothers and often becomes an obstacle for the mobilization process and breastfeeding for newborns because mothers feel uncomfortable with the pain it causes. Medical efforts taken to reduce pain can be pharmacological and non-pharmacological. One of the non-pharmacological actions that help reduce pain is deep breathing relaxation techniques. This study aims to determine the effect of deep breathing relaxation techniques on reducing pain intensity in post SC mothers at Dr Darsono Hospital, Pacitan Regency. The research design used in this study was a pre and post test with a purposive sampling approach. This study used 2 variables, namely deep breathing relaxation technique as the independent variable and pain intensity as the dependent variable. Data from the research results will be analyzed using the Paired T-test statistic to determine the decrease in pain intensity before and after deep breathing relaxation techniques are performed. The results of the study showed that the majority of respondents experienced mild pain, 24 respondents (85.7%) and 4 respondents felt no pain (14.3%). Based on the results of the Paired T-test, it shows a significance value (p sign = 0.000) which means p sign <0.05. The conclusion of this study is that there is an effect of deep breathing relaxation techniques on reducing pain intensity in postoperative sectio caesarea mothers in the Flamboyan Room of RSUD dr. Darsono, Pacitan Regency.
