Abstract

Background: The prevalence of erosive esophagitis tends to increase recently. It induces higher medical expense, loss of working time, and decreases quality of life. However the study on risk factors of erosive esophagitis scarcely reported in Indonesia. This study aimed to find the association between age, sex, smoking, alcohol drinking, body mass index, hiatal hernia, the use of non steroidal anti-inflammatory drugs (NSAID), and drugs that decrease lower esophageal sphincter (LES) tone with the occurrence of erosive esophagitis in dyspeptic patients. Method: A case-control study was conducted on patients with dyspepsia who underwent upper gastrointestinal endoscopy procedure and had been interviewed to determine risk factors for erosive esophagitis in July - September 2008. The association between risk factors and the occurrence of erosive esophagitis were analyzed using Chi-square, which subsequently revealed p 0.25, this variable included in multivariate analysis. Result: There were 135 patients fulfilled criteria; 45 patients as cases and 90 patients as controls. The association was found between the occurrence of erosive esophagitis in dyspeptic patients and smoking more than 15 cigarette/day (OR 15.43; p = 0.00; CI 95% 4.77-49.88), the use of NSAID (OR 9.49; p = 0.00; CI 95% 2.77-32.53) and the consumption of drugs that decrease LES tone (OR 3.56; p = 0.02; CI 95% 1.26-10.02). Conclusion: Smoking more than 15 cigarettes/day, use of NSAID and drugs that decrease LES tone is a risk factors for the occurrence of erosive esophagitis. Abstract

Gastro-esophageal reflux disease is a pathological condition of esophagus which is caused by gastric content reflux into esophagus. There is an increased prevalence of gastro-esophageal reflux disease. The roles of esophageal pH-metry in clinical application include looking for abnormal acid exposure on esophagus with no abnormality found in endoscopy; evaluating patients following the anti-reflux surgery who are being suspected for abnormal esophageal reflux; evaluating patients with normal endoscopic result but still having refractory reflux symptoms against proton pump inhibitor medication; detecting refractory reflux in patients chest pain following the heart evaluation; evaluating patients with otolaryngologic manifestations (laryngitis, pharyngitis, chronic cough) of the esophageal reflux disease after therapeutic failure of 4-weeks proton pump inhibitor treatment; and looking for correlation between adult onset gastro-esophageal reflux disease and non-allergic asthma. Abstract

Tuberculosis (TB) is an infectious disease that is still a public health problem, and one of the biggest causes of death in Indonesia. According to BPS, the number of elderly people in 2022 is around 30 million people and in 2050 will increase by 74 million people. This demographic shift shows that the elderly are an age group that deserves attention, especially in terms of Health. The impact of the Covid 19 pandemic coupled with decreased endurance and lung capacity that is not optimal, can cause the elderly to be susceptible to tuberculosis. The team of lecturers and students of FKK UMJ seeks to prevent tuberculosis by providing special education for elderly companions or caregivers at the Tresna Werdha Budi Mulya 1 social institution in East Jakarta. The event includes pre-test, education on the prevention and diagnosis of tuberculosis in the elderly, and post-test. T test results obtained p-value of 0.000, with an increase in the average knowledge after education of 6.72. It can be stated that educational activities for caregivers provide a significant increase in knowledge.
