Abstract

Acute coronary syndrome (ACS) is a life-threatening disease which remains one of the causes of high morbidity and mortality despite the current advances in treatment. The relationship between the serum uric acid  (SUA) level and ischemic heart disease continues to be controversial and still has not been established as a cardiovascular risk factor. The cooperative interaction between the two factors has not yet fully understood. Prior epidemiological evidence of the causal relationship between the too is still argumentative. Various studies have been done using the same methods; yet, the outcomes were different. This study aimeds to conduct a meta-analysis to synthesize the results of recent studies in order to obtain accurate quantitative data. This systematic study followed the preferred reporting items for systematic reviews and meta-analysis (PRISMA) guideline and studies published in the period of  January 2010 to May 2020 were screened using the Cochrane Library, Ebsco, Medline/PubMed, ProQuest and Science Direct as the sources. Meta-analysis was conducted to synthesize the association between the SUA level and severity of coronary artery stenosis using random effect model to account for possible study heterogeneity. Heterogeneity was assessed using I2 and the meta-analysis was performed using Comprehensive Meta Analysis Version 3 (CMA3) software. Five studies (n = 601 subjects) identified a correlation between serum uric acid level and Gensini score (r = 0.548; p <0.001) in ACS patients. Heterogeneity bias was found in the analysis, whereas publication bias was not found. Thus, the severity of coronary artery stenosis in patients with ACS is positively correlated with serum uric acid level.

Abstract

(Health Science Journal of Indonesia 2019;10(2):96-102) Kata Kunci: Kanker kolorektal HT-29, ekstrak etanol daun sirsak, viabilitas sel, molecular docking, cyclin D1 Abstract Introduction: Colorectal cancer is a pathological transformation of normal colon and rectum epithelial that becomes an abnormal tissue mass, due to the overexpression of cyclin D1 protein that inducing excessive proliferation of colorectal cell. The treatment and prevention of colorectal cancer could be done naturally by consuming leaves extract of Annona muricata L. (soursop). Soursop is known for many phytochemical components that serve as an anti-cancer. Methods: This study was used HT-29 colorectal cancer cell that treated with ethanolic leaves extract of soursop and 5-Fluorourasil (5-FU) to find the cytotoxicity concentration that can inhibit 50% of HT-29 cell population (CC50) and the next concentrations of them were treated for next treatment with MTT assay. Molecular docking analysis of the compounds of ethanolic leaves extract of soursop to cyclin D1 protein used molecular operating environment (MOE) 2013.08 software. Results: CC50 of ethanolic leaves extracts of soursop was 278 μg/mL dan 5-FU was 88 μg/mL. The lowest percentage of viable HT-29 cell was 2 x CC50 after ethanolic leaves extract of soursop treatment (40,4±1,3%) was compared to 5-FU (52,8±4,3%), solvent control (97,2±1,4%), and cells control (100%). Analysis of molecular docking to cyclin D1 protein was obtained N-hexadecanoic acid and phytol molecules as good candidates to inhibit cyclin D1 protein. Conclusions: The ethanolic leaves extract of soursop could be a good alternative treatment for colorectal cancer and its compounds had ability to inhibit cyclin D1 protein (the highest gibbs free energy (ΔG) and affinity (pKi)). Abstract

Annona muricata leaves contains phytochemical substances, such as alkaloids, tannins, flavonoids, saponins, anthraquinones and cardiac glycosides, ellagic acid, triterpenoids, β- sitosterol. Polyphenols are considered to be potential in providing health benefits via modulation the gut microecology. Alteration of the gut microbiota composition can be achieved by consuming of flavonol-rich foods that exerting prebiotic-like effects. Short chain fatty acids (SCFA) that are produced from highly fermentable fibers is considered to be protective against colon cancer. Subjects were assigned consecutively into two group: ethanolic extract of A. muricata, and maltose as placebo. Ssupplementation was conducted for 8 weeks in a capsule. SCFA level assessment was conducted at baseline and the end of the study period. Gas liquid chromatography was used to determine concentration of butyrate. Fiber intake was measured using food record. Concentrations of fecal butyrate levelwas not significantly different between Fraction of A. muricata water extract that is soluble in ethanol (FAMSE) and placebo (p=0.854). Level of others SCFA were also not significantly different. The low fiberintakeisconsistent with the finding on fecal butyrate concentration, as shown by no significant increase in both groups. There is no statistically significant effect of supplementation with ethanol-soluble fraction of A muricata leaves water extract on fecal SCFA level of colo rectal cancer (CRC) patients, andin line with fiber intake below recommended daily allowance throughout the study period. Keywords: phytochemical substances, fiber intake, gut microbiota Abstrak Daun sirsak (Annona muricata) mengandung senyawa fitokimia seperti alkaloid, tanin, flavonoid, saponin, antraquinon, cardiac glycosides, ellagic acid, triterpenoid, dan β-sitosterol. Polifenol memiliki potensi yang baik dalam kesehatan dengan cara memodulasi mikro-ekologi usus. Abstract

BACKGROUND: Cancer-associated fibroblast (CAF) is the most abundant tumor stroma. Our previous study has demonstrated that the secretomes of CAF isolated from colorectal carcinoma (CRC) patients could induce epithelial-mesenchymal transition in the HT-29 CRC cell line. However, the role of CAF secretomes in CRC stemness is needed to be further investigated. Therefore, the present study aimed to investigate the effect of CAF secretomes from CRC patients on the expression of stemness markers in HT-29 CRC cells in comparison with the secretomes from normal fibroblasts.METHODS: Fibroblasts were isolated from tumor (CAF) and their counterpart non-tumor (NF) areas of three CRC patients undergone surgical resection. Normal preputium fibroblasts (PF) were isolated during circumcision of three healthy boys aged 8 years. All fibroblasts were grown in free-serum culture medium for 24 hours to collect 50% (v/v) conditioned medium (CM). Then, CM was supplemented to HT-29 CRC cells for 72 hours. The effects of CAF- and NF-CM on the mRNA expression of CD44, CD133, OCT4, and ALDH1A1 were analysed using qRT-PCR. Cells proliferation was measured using the trypan blue exclusion assay.RESULTS: Supplementation of CAF-CM (50% v/v) significantly increased CD44, CD133, OCT4, and ALDH1A1 mRNA expressions compared to that of NF-CM and control without supplementation but had no effect on the proliferation of HT-29 cells.CONCLUSION: CAF secretomes from CRC patients upregulate the expression of CRC stemness.KEYWORDS: cancer-associated fibroblasts, ALDH1A1, OCT4, CD44, CD133, colorectal carcinoma

Abstract

BACKGROUNDData collected in 2010 from Cipto Mangunkusumo Hospital indicate thatcomplicated skin and soft tissue infections accounted for more than 10% ofcases. Etiological diagnoses are based on the findings on bacterial culture andthus evaluation of the effectiveness of bacterial culture becomes a necessity. The purpose of this study was to evaluate the operational effectiveness ofbacterial culture for etiological diagnosis of complicated skin and soft tissueinfections.METHODSThis was a historical cohort study using secondary data of patients withcomplicated skin and soft tissue infections admitted for hospitalization to CiptoMangunkusumo Hospital, Jakarta from July 2011 to July 2012. The 90 subjectsmeeting the inclusion and exclusion criteria were divided into 2 groups of 45patients each. Group 1 comprised patients who received initial antibiotic therapyaccording to cultural results, while the patients in group 2 received initial antibiotictherapy without reference to cultural results. Successful diagnostic culture wasassessed by the absence of therapeutic failure. Therapeutic failure wasdetermined using 3 parameters that had to be fulfilled, viz. absence of antibioticescalation, repeat operations, and clinical deterioration. The latter parameterwas assessed by clinical judgement of the attending physician.RESULTSAfter controlling for confounding variables (age, severity of infection, comorbidity), there was no statistical difference in therapeutic success betweenculture-based and non-culture based initial antibiotic therapies (OR=0.45,p=0.085).CONCLUSIONThis study demonstrates the ineffectiveness of bacterial culture as a diagnosticcriterion for appropriate antibiotic therapy of complicated skin and soft tissueinfections.
