Abstract

Kata kunci : Infeksi virus dengue, hematokrit, trombositopenia, BPJS Abstract Dengue virus infection results in dengue fever (DF) and dengue hemorrhagic fever (DHF) diseases. The study was conducted to evaluate its distributions, as well as for medical audit in Universitas Kristen Indonesia General Hospital (RSU UKI). A retrospective descriptive study was done using medical records of patients diagnosed clinically having dengue infection. Indications for treatment and management of care were evaluated and compared to standard WHO protocols. The cost were also evaluated and compared with the coverage of the social insurance (BPJS). Dengue virus infection is mostly DF (79.5%), and mostly found in children aged 5-14 years (75.0%), with a normal nutritional status (70.5%). The majority of patients were treated at the request of the family (76.5%) and admitted with the typical symptoms of DHF. Patients with DF came on day-4 or day-5 of fever, whereas DHF patients on day-4. Decreased platelet counts <100 000 cell/mm3 was found between day-3 and day-5 of fever, while increased in platelets counts > 50 000 cell/mm3 was found between day-6 and day-7. Management of DHF was in accordance with the standard WHO protocol, but indication for discharging patient was not in accordance with WHO indication. The cost of hospitalization was in concordance with BPJS package, but the cost of the class-1 patients still need to be reviewed. It is concluded that dengue fever was the most frequent dengue virus infection in children aged 5-14 years old. Fever between day-3 and day-5 needs special attention because of a substantial decline in platelet counts. Keywords : dengue fever, hematocrit, thrombocytopenia, BPJS package

Abstract

Cataract is the most common cause of vision loss in Indonesia. It involves many risk factors, such as age, trauma, persistent intra-ocular inflammation, ultraviolet radiation, diabetes mellitus, hypoparathyroidism, prolonged corticosteroid administration, and high body mass index. However, the role of smoking and sun exposure in cataracts has received less attention despite of the high number of cigarette smokers in Indonesia and this country also has strong sunshine all year round. The aim of this study was to investigate the relationship of cigarette smoking and duration of sun exposure with senile cataracts in Jakarta, Indonesia. This was a cross-sectional study without knowing risk factor before. A total of 100 patients participated in this study selected with consecutive sampling technique. They were divided into two groups: first was the case group consisting of 50 subjects aged more than 40 years old and diagnosed with senile cataracts and the second was the control group consisting of subjects aged more than 40 years old without cataract disease. All participants had to complete the questionnaire related to the history of smoking and duration of outdoor activity per day as it related to sun exposure. The sample was taken from the eye screening program in Puskesmas Cawang, East Jakarta. This research discovered a relationship between cigarette smoking and duration of sun exposure more than seven hours per day with the incidence of senile cataract (p=0.001) Statistical tests also showed an association between smoking duration more than 20 years and incidence of senile cataract (OR 7.25, 95% CI: 1.43 - 36.6; p = 0.02). However, the results revealed that there was no relationship between the number of cigarettes smoked per day with the incidence of senile cataract (p = 0.915). Overall, cigarette smoking and its duration appear to increase the risk of senile cataract formation.

Abstract

Introduction : The incidence of early postoperative intraocular pressure (IOP) increase is reported to be 2.3–8.9% in phacoemulsification surgery. This increase typically peaks at 3–7 hours after surgery and persists during the first 24 hours. Although most eyes can tolerate the transient IOP elevation that occurs after cataract surgery, IOP spikes are potentially more dangerous if it persist. In order to minimize postoperative intraocular pressure (IOP) rise, routine medication by oral acetazolamide has been widely used among ophthalmologists. Objective of this study is to investigate the relationship between IOP changes on first day after phacoemulsification with administration of oral acetazolamide. Method : This is a descriptive study. The samples were include 50 patients diagnosed with cataract who underwent phacoemulsification surgery with intraocular lens implantatios. Samples were randomized divided into two groups. First group is patient who treated with 250 mg acetazolamide post operative and other group is untreated. All respondents must have no history of glaucoma or ocular hypertensive and attend follow up on the first day postoperative. IOP is measured with non-contact tonometer. Results : Mean age of respondents is 61  9.8 years old. Preoperative IOP mean is 18.04  3.6 mmHg and postoperative IOP mean is 18.58  6.9 mmHg. This study revealed that there is no significant relation between oral administration of acetazolamide and the lowering of intraocular pressure on first day post phacoemulsification(p = 0.2) Visual acuity pre operative and nuclear density of cataract are also no relationship with the IOP on first day postoperative (p = 0.2 ) Conclusion : The use of oral acetazolamide is not significantly related to the IOP on first day post phacoemulsification surgery.
