Abstract

Abstract

Abstract In Indonesia, diabetes mellitus continues to show a significant increase in cases each year. The development of various degenerative diseases are currently very rapidly. Degenerative diseases are usually referred to diseases that accompany the aging process.Blood sugar levels are a form of carbohydrate metabolism that serves as the main energy source is isontrolled by insulin. Diabetes occurs due to increased blood sugar levels or hyperglycemia due to insulin deficiency. The World Health Organization (WHO) states that cases of diabetes mellitus in 2015 reached 415 million cases of adults with diabetes. It is due to never stretching joint muscles such as aerobic exercise or jogging, it is necessary to do prolanis gymnastics. The purpose of this study was to identify the effect of prolanis exercise on reducing blood sugar levels in the elderly at the DolatRayat Health Center in Karo District. The type of research is a pre-experimental design with pre and post test one group design. The population of this study was 40 people. The number of samples in this study were 40 people with saturated sample techniques (total sampling). Data were analyzed by using paired sample t-test. The results showed that there was an effect of Prolanis exercises on reducing blood sugar levels in the elderly at the Dolat Rayat Health Center in Karo District, where a p-value obtained 0.000 <0.05. Therefore, it is expected that the results of the research can be additional information for Public Health Centre and related institutions to continue promoting this program.

Abstract

Rheumatism (Rheumatoid Arthritis) is a chronic of systemic inflammatory the disease, systemic inflammation that can be affect tissues and organs, especially attacking flexible (synovial) joints (WHO, 2016). Based on the American of college Rheumathology (2013) states that 52.5 million or approximately 23 % of the population of United States suffer from rheumatoid arthritis. It is estimated that at least 355 million of the world population suffer from rheumatism, which means that 1 in 6 of the world's population of the experiences rheumatic disease. The results of the survey on the European continent in 2004 showed that rheumatic disease was the most common chronic disease. Approximately 50% of Europeans aged over 50 years experience musculoskletal pain complaints (North Sumatra Province Health Service, 2010). All the types of rheumatism cause disruptive pain so that one's of the ability to move can be disrupted by rheumatic disease (Riskesdes, 2007 in Maj. Medicine Indonesia, 2009). One of the non-pharmacological interventions that nurses can do independently in reducing the scale of rheumatoid arthritis pain is by compressing warm ginger (Sentoso, 2013). The design of the reseach used Pre-Experiment using One Grop design. The design of theTest is Pre-Post. The sample of the Reseach was 13 people. the results of the test is t test statistical test the effect of ginger compresses on the decrease in pain scale is known that the value of p = 0.001 is p <0.05 thus Ho is rejected which means there is the effect of ginger warm compresses on the reduction in rheumatoid arthritis pain scale.

Abstract

ABSTRACT Background : Familial adenomatous polyposis (FAP) is a rare autosomal dominant disorder characterized by the development of hundreds to thousands of adenomatous polyps in the colon and rectum. Its incidence is estimated between 1 in 7,000 and 1 in 30,000 live births, accounting for less than 1% of all new cases of colorectal cancer. If left undiagnosed and untreated, nearly 100% of FAP cases progress to colorectal cancer, often by the age of 40-50. Case Description : A 55-year-old male presented with complaints of abdominal pain and a palpable mass in the abdominal region. Diagnostic colonoscopy revealed numerous sessile polyps, exceeding 100 in count, located primarily in the ascending colon. A total colectomy was performed. Histopathological examination of the resected specimen revealed: Macroscopic Findings: A white, endophytic mass in the ascending colon and over 100 sessile polyps. Microscopic Findings: Tubular adenomas with moderate dysplasia and a focus of well-differentiated adenocarcinoma. Seventeen mesenteric lymph nodes were examined and found free of metastatic involvement. Discussion : FAP is primarily caused by mutations in the APC gene, located on chromosome 5q22, which is essential for regulating cell growth and apoptosis. Loss of function in this gene leads to uncontrolled proliferation of epithelial cells in the colon, resulting in the formation of numerous polyps. Without intervention, this predisposes individuals to colorectal carcinoma. In this patient, the clinical, macroscopic, and microscopic findings strongly suggest that the adenocarcinoma developed secondary to FAP. However, a definitive diagnosis requires genetic confirmation of an APC gene mutation. Screening of family members is critical to identify asymptomatic carriers who may benefit from prophylactic interventions. Conclusion : The diagnosis of well-differentiated adenocarcinoma in the ascending colon in this patient is highly suggestive of progression from FAP. While clinical, macroscopic, and histological evidence supports this conclusion, genetic analysis of the APC gene mutation is essential for confirmation. Early detection and management of FAP, including regular surveillance and prophylactic colectomy, are paramount in preventing the development of colorectal carcinoma. Abstract

This research aims to analyze the influence of quality of work life and servant leadership on ASN performance, with Organizational Citizenship Behavior (OCB) as a mediating variable. This study was conducted on the State Civil Apparatus (ASN) in Tegal City, with a population of 2,500 people. The research sample was 104 ASN selected using purposive sampling technique. Data collection was carried out through questionnaires, and data analysis used the Partial Least Squares-Structural Equation Modeling (PLS-SEM) method. The research results show that the quality of work life and servant leadership have a positive and significant effect on OCB. Furthermore, OCB has a positive and significant effect on ASN performance. In addition, OCB has been proven to mediate the influence of quality of work life and servant leadership on ASN performance. These findings indicate that improving the quality of work life and implementing servant leadership can increase ASN extra-role behavior, which ultimately contributes to increased performance. The implications of this research highlight the importance of improving the work environment and a service-oriented leadership style to improve ASN performance through strengthening OCB. These results can be a basis for policy makers in designing strategies to increase the effectiveness and productivity of ASN in Tegal City.
