Abstract

ABSTRACT Background : Familial adenomatous polyposis (FAP) is a rare autosomal dominant disorder characterized by the development of hundreds to thousands of adenomatous polyps in the colon and rectum. Its incidence is estimated between 1 in 7,000 and 1 in 30,000 live births, accounting for less than 1% of all new cases of colorectal cancer. If left undiagnosed and untreated, nearly 100% of FAP cases progress to colorectal cancer, often by the age of 40-50. Case Description : A 55-year-old male presented with complaints of abdominal pain and a palpable mass in the abdominal region. Diagnostic colonoscopy revealed numerous sessile polyps, exceeding 100 in count, located primarily in the ascending colon. A total colectomy was performed. Histopathological examination of the resected specimen revealed: Macroscopic Findings: A white, endophytic mass in the ascending colon and over 100 sessile polyps. Microscopic Findings: Tubular adenomas with moderate dysplasia and a focus of well-differentiated adenocarcinoma. Seventeen mesenteric lymph nodes were examined and found free of metastatic involvement. Discussion : FAP is primarily caused by mutations in the APC gene, located on chromosome 5q22, which is essential for regulating cell growth and apoptosis. Loss of function in this gene leads to uncontrolled proliferation of epithelial cells in the colon, resulting in the formation of numerous polyps. Without intervention, this predisposes individuals to colorectal carcinoma. In this patient, the clinical, macroscopic, and microscopic findings strongly suggest that the adenocarcinoma developed secondary to FAP. However, a definitive diagnosis requires genetic confirmation of an APC gene mutation. Screening of family members is critical to identify asymptomatic carriers who may benefit from prophylactic interventions. Conclusion : The diagnosis of well-differentiated adenocarcinoma in the ascending colon in this patient is highly suggestive of progression from FAP. While clinical, macroscopic, and histological evidence supports this conclusion, genetic analysis of the APC gene mutation is essential for confirmation. Early detection and management of FAP, including regular surveillance and prophylactic colectomy, are paramount in preventing the development of colorectal carcinoma. Abstract

Ideologi Pancasila suatu dasar negara yang mengakui dan mengagungkan keberadaan agama dalam pemerintahan. Agar warga negara Indonesia kita tidak meragukan kesinambungan ideologi Pancasila dalam kaitannya dengan agama. Tidak perlu mencoba mengganti ideologi pancasila dengan ideologi berbasis agama dengan alasan bahwa ideologi pancasila bukanlah ideologi agama. Ideologi pancasila adalah ideologi agama. Sebagai umat yang beragama kita harus saling membantu. Tidak perlu mentolerir kebencian atau diskriminasi terhadap orang yang berbeda agama, berbeda keyakinan, atau berbeda adat. Pengamalan butir-butir Pancasila, khususnya sila ke-1 semakin berkembang. Salah satunya adalah sikap saling menghormati umat beragama, yang menjadikan negara ini negara Pancasila yang nyaman bagi rakyat. Apalagi ketika jaminan keamanan dan kesejahteraan setiap masyarakat meningkat. Terutama jaminan keamanan untuk layanan keagamaan.

Abstract

Indonesia has adopted a hybrid carbon pricing strategy, combining a postponed carbon tax with operational instruments such as the Emissions Trading System (ETS) and the IDXCarbon exchange. This study examines whether these early pricing signals have already shaped firm-level investment behavior before the formal enforcement of the carbon tax. The methodology incorporates firm level variables such as ETS exposure, carbon intensity, and export dependence on the EU Carbon Border Adjustment Mechanism (CBAM), while controlling for financial indicators like profitability, size, and leverage. Key results indicate that firms with higher exposure to ETS and CBAM related markets significantly increased their environmental CapEx during the observed period. Event analysis reveals marked investment shifts following the launch of IDXCarbon and the government's carbon tax announcements. These findings suggest that Indonesian firms are responsive to carbon pricing signals even before full regulatory enforcement, especially when those signals are perceived as credible and market relevant. However, policy delays and uncertainty have moderated the pace and scale of investment responses. The discussion highlights the importance of internal carbon pricing, sectoral heterogeneity, governance dynamics, and comparisons with early phase ETS experiences in the EU, China, and Korea. The study concludes that clear and credible carbon pricing frameworks are essential for mobilizing private sector investment in sustainability. Indonesia must enhance the regulatory predictability and institutional strength of its carbon pricing system to fully realize its climate and investment objectives.
