Abstract

BACKGROUND: Posterior capsule opacification (PCO) often occurs after cataract surgery. Metformin has been known to have an ability to inhibit fibrosis. This study aimed to investigate the effects of metformin on cell contractility, collagen deposition and degradation in human lens epithelial cells (HLEC) of cataract patients.METHODS: HLEC were isolated from the anterior lens capsule of patients undergoing cataract surgery. The HLEC culture was carried out using explant culture technique. The in vitro PCO model was created by scratching technique on HLEC cultures. The treatment groups were given 0.1, 0.5 and 1 mM metformin, respectively, while the control group were given 10% fetal bovine serum (FBS). On the 7th day after scratching technique, the collagen deposition, collagen degradation and cell contractility were evaluated.RESULTS: Collagen deposition in HLEC was significantly reduced after given 0.1 mM, 0.5 mM and 1 mM metformin (17.92±6.16 mg/mL, 12.92±4.31 mg/mL, 11.25±5.30 mg/mL, respectively), compared to the control group (31.46±7.52 μg/mL, p=0.002). Collagen degradation significantly was increased in the 0.1 mM, 0.5 mM and 1 mM metformin groups (4.77±9.27 mg/mL, 6.59±1.16 mg/mL, 6.35±1.90 mg/mL, respectively) compared to the control group (2.21±2.78 μg/mL, p=0.002). While, collagen contractility in HLEC was significantly reduced in 0.1mM, 0.5mM and 1 mM metformin groups (16.39±3.89%, 13.89±2.59%, 11.93±2.44%, respectively), compared to the control group (44.25±4.95%, p=0.000).CONCLUSION: Metformin reduced collagen deposition and contractility, but increased collagen degradation in HLEC of cataract patients through mechanism of extracellular matrix remodeling.KEYWORDS: metformin, human lens epithelial cell, fibrosis

Abstract

Introduction: One of the leading causes of infant blindness is cataract. The prevalence of congenital cataract has been estimated in the range between 1 and 15 per 10.000 children globally. Congenital cataract in twin babies is a rare case. This case will report the outcome of comprehensive management of congenital cataract in dizygotic twin babies. Case presentation: Eight months old dizygotic twin babies came to our clinic with the primary complaint of whitish appearance in the pupil of both eyes since birth. They were born aterm with cesarean delivery. The birth weight was 2.700 g and 3.100 g respectively. They were the fourth and fifth children in the family. There was no family history of congenital cataract. The visual acuity of both eyes was positive response to light stimuli. The cataract was bilateral and dense on both babies. Fundus reflex and TORCH examinations were negative. Ultrasonography (USG) of the retina was normal on both babies. Conclusions: Isolated congenital cataract in twin babies is an unusual condition that should be treated comprehensively. Congenital cataract may cause deprivation amblyopia, refractive amblyopia, and permanent vision impairment. Early diagnosis and quick treatment, such as surgical timing and visual rehabilitation, are critical to perform successful management. Comprehensive care is required to monitor the visual result of cataract surgery.

Abstract

Highlights:1. Combination of medication and surgery on glaucoma had the least post-therapy intraocular pressure.2. There was no significant difference in the efficacy of drugs, surgeries, and combination of both therapies for glaucoma intraocular pressure reduction. AbstractIntroduction: Glaucoma leads to blindness in its end stage. Quality of life drops as visual field loss as progressive optic nerve damage since high intraocular pressure elevation. Glaucoma treatment focused on aqueous humor regulation, thus the intraocular pressure is maintained at normal.Methods: This was a retrospective analytical study using secondary data gathered from medical records as total sampling data. The sample was determined by the purposive non-probability technique. The mean of intraocular pressure after therapies was defined as the dependent variable while the independent variable was the type of therapy.Results: Combined therapy (16.00 ± 6.77 mmHg) had the least post-therapy intraocular pressure. The statistical test indicated no significant difference in the effectiveness of medication, surgery, and combination therapy for glaucoma intraocular pressure reduction.Conclusion: There was no prominent difference in the efficacy of drugs, surgeries, and combined therapies for glaucoma intraocular pressure reduction.

Abstract

Diabetes melitus tipe 2 adalah penyakit akibat penurunan produksi insulin, penurunan kerja insulin atau akibat kedua kondisi di atas yang terjadi secara bersamaan. Ciri khas diabetes melitus tipe 2 adalah hiperglikemia yang lambat laun akan menjadi masalah serius untuk berbagai sistem organ salah satunya mata (WHO,2021). Katarak merupakan salah satu bentuk komplikasi diabetes pada mata yang dapat terjadi dalam beberapa derajat sesuai kekeruhannya yaitu derajat 1, derajat 2, derajat 3, derajat 4, dan derajat 5 (Buratto, Brint, and Sacchi, 2014). Penelitian ini bertujuan untuk mengetahui hubungan antara derajat katarak dan durasi diabetes melitus tipe 2. Penelitian ini merupakan penelitian observasional analitik dengan desain cross sectional menggunakan data sekunder berupa rekam medis pasien di instalasi rawat jalan mata di RSUD Dr. Soetomo Surabaya periode Januari 2019 - Desember 2021. Sampel berjumlah 29 sampel sesuai dengan kriteria inklusi dan eksklusi yang diambil dengan teknik total sampling. Analisis data dilakukan dengan aplikasi pengolahan data SPSS dengan uji kolerasi Rank Spearman. Pada penelitian ini diketahui sebanyak 17 mata dari responden yang memiliki durasi diabetes melitus tipe 2 ?5 tahun mengalami katarak derajat 5 (29,3%) dan diantara 15 mata responden yang memiliki diabetes melitus tipe 2 <5 tahun, 6 diantaranya responden (10,3%) mengalami katarak derajat 1 dengan p value=0,385 dan koefisien korelasi=0,116. Terdapat korelasi yang sangat lemah antara derajat keparahan katarak dengan durasi diabetes melitus tipe 2.
