Abstract

Latar Belakang : Heart Failure With Preserved Ejection Fraction  (HFpEF) masih menjadi masalah utama penyebab morbiditas dan mortalitas cukup tinggi yang hingga saat ini belum adanya pengobatan yang optimal. Ciri khas dari HFpEF adalah terdapat penurunan bioenergetika miokardium yang dapat mempengaruhi kualitas hidup pasien gagal jantung. Pemberian koenzim Q10 dapat memperbaiki bionergetika miokaridum. Namun belum ada penelitian yang menilai kualitas hidup pasien HFpEF yang telah diberikan koenzim Q10. Tujuan  : Membuktikan apakah koenzim Q10 dapat memperbaiki kualitas hidup HFpEF Metode : Randomisasi dilakukan pada 25 pasien HFpEF pada penelitian acak terkontrol, unblinded, pre-post test control group design. Pasien menerima koenzim Q10 100 mg tiga kali sehari (kelompok koenzim Q10) atau tanpa  koenzim Q10 (kontrol)  selama 30 hari. Kemudian menilai kualitas hidup pasien HFpEF sebelum dan sesudah perlakuan dengan Minnesota Living Heart Failure Question (MLHFQ) dengan mengukur dimensi psikis, fisik dan total skor. Hasil : Dua puluh lima pasien menyelesaikan penelitian ini tanpa menimbulkan efek samping. Pada akhir penelitian didapatkan perbedaan yang bermakna pada selisih perbandingan dimensi fisik sebelum dan sesudah perlakuan tiap kelompok (p=0,015), sedangkan  pada dimensi fisik terdapat perbedaan yang tidak bermakna (p=0,153) sama halnya dengan total skor MLHFQ terdapat perbedaan yang tidak bermakna (p=0,235). Kesimpulan : Suplementasi koenzim Q10 memberikan perbaikan yang berpengaruh pada dimensi psikis, akan tetapi tidak memberikan pengaruh terhadap kesuluruhan skor total MLHFQKata kunci : HFpEF, koenzim Q10, MLHFQ

Abstract

Latar belakang: Kondisi hipervolemia pada pasien gagal jantung merupakan situasi dimana seorang pasien harus mendapat penanganan darurat. Tekanan vena jugularis (Jugular Venous Pressure “JVP” adalah pengukuran tidak langsung dari tekanan vena kava. Vena kava menentukan gambaran dari kondisi atrium kanan pada jantung. Meningkatnya JVP diakibatkan adanya kegagalan jantung dalam memompa darah ke dalam sirkulasi. Sehingga, pemantauan JVP sebagai prediktor kondisi jantung pada pasien merupakan komponen penting dalam pengelolaan dan perawatan pada pasien gagal jantung.Metode: Metodologi yang digunakan dalam penulisan ini adalah konsep analisis. Konsep ini bertujuan untuk melakukan analisa terhadap konsep pemeriksaan JVP berdasarkan strategi analisis konsep Walker dan Avant, meliputi indentifikasi antesenden, mendefinisikan atribut, konsekuensi, referensi empiris, dan kasus yang terkait dengan konsep tersebut.Hasil: Model konseptual pemeriksaan JVP memberikan petunjuk dalam penerapan asuhan keperawatan sehingga memberikan mutu asuhan keperawatan yang berkualitas. Dampaknya, kualitas hidup pasien dengan masalah pada sistem kardiovaskuler semakin meningkat dan terhindar dari perburukan kondisi.Kesimpulan: Penguasaan terhadap konsep pemeriksaan JVP sangat diperlukan oleh seluruh tenaga kesehatan. Konsep menjadi dasar dalam menerapkan suatu teori. Penerapan pemeriksaan JVP berdasarkan konsepnya, akan memberikan hasil pemeriksaan yang akurat, sehingga dapat dijadikan sebagai pedoman dalam penegakan diagnosa dan penatalaksanaan pada pasien. Keyword : Gagal Jantung, Konsep Analisis, Pemeriksaan Fisik, Tekanan Vena Jugularis

Abstract

Background: Atrial fibrillation is one of the factors forming thromboembolism. The thromboembolism can be prevented with warfarin. A side effect of warfarin is increasing the occurrence of bleeding, these therapy requires an evaluation of its usage. These evaluations can be seen in the Time in Therapeutic Range (TTR) patients. TTR is the duration in which the International Normalized Ratio (INR) patients in referral value are 2.0-3.0. TTR > 70% can prevent bleeding on FA patients with warfarin therapy.Aim: To determine TTR in patients receiving warfarin with atrial fibrillation.Methods: This study was descriptive with a cross-sectional design for six months at Dr. Kariadi Hospital Semarang. TTR was calculated using Rosendaal's Methods after collecting three INRs and patients' demographics.Results: Total subject was 111 patients with an average age of 52,63 and the TTR average is 53,75. TTR in this study was divided into two groups, that groups were poor TTR (TTR < 70%) with 82 patients (73.88%) and good TTR (≥70%) with 29 patients (26.12%).Conclusions: The average TTR in FA patients with warfarin therapy was 53,75%.

Abstract

Background: Furosemide as a diuretic loop is one of the main therapies given to heart failure patients with congestion, but in its use furosemide requires a lot of consideration related to toxicity, side effects, and drug interactions.Objective: To determine the toxicity, side effects and drug interactions of furosemide on heart failure patients.Methods: This research is a Systematic Review. Samples were obtained by searching for journals in the online databases of Pubmed, Scopus and Springer Link then adjusted to inclusion criteria and research questions. Data analysis was based on the PRISMA checklist, then searched for similarities and differences.Results: This study found the side effects of furosemide therapy in heart failure patients were: electrolyte disturbance such as: hyponatremia; hypokalemia; and hypomagnesemia, arrythmia, worsening renal function and worsening of AKI, hypotension, increase of plasma renin, and increased risk of fractures. No incidence of furosemid toxicity. The interaction of furosemide on heart failure patients may occur in several drugs like aspirin, digoxin, ACE-inhibitor, and bronchodilator.Conclusion: There is some side effect of furosemide and drug interactions occured in therapy of heart failure patients.Keywords: Furosemide, Toxicity, Side Effects, Interactions, Heart Failure.

Abstract

Background: There is an increasing global prevalence of atrial fibrillation (AF), including in Indonesia. Based on the presence of mitral stenosis and/or prosthetic valve, AF is divided into two groups, namely valvular AF and non-valvular AF. The differences in clinical characteristics between valvular and non-valvular AF in Indonesia, especially in Semarang, have not been described. This study aimed to determine the differences in clinical characteristics between valvular and non-valvular AF in Semarang. Methods: This study was an observational analytic study with a cross-sectional design and was performed in July-August 2020. The subjects were 54 AF patients, which were divided into two categories, namely valvular (n=28) and non-valvular (n=26) AF. The data were collected from medical records. The differences between the two groups were analyzed with bivariate analysis. Results: The patients with valvular AF were predominantly female (82.1% vs 69.2%; p <0.001), of a younger age (46.54 ± 12.20 vs 61.04 ± 8.68; p <0.001), more likely to have rheumatic heart disease (46.4% vs 3.8%; p <0.001), and have had heart surgery (57.1% vs 0%; p <0.001). The patients with non-valvular AF were more likely to have hypertension (17.9% vs 80.8%; p <0.001), myocardial infarction (0% vs 19.2%; p = 0.021), dyslipidemia (7.1% vs. 42.3%; p = 0.003), and higher BMI (21.03 ± 4.16 vs 25.48 ± 4.20; p <0.001). The INR values in most of the AF patients have not reached the target. The valvular AF patients were more likely to be taking warfarin (100% vs 80,8%, p=0,021) and diuretic therapy (96,4% vs 57,7%, p=0,001). Conclusion: There are significant demographic and clinical characteristics differences between valvular and non-valvular AF.

Abstract

Background: Microvascular injury after primary percutaneous coronary intervention (PPCI) reperfusion contributes to necrosis propagation. Corrected TIMI Frame Count (CTFC) is a surrogate marker of microvascular dysfunction and can stratify in-hospital mortality risk in patients with final TIMI flow 3. The extent of myocardial fibrosis after STEMI is associated with a higher incidence of major cardiovascular events. This study aimed to determine the relationship between CTFC in the infarct-related artery and myocardial fibrosis area based on cardiac magnetic resonance (CMR) in STEMI patients undergoing PPCI.Methods: This retrospective cohort study included 31 STEMI patients who had undergone PPCI and CMR examination between days 60 and 75 after STEMI as the sample. CTFC was measured in the infarct-related artery from post-PPCI angiogram recordings. Myocardial fibrosis area was measured from late gadolinium enhancement CMR (LGE-CMR) imaging results.Results: In this study, the mean age was 51.61±10.49 years, 90.3% were male, non-anterior infarction location was 58.1%, mean total ischemic time was 489.48±228.33 minutes, mean CTFC was 27.4±9.3 frames, and mean myocardial fibrosis was 18.33±7.87%. There was no significant correlation found between CTFC and myocardial fibrosis (p=0.530), however total ischemic time had a positive and significant correlation with myocardial fibrosis (p=0.025, r=0.403).Conclusion: CTFC in the infarct-related artery is not correlated with myocardial fibrosis area in STEMI patients undergoing PCI.

Abstract

Nursing services worldwide today strive to incorporate a holistic approach, where individuals are regarded as complete beings encompassing biological, psychological, social, cultural, and spiritual dimensions. Within the context of intensive care units (ICUs), spiritual care is recognized as a fundamental element that can foster hope, provide a sense of life's purpose, and alleviate anxiety in patients. This research aimed to gain insight into the provision of spiritual care for ICU patients and utilized a narrative review methodology. Searches were conducted in clinical databases such as Scopus and ProQuest, using keywords like "intensive care unit AND patients AND psychiatric care AND spirituality." The selected articles adhered to inclusion criteria, pertained to the subject matter, and were published within the last seven years (2016-2022). In total, 11 articles were included as primary references in this study. The findings of this research reveal that ICUs offer three categories of spiritual services to patients: tailored spiritual services that address individual needs, spiritual support provided by the patient's family, and spiritual support delivered by healthcare professionals. This study underscores the multiple facets of spiritual care that benefit ICU patients, encompassing the recognition and response to their spiritual needs, the assistance of family members, and the support provided by the medical team. All of these components contribute positively to the recovery of ICU patients and meet their spiritual requirements.
