Abstract

Background: Migraine and tension type headache (TTH) are the primary headaches that most complained and can lead to disability significantly. Obesity is already known to be one of the headache chronification risk factors, and adiponectin is a cytokine that is secreted by adipocytes tissue primarily. Adiponectin has been known to play a role in modulated some inflammatory pathways of the primary headache pathophysiology or in central sensitization process in conjunction with chronic headache. Objective: To evaluate association level of adiponectin with migraine and TTH.Methods: This cross-sectional study was conducted in migraine and TTH patients who came to sefalgia outpatient department of Adam Malik Hospital, which sex-and age matched for this study sample. Each subject completed a questionnaire adapted from the research Ho KH and BK Ong-C, and do anthropometric examination also adiponectin examination at Prodia laboratory. Result: There is significantly difference of mean adiponectin level between migraine 7.58 (SD 4.69) and TTH 4.99 (SD 2.63) (P = 0.008). Association of adiponectin level with episodic migraine is found not significant (P = 0.35), but the significant association is found with TTH (P = 0.013), which adiponectin level is found higher in chronic TTH 8.5 (SD 1.39) than episodic TTH 4.64 (SD 2.47). From the Spearman correlation test, there is negative association between WHR with adiponectin level significantly (r = -0.28. P = 0.02).Conclusion: There is association between adiponectin level with TTH, but it’s not significant with episodic migraine, and there is negative association between WHR and adiponectin level. Keywords : migraine; tension type headache; adiponectin level; obesity; waist hip ratio

Abstract

Background : Stroke constitutes a national health problem with many risk factor. Leptin, produced by adipocyte was shown to be important risk factor for stroke. The circulating lipids and lipoproteins plays an important etiologic role in stroke. Waist to hip ratio (WHR) has been shown to be predictor of stroke. The purpose of this study is to investigate the correlation between plasma leptin, plasma lipid and WHR with stroke. Methods : A cross-sectional study performed in stroke and non stroke matched patients in Adam Malik General Hospital from March until June 2012. Concentration of plasma leptin, plasma lipid and WHR was examined and then performed a statistical analyzed. Results : From 62 subjects with stroke and non stroke have a significant correlation between plasma leptin with stroke in female group (p=0.036), but not in male (p=0.454), a significant correlation between HDL (p=0.000) and triglyceride (p=0.000) with stroke, no significant correlation between WHR and stroke in male (p=0.776), significant correlation between waist circumferences (p=0.005) and hip circumferences (p=0.002) with stroke in male. There was moderate correlation between plasma leptin and total cholesterol in stroke (r=0.413), weak correlation between plasma leptin and LDL in stroke (r=0.392), moderate correlation between plasma leptin and waist circumferences in stroke (r=0.505), strong correlation between plasma leptin and hip circumferences in stroke (r=0.624). No significant correlation between plasma leptin (p= 0.937), plasma lipid and WHR in stroke Conclusion : There was significant correlation between plasma leptin and stroke in female group, significant correlation between HDL and triglyceride with stroke, significant correlation between total cholesterol and LDL with plasma leptin in stroke. Keywords : stroke; plasma leptin; plasma lipid; WHR.

Abstract

Background and Purpose : Body mass index and waist circumference are the simple anthropometric measurement which can be used to assess nutritional state. Body mass index and waist circumference has been related with several conditions including headache and quality of sleep. The aim of this study was to assess the association between body mass index and waist circumference with headache and quality of sleepMethods : This cross sectional study had subjects which divided into two groups : headache group from Cephalgia Clinic of Neurology Department Haji Adam Malik Hospital and non headache group by matching age and gender from General Clinic of Neurology Department Haji Adam Malik Hospital. Every subject experienced the measurement of body weight, height to assess body mass index and measurement of waist circumference. A questionnaire about headache adapted from study HO K-H & Ong BK-C was given to subject from headache group. Pittsburgh Sleep Quality Index was given to subject from headache group and non headache group.Result : From 108 subjects consist of 54 subjects from headache group and 54 subjects from non headache group, there were 10 men (18.5%) and 44 women (81.5%) in each group. From 54 subjects of headache group there were 22 subjects (40,7%) with migren and 32 subjects (59.3%) with tension type headache. There were 16 subjects had good sleep quality and 38 subjects had poor sleep quality in headacahe group. There were 28 subjects had good sleep quality and 26 subjects had poor sleep quality in non headache group. There was no significant association between body mass index (p= 1.000) and waist circumference (p= 0.241) with headache. There was a significant association between body mass index (p= 0.041) and waist circumference (p= 0.023) with quality of sleep. Conclusion : There was no significant association between body mass index and waist circumference with headache. There was a significant association between body mass index and waist circumference with quality of sleepKey words : body mass index; waist circumference; headache; quality of sleep.
