<![CDATA[Unsafe behavior is a human error in taking attitudes and actions. Factors that influence unsafe behavior include predisposing factors, namely knowledge, attitudes, and motivation. Enabling Factors are Availability of PPE and Safety Regulations. Reinforcing Factors are Supervision and Colleagues. Based on the results of observations in September in the field conducted on Formwork Workers of PT Beton Konstruksi Wijaksana, it was found that some unsafe actions were carried out by employees, such as not using PPE properly such as not using a body harness when working at heights and not using PPE. hand protection when using cutting machines, lifting weights in the wrong method chatting and joking while working at work. This study aims to determine the relationship between knowledge and supervision with unsafe behavior in Formwork Workers of PT Beton Konstruksi Wijaksana in 2020. The research method used is quantitative with cross sectional research techniques. To obtain data for 46 respondents, it was done by filling out a questionnaire through google forms with a total sampling technique. The results of the Chi Square test showed that there was a relationship between knowledge and supervision with unsafe behavior among workers in the Formwork Operations Department of PT Beton Konstruksi Wijaksana (P value 0.000).
